    • ABSTRACT The purpose of this article is to describe the emerging science of improvement in health care and to add a perspective from the industrial quality improvement movement, the use of data from quality registers, and to give some personal reflections and suggestions. Furthermore, we want to introduce to the broader quality management community what is happening in healthcare with respect to quality improvement. We will discuss some of the challenges of the health care system and the current status of a science ofimprovement and give some suggestions for further improvements to the area.We discuss a possible extension of improvement knowledge and thetheoretical and practical arsenal of a science of improvement, in particular, understanding variation and implications for the use of, for, example controlcharts. In addition, the normative side of a science of improvement is discussed. The article ends with some brief reflections of use for future research agendas.

    • The authors agree that operations management is a discipline of great importance to a science of improvement in health care. They also believe that action research (AR) aiming for sustainable system changes and also providing a more beneficial context for process improvements is an important and fruitful way forward. AR is an approach to inquiry where the researcher(s) engage in collaborative communities with practitioners on an equal grounding. The role of the patient as a co-producer illuminates the importance of person-centered care, where patients and health care professionals collaborate on stated care plans. When the patient is included in the care plan as an equal partner there can be room for increased self-management and empowered patients contributing to safe care. In conclusion, the authors agree with Does and colleagues about the importance of operations management and its different practices.

    • Drawing from collaborative public management, this article seeks to contribute topublic service logic by focusing on what precedes the public service user’s realizationof value: the value proposition. A new care model for elderly people with multiplechronic diseases shows that coordinators with an inter-organizational mission, ver-tical and horizontal supporting structures, trust established through relationships,and recognition of service systems’embeddedness in social systems are pivotal forthe ability of public service organizations to develop coordinated value propositions.The contribution to policy and practice is an increased understanding of a coherent,rather than fragmented, welfare system for users/citizens.

  • Value Propositions in Public Collaborations : Regaining Organizational Focus Through Value Configurations

    • There is consensus that complex problems of contemporary society call for public service collaborations. So-called public service logic (PSL) focuses on joint value creation among a multiplicity of actors in service ecosystems. Despite recognizing various actors, this logic is essentially user-centric, with the service user being the one realizing the value. Consequently, single and collaborating organizations cannot deliver value, only potential value, or so-called value propositions. The elusive public service logic takes a network value conguration for granted and as a starting point. Drawing from two cases in Swedish healthcare, this paper argues that two other value congurations (chain and shop) are also relevant for understanding the development of value propositions – and that these may be related to both intra- and inter-organizational processes. Theoretically, we conclude that just like public service logic, other collaborative public management theories need to recognize the importance of a multiplicity of value congurations and that these are often related to both intra- and inter-organizational processes. We conclude that managers should not adopt the latest network trends without first reflecting on the relevance of existing internal processes.

  • A Physician-Led and Learning-Driven Approach to Regional Development of 23 Cancer Pathways

    • Sweden has one of the best cancer care systems in the world when it comes to survival. However, prognoses shows that by 2030 the number of men with cancer will increase by almost 130% compared to today – the corresponding increase for women is around 70%. Approximately half of this increase can be explained by population changes, i.e. population size and age structure. Due to this future challenge, a national cancer strategy has been created. We will present some strategies that the Regional Cancer Centre West is working on to face the future challenge. A central part of this work is to create patient-focused, coherent regional care pathways for patients with malignant diseases. For this purpose, 30 experienced senior physicians, specialized in different tumour diseases, have been appointed as regional process (pathway) owners. Together, they share a regional responsibility for 23 cancer pathways, and nine support processes.
