    • Objectives: HIV-positive people have increased risk of infection-related malignancies (IRMs) and infection-unrelated malignancies (IURMs). The aim of the study was to determine the impact of aging on future IRM and IURM incidence. Methods: People enrolled in EuroSIDA and followed from the latest of the first visit or 1 January 2001 until the last visit or death were included in the study. Poisson regression was used to investigate the impact of aging on the incidence of IRMs and IURMs, adjusting for demographic, clinical and laboratory confounders. Linear exponential smoothing models forecasted future incidence. Results: A total of 15 648 people contributed 95 033 person-years of follow-up, of whom 610 developed 643 malignancies [IRMs: 388 (60%); IURMs: 255 (40%)]. After adjustment, a higher IRM incidence was associated with a lower CD4 count [adjusted incidence rate ratio (aIRR) CD4 count < 200 cells/μL: 3.77; 95% confidence interval (CI) 2.59, 5.51; compared with ≥ 500 cells/μL], independent of age, while a CD4 count < 200 cells/μL was associated with IURMs in people aged < 50 years only (aIRR: 2.51; 95% CI 1.40–4.54). Smoking was associated with IURMs (aIRR: 1.75; 95% CI 1.23, 2.49) compared with never smokers in people aged ≥ 50 years only, and not with IRMs. The incidences of both IURMs and IRMs increased with older age. It was projected that the incidence of IRMs would decrease by 29% over a 5-year period from 3.1 (95% CI 1.5–5.9) per 1000 person-years in 2011, whereas the IURM incidence would increase by 44% from 4.1 (95% CI 2.2–7.2) per 1000 person-years over the same period. Conclusions: Demographic and HIV-related risk factors for IURMs (aging and smoking) and IRMs (immunodeficiency and ongoing viral replication) differ markedly and the contribution from IURMs relative to IRMs will continue to increase as a result of aging of the HIV-infected population, high smoking and lung cancer prevalence and a low prevalence of untreated HIV infection. These findings suggest the need for targeted preventive measures and evaluation of the cost−benefit of screening for IURMs in HIV-infected populations.

  • Optimisation of slab track design considering dynamic train–track interaction and environmental impact

    • Modern railway tracks for high-speed traffic are often built based on a slab track design. A major disadvantage of slab track compared to conventional ballasted track is that the environmental impact of the construction is higher due to the significant amount of concrete required. In this paper, the dimensions of the rectangular cross-sections and the types of concrete used in slab tracks are optimised with the objective to minimise greenhouse gas emissions, while considering the constraint that the design must pass the static dimensioning analysis described in the European standard 16432-2. The optimised track design is also analysed using a three-dimensional (3D) model of vertical dynamic vehicle–track interaction, where the rails are modelled as Rayleigh–Timoshenko beams and the concrete parts are represented by quadratic shell elements. Wheel–rail contact forces and the time-variant stress field of the concrete parts are calculated using a complex-valued modal superposition for the finite element model of the track. For the studied traffic scenario, it is concluded that the thickness of the panel can be reduced compared to the optimised design from the standard without the risk of crack initiation due to the dynamic vehicle load. In parallel, a model of reinforced concrete is developed to predict crack widths, the bending stiffness of a cracked panel section and to assess in which situations the amount of steel reinforcement can be reduced. To reduce the environmental impact even further, there is potential for an extended geometry optimisation by excluding much of the concrete between the rails.

  • Association between convalescent plasma treatment and mortality in COVID-19 : a collaborative systematic review and meta-analysis of randomized clinical trials

    • Background: Convalescent plasma has been widely used to treat COVID-19 and is under investigation in numerous randomized clinical trials, but results are publicly available only for a small number of trials. The objective of this study was to assess the benefits of convalescent plasma treatment compared to placebo or no treatment and all-cause mortality in patients with COVID-19, using data from all available randomized clinical trials, including unpublished and ongoing trials (Open Science Framework, ). Methods: In this collaborative systematic review and meta-analysis, clinical trial registries (ClinicalTrials.gov, WHO International Clinical Trials Registry Platform), the Cochrane COVID-19 register, the LOVE database, and PubMed were searched until April 8, 2021. Investigators of trials registered by March 1, 2021, without published results were contacted via email. Eligible were ongoing, discontinued and completed randomized clinical trials that compared convalescent plasma with placebo or no treatment in COVID-19 patients, regardless of setting or treatment schedule. Aggregated mortality data were extracted from publications or provided by investigators of unpublished trials and combined using the Hartung-Knapp-Sidik-Jonkman random effects model. We investigated the contribution of unpublished trials to the overall evidence. Results: A total of 16,477 patients were included in 33 trials (20 unpublished with 3190 patients, 13 published with 13,287 patients). 32 trials enrolled only hospitalized patients (including 3 with only intensive care unit patients). Risk of bias was low for 29/33 trials. Of 8495 patients who received convalescent plasma, 1997 died (23%), and of 7982 control patients, 1952 died (24%). The combined risk ratio for all-cause mortality was 0.97 (95% confidence interval: 0.92; 1.02) with between-study heterogeneity not beyond chance (I-2 = 0%). The RECOVERY trial had 69.8% and the unpublished evidence 25.3% of the weight in the meta-analysis. Conclusions: Convalescent plasma treatment of patients with COVID-19 did not reduce all-cause mortality. These results provide strong evidence that convalescent plasma treatment for patients with COVID-19 should not be used outside of randomized trials. Evidence synthesis from collaborations among trial investigators can inform both evidence generation and evidence application in patient care.

    • We examine if the Nordic power market, Nord Pool, has been competitive or if electricity suppliers have had market power. Specifically, since the evolution from national markets to a multi-national and largely deregulated power market has taken place stepwise, we also examine how the degree of market power has evolved during this integration process. The Bresnahan-Lau method together with weekly data during 1996-2004 are used in the analysis, which shows that electricity suppliers have had small, but statistically significant, market power, but that the market power has been reduced as the Nord Pool area has expanded

    • Introduction Early-stage chronic lymphocytic leukemia (CLL) challenges specialized management and follow-up.Methods We developed and validated a prognostic index to identify newly diagnosed patients without need of treatment (CLL-WONT) by a training/validation approach using data on 4708 patients. Composite scores derived from weighted hazards by multivariable analysis defined CLL-WONT risk groups.Results Age (>65 years: 1 point), Binet stage (B: 2 points), lactate dehydrogenase (LDH) (>205 U/L: 1 point), absolute lymphocyte count (15-30 x 10(9)/L: 1 point; >30 x 10(9)/L; 2 points), beta 2-microglobulin (>4 mg/L: 1 point), IGHV mutation status (unmutated: 1 point), and 11q or 17p deletion (1 point) were independently associated with shorter time to first treatment (TTFT). Low-risk patients demonstrated 5-year TTFT of 2% by internal validation, but 7-19% by external validation. Including all patients with complete scores, the 5-year TTFT was 10% for the 756 (39%) CLL-WONT low-risk patients, and the 704 (37%) patients who were both CLL-WONT and CLL-IPI low risk demonstrated even lower 5-year TTFT (8%).Conclusion We have adopted the CLL-WONT at an institution covering 1 800 000 individuals to allow patients with both low-risk CLL-WONT and CLL-IPI to be managed by primary healthcare providers, thereby prioritizing specialized hematology services for patients in dire need.

  • Methanol production via black liquor co-gasification with expanded raw material base : Techno-economic assessment

    • Entrained flow gasification of black liquor combined with downstream-gas-derived synthesis of biofuels in Kraft pulp mills has shown advantages regarding energy efficiency and economic performance when compared to combustion in a recovery boiler. To further increase the operation flexibility and the profitability of the biofuel plant while at the same time increase biofuel production, black liquor can be co-gasified with a secondary feedstock (blend-in feedstock). This work has evaluated the prospects of producing biofuels via co-gasification of black liquor and different blend-in feedstocks (crude glycerol, fermentation residues, pyrolysis liquids) at different blend ratios. Process modelling tools were used, in combination with techno-economic assessment methods. Two methanol grades, crude and grade AA methanol, were investigated. The results showed that the co-gasification concepts resulted in significant increases in methanol production volumes, as well as in improved conversion efficiencies, when compared with black liquor gasification; 5-11 and 4-10 percentage point in terms of cold gas efficiency and methanol conversion efficiency, respectively. The economic analysis showed that required methanol selling prices ranging from 55-101 €/MWh for crude methanol and 58-104 €/MWh for grade AA methanol were obtained for an IRR of 15%. Blend-in led to positive economies-of-scale effects and subsequently decreased required methanol selling prices, in particular for low cost blend-in feedstocks (prices below approximately 20 €/MWh). The co-gasification concepts showed economic competitiveness to other biofuel production routes. When compared with fossil fuels, the resulting crude methanol selling prices were above maritime gas oil prices. Nonetheless, for fossil derived methanol prices higher than 80 €/MWh, crude methanol from co-gasification could be an economically competitive option. Grade AA methanol could also compete with taxed gasoline. Crude glycerol turned out as the most attractive blend-in feedstock, from an economic perspective. When mixed with black liquor in a ratio of 50/50, grade AA methanol could even be cost competitive with untaxed gasoline.

    • This study evaluated the techno-economic performance and potential benefits of methanol production through catalytic gasification of forest residues and lignin. The results showed that while catalytic gasification enables increased cold gas efficiencies and methanol yields compared to non-catalytic gasification, the additional pre-treatment energy and loss of electricity production result in small or no system efficiency improvements. The resulting required methanol selling prices (90–130 €/MWh) are comparable with production costs for other biofuels. It is concluded that catalytic gasification of forest residues can be an attractive option as it provides operational advantages at production costs comparable to non-catalytic gasification. The addition of lignin would require lignin costs below 25 €/MWh to be economically beneficial.
