  • The Prevalence of Alcohol Prevention Efforts in Swedish Workplaces

    • This article presents results from a survey aimed at identifying the extent of alcohol prevention efforts in Swedish workplaces focusing on the dissemination of a model for secondary prevention — the 'Risk Drinking model'. From a random sample of 929 human resource managers at Swedish workplaces, 374 answered a web-based questionnaire (response rate = 40%). Results showed that about 70% had an alcohol policy at their workplace, 51% were engaged in alcohol prevention efforts and about 19% used the Risk Drinking model. Regression analyses showed that large workplaces, workplaces where employees received training on alcohol, public sector and female-dominated workplaces were significantly associated with alcohol prevention activities. We conclude that training is important for the implementation of alcohol prevention at Swedish workplaces, and is of particular importance in small enterprises.

    • Målsättningen har varit att ta fram ny kunskap inom miljötillsynen och därigenom uppnå en effektivare miljötillsyn samt att få in nya vetenskapliga perspektiv på miljötillsyn.I rapporten studeras metoder för inspektioner och det kommunikativa samspelet mellan inspektören och företrädare för den verksamhet som inspekteras, hur den institutionella ramen för inspektionsprocessen fungerar samt visar på möjligheter att mäta effekterna av inspektioner och tillsyn.Naturvårdsverket kommer att ha resultatet som ett kunskapsunderlag i fortsatt arbete med tillsynsvägledning och utveckling av hur tillsyn och tillsynsvägledning kan följas upp och utvärderas.

  • Novel biochemical markers of psychosocial stress in women.

    • BACKGROUND: Prolonged psychosocial stress is a condition assessed through self-reports. Here we aimed to identify biochemical markers for screening and early intervention in women. METHODS: Plasma concentrations of interleukin (IL) 1-alpha, IL1-beta, IL-2, IL-4, IL-6, IL-8, IL-10, interferon-gamma (INF-gamma), tumor necrosis factor-alpha (TNF-alpha), monocyte chemotactic protein-1 (MCP-1), epidermal growth factor (EGF), vascular endothelial growth factor (VEGF), thyroid stimulating hormone (TSH), total tri-iodothyronine (TT3), total thyroxine (TT4), prolactin, and testosterone were measured in: 195 women on long-term sick-leave for a stress-related affective disorder, 45 women at risk for professional burnout, and 84 healthy women. RESULTS: We found significantly increased levels of MCP-1, VEGF and EGF in women exposed to prolonged psychosocial stress. Statistical analysis indicates that they independently associate with a significant risk for being classified as ill. CONCLUSIONS: MCP-1, EGF, and VEGF are potential markers for screening and early intervention in women under prolonged psychosocial stress.

    • Formulären AUDIT och DUDIT är evidensbaserade och lättillgängliga verktyg som identifierar problematiskt alkohol- och drogbruk och ger ett bra underlag för dialog mellan behandlare och klient/patient. AUDIT och DUDIT rekommenderas som förstahandsval för screening (identifiering) i Socialstyrelsens riktlinjer för missbruks- och beroendevård. Det här är en handbok som tar upp användning, poängsättning och tolkning av formulären samt olika alternativ för att gå vidare vid positivt utfall. Den presenterar också hur man kan samtala kring resultatet och hur vidare utredning, diagnostisering och uppföljning går till. Boken AUDIT och DUDIT - identifiera problem med alkohol och droger vänder sig till behandlare och rådgivare inom bland annat hälso- och sjukvård, elevhälsa, mödravård, socialtjänst, beroendevård, kriminalvård och psykiatri.

  • Hopeless patients? : A study of illicit opiate users who drop out from in-patient detoxification

    • Individual and treatment-related factors associated with early self-discharge were studied among illicit opiate users consecutively admitted for hospital detoxification (ITT=68, n=45). Only 20% of the drop-outs had a detailed treatment plan, 20% participated in their own treatment planning, and none were enrolled in a methadone/buprenorphine programme at admission (compared with 60, 80, and 66%, respectively, of those retained in treatment). Having a treatment plan at intake improved the odds for planned discharge by 13 and perceiving fewer positive aspects of drug use increased the odds for drop-out by 12.6. A configural frequency analysis (CFA) showed one significant type: patients with sketchy or no treatment planning at intake, no maintenance treatment, and a low score on positive aspects of drug use were 7.5 times more likely to drop out than expected. Lacking a close maternal relationship characterized patients in this type (83% compared with 31% of non-types). Drop-outs seem to have insecure attachment patterns, which make contact difficult with social services and substance abuse treatment providers. Future research should devise ways of establishing a working alliance with patients likely to drop out, as well as investigating management policies for this patient group.

    • Among clients who have been screened already for drug-related problems, the Drug Use Disorders Identification Test - Extended (DUDIT-E) maps the frequency of illicit drug use (D), the positive (P) and negative (N) aspects of drug use, and treatment readiness (T). D scores correlated with Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition diagnoses among 154 heavy drug users from criminal justice and drug detoxification settings, as well as with urine test results in drug detoxification units. One-week test/retest intraclass correlation coefficients among 92 male prison inmates were .90, .78, .75, and .84 for D, P, N, and T scores, respectively. Cronbach's alpha were .88-.95 for P score, .88-.93 for N score, and .72-.81 for T score. Principal components analysis supported construct validity for P, N, and T scores. T scores were higher in prison treatment units than in motivational and regular units without treatment emphasis. Motivational index scores differentiated between three categories of heavy drug users; they did not differentiate between prisons and unit types, but this corresponded to unclear structural differentiation between units.

  • En kvalitativ studie av hemlösa människors erfarenhet av boendestöd i två  boendeprogram

    • It is widely accepted that homelessness has increased in many countries and nationally the number also seems to increase (EU, 2011; The National Board of Welfare and Health, 2011). Ways to respond to that increase is by developing and implement new programs to handle long-time homelessness, and in Sweden and in Stockholm a version of the Housing First program has been tried out for almost two years. That project has being evaluated (Källmén & Blid, 2013) and this study is an extension of the original study and further exploration of questions concerning users’ and social workers´ experiences and satisfaction of housing support within  two different programs.   The aim of this study is to follow-up people who have experienced long-time homelessness and their experiences of the two programs: the staircase model and housing First model in a Swedish context.A qualitative study was undertaken in inner city Stockholm with adults who have experienced homelessness. Information came from interviews. Results shows that participants experiences primarily didn´t depend on what type of program they were in but rather if it succeeded in filling their days with meaningful activities including help to handle misuse problems.
