  • Age dependence of renal function: clearance of iohexol and p-amino hippurate in healthy males

    • Iohexol, a newly developed non-ionic contrast agent, has been recently documented as a reliable glomerular filtration marker. This study describes the age dependence of the single injection clearance of iohexol in a sample of healthy male volunteers ranging from 21 to 77 years of age. In parallel, renal plasma flow was studied by measuring the total clearance of p-amino hippuric acid administered as a continuous infusion. In subjects older than 50 years a negative correlation to age was found for both p-amino hippuric acid and iohexol clearance, with a reduction of 52 ml/min and 12 ml/min per decade, respectively, whereas no age dependence was found for younger subjects. Correlation between p-amino hippuric acid and iohexol clearances was 0.81. However, the filtration fraction, defined as the ratio of iohexol to p-amino hippuric acid clearance, was higher in the elderly subjects. A consistent discrepancy was found between total and renal clearances of p-amino hippuric acid, indicating significant renal metabolism. Renal clearance of creatinine was poorly correlated to iohexol clearance and did not show any relationship to age.

  • Identification of four novel T cell autoantigens and personal autoreactive profiles in multiple sclerosis

    • Multiple sclerosis (MS) is an inflammatory disease of the central nervous system (CNS), in which pathological T cells, likely autoimmune, play a key role. Despite its central importance, the autoantigen repertoire remains largely uncharacterized. Using a novel in vitro antigen delivery method combined with the Human Protein Atlas library, we screened for T cell autoreactivity against 63 CNS-expressed proteins. We identified four previously unreported autoantigens in MS: fatty acid-binding protein 7, prokineticin-2, reticulon-3, and synaptosomal-associated protein 91, which were verified to induce interferon-gamma responses in MS in two cohorts. Autoreactive profiles were heterogeneous, and reactivity to several autoantigens was MS-selective. Autoreactive T cells were predominantly CD4(+) and human leukocyte antigen-DR restricted. Mouse immunization induced antigen-specific responses and CNS leukocyte infiltration. This represents one of the largest systematic efforts to date in the search for MS autoantigens, demonstrates the heterogeneity of autoreactive profiles, and highlights promising targets for future diagnostic tools and immunomodulatory therapies in MS.

  • Helicobacter species DNA in liver and gastric tissues in children and adolescents with chronic liver disease

    • Objective. Enterohepatic Helicobacter species (EHS) have previously been found in adults with hepatobiliary diseases. Here, we report the prevalence of Helicobacter pylori and EHS in liver and gastric tissue in children and adolescents with chronic liver disease (CLD). Material and methods. Seventy-seven consecutive children and adolescents with CLD with or without ulcerative colitis or Crohn's disease (UC/CD) were investigated. Tissue samples were analysed using a Helicobacter genus specific 16S rDNA polymerase chain reaction (PCR) assay and DNA-sequence analysis. Sera from 61 subjects were also analysed using enzyme immunoassay and immunoblotting. Results. The Helicobacter PCR was positive in 3/23 (13%) livers from patients with primary sclerosing cholangitis and UC, and in 1/2 livers from patients with autoimmune hepatitis (AIH) and UC. Sequenced PCR products matched the 16S rDNA of H. hepaticus, H. muridarum, H. canis, and H. pylori, respectively. H. ganmani and H. bilis were detected in gastric tissues from two AIH patients. H. hepaticus and H. pullorum were found in livers from two patients with acute liver failure and intrahepatic cholestasis. Antibody reactivity to Helicobacter cell-surface proteins was negative. Conclusions. H. pylori and EHS can be detected in the livers of some patients with UC and concomitant liver disease, as well as in other children with liver diseases. Multicentre studies from different locations are needed to find out whether these bacteria play a pathogenetic role or whether their presence is an epiphenomenon.

  • Comparative Proton and Photon Treatment Planning in Pediatric Patients with Various Diagnoses

    • Purpose: Radiation therapy with protons, owing to its physical properties, can beadvantageous for the treatment of children. This study was conducted in order to quantify the advantages of proton therapy from a treatment planning point of view in a consecutive, realistic, and mixed pediatric/adolescent population with varying diagnoses and target locations.Patients and Methods: Forty-five patients treated with conventional 3 dimensional conformal radiation therapy photon radiation therapy were retrospectively re-planned with scanned proton beams. Treatment sites represented were the central nervous system, head and neck, thorax, and abdomen. Median age was 8 years (range, 2-18 years). All plans were optimized with intensity-modulated proton therapy (multi-field optimization). We analyzed a number of dose-volume descriptors for planned targetvolumes (PTVs). Organ-specific mean doses and relevant DV -values were derived fororgans at risk. In addition, homogeneity index, conformity index, treated volume, and integral dose were calculated for each treatment plan. The Wilcoxon matched-pairs signed rank test was used for studying differences between these variables for the 2 treatment modalities.Results: Planned target volume coverage (V95%) was similar when comparing photons and protons. Conformity and homogeneity indices were similar or better for protons for most cases. In general, doses to organs at risk were lower with protons. In cases with organs at risk in close vicinity to the PTV, the gain with protons is less.Conclusions: The patient cohort benefits from reduced integral dose with protons compared with photons. Patients with tumors in the central nervous system, head and neck, upper mediastinum, and some abdominal locations will gain significantly if treated with protons compared with photons. For 7 of the 45 consecutive patients studied (whole brain, whole lung, whole abdomen, flank treatment), we found no gain with protons (difference in integral dose less than 8%).

  • MTH1 inhibition eradicates cancer by preventing sanitation of the dNTP pool

    • Cancers have dysfunctional redox regulation resulting in reactive oxygen species production, damaging both DNA and free dNTPs. The MTH1 protein sanitizes oxidized dNTP pools to prevent incorporation of damaged bases during DNA replication. Although MTH1 is non-essential in normal cells, we show that cancer cells require MTH1 activity to avoid incorporation of oxidized dNTPs, resulting in DNA damage and cell death. We validate MTH1 as an anticancer target in vivo and describe small molecules TH287 and TH588 as first-in-class nudix hydrolase family inhibitors that potently and selectively engage and inhibit the MTH1 protein in cells. Protein co-crystal structures demonstrate that the inhibitors bindin the active site of MTH1. The inhibitors cause incorporation of oxidized dNTPs in cancer cells, leading to DNA damage, cytotoxicity and therapeutic responses in patient-derived mouse xenografts. This study exemplifies the non-oncogene addiction concept for anticancer treatment and validates MTH1 as being cancer phenotypic lethal.

    • The official control programme for organochlorine (OC) contaminants in food producing animals in Sweden was used to study temporal and spatial trends of the polychlorinated biphenyl CB 153, hexachlorobenzene (HCB) and p,p'-DDE in adipose tissue from bovines and swine 1991-2004. Our results show that efforts to decrease OC contamination of animal feed and the environment have had a positive impact on the contamination of the animal production. OC concentrations declined significantly in almost all studied regions of Sweden. OC temporal trends were slower in bovines (6-8% per year) than in swine (10-12%). Power analyses showed that data from more than 10 years of sampling were needed for a detection of an annual OC level change of 5% in both species in the control programme, due to large within- and between-year variation in OC levels. CB 153 and p,p'-DDE levels were higher in southern than in northern Sweden. Levels decreased with age in milk cows, but not in young nulliparous cows (heifers) and bulls. Moreover, milk cows and bulls had significantly lower OC levels than heifers. Levels were not age-dependent among swine, but castrated male swine (barrows) had significantly lower OC levels than young female swine (gilts). Levels of the studied OCs are now in many cases below the LOQ of the analytical method used. Future time trend studies of these OCs thus depend on lowered LOQs in the control programme.

  • Minskande nivåer av PCB, PCB och p,p´-DDE i fett från nötboskap och svin i Sverige 1991-2004

    • Syftet med studien var att undersöka om halterna av polyklorerade bifenyler (PCB kongen CB 153), hexaklorbensen (HCB) och p,p´-DDE i fett från nötboskap och svin i svensk livsmedelsproduktion förändrats under perioden 1991-2004. Eventuella skillnader i halter mellan olika regioner i Sverige studerades också. Data erhölls från Livsmedelsverkets ordinarie kontrollprogram för kontaminanter i nöt och svin. Multipel regressionsanalys användes för att studera tidstrender, regionala skillnader, könsskillnader och åldersskillnader i halter. Analysen visade att halterna av miljöföroreningarna i nötfett minskade med ca. 6-8% per år, medan halterna i svinfett minskade med 10-12% per år. Sjunkande trender påvisades i praktiskt taget alla de 6 svenska regioner som studerades. Halterna av CB 153 och p,p´-DDE i nöt och svin var klart lägre i norr än i söder. För HCB i nötfett erhölls inga markanta regionala skillnader. Bland nötkreaturen var halterna lägre hos mjölkkor än hos köttdjur (både kvigor och tjurkalvar/tjurar). Halterna minskade också med åldern hos mjölkkor men inte hos köttdjur. Bland svinen antyddes lägre halter av CB 153 och p,p´-DDE hos kastrerade hanar än bland suggor och galtar, men vi fann inga samband mellan svinens ålder och nivåer av miljöföroreningarna. Mot slutet av studieperioden låg en stor del av analysresultaten under kvantifieringsgränsen för analysmetoden (0,5-1 μg/kg fett). Om fortsatta tidstrendsstudier ska vara möjliga måste kvantifieringsgränsen för metoden sänkas. De sjunkande halterna av PCB, HCB och p,p´-DDE som obserververades i livsmedelsproducerande nötkreatur och svin är troligen en följd av tidigare åtgärder för att minska miljöbelastningen av dessa föroreningar. Resultaten antyder också att en lägre miljöbelastning av norra Sverige avspeglas som lägre halter i köttproduktionen och troligen också i mjölkproduktionen (studien inkluderade också mjölkkor).
