  • Ataxia-pancytopenia syndrome with SAMD9L mutations

    • OBJECTIVE: We describe the neurologic, neuroradiologic, and ophthalmologic phenotype of 1 Swedish and 1 Finnish family with autosomal dominant ataxia-pancytopenia (ATXPC) syndrome and SAMD9L mutations.METHODS: Members of these families with germline SAMD9L c.2956C>T, p.Arg986Cys, or c.2672T>C, p.Ile891Thr mutations underwent structured interviews and neurologic and ophthalmologic examinations. Neuroimaging was performed, and medical records were reviewed. Previous publications on SAMD9L-ATXPC were reviewed.RESULTS: Twelve individuals in both families were affected clinically. All mutation carriers examined had balance impairment, although severity was very variable. All but 1 had nystagmus, and all but 1 had pyramidal tract signs. Neurologic features were generally present from childhood on and progressed slowly. Two adult patients, who experienced increasing clumsiness, glare, and difficulties with gaze fixation, had paracentral retinal dysfunction verified by multifocal electroretinography. Brain MRI showed early, marked cerebellar atrophy in most carriers and variable cerebral periventricular white matter T2 hyperintensities. Two children were treated with hematopoietic stem cell transplantation for hematologic malignancies, and the neurologic symptoms of one of these worsened after treatment. Three affected individuals had attention deficit hyperactivity disorder or cognitive problems. Retinal dysfunction was not previously reported in individuals with ATXPC.CONCLUSIONS: The neurologic phenotype of this syndrome is defined by balance or gait impairment, nystagmus, hyperreflexia in the lower limbs and, frequently, marked cerebellar atrophy. Paracentral retinal dysfunction may contribute to glare, reading problems, and clumsiness. Timely diagnosis of ATXPC is important to address the risk for severe hemorrhage, infection, and hematologic malignancies inherent in this syndrome; regular hematologic follow-up might be beneficial.

  • A comparison of very old patients admitted to intensive care unit after acute versus elective surgery or intervention

    • Background: We aimed to evaluate differences in outcome between patients admitted to intensive care unit (ICU) after elective versus acute surgery in a multinational cohort of very old patients (80 years; VIP). Predictors of mortality, with special emphasis on frailty, were assessed.Methods: In total, 5063 VIPs were induded in this analysis, 922 were admitted after elective surgery or intervention, 4141 acutely, with 402 after acute surgery. Differences were calculated using Mann-Whitney-U test and Wilcoxon test. Univariate and multivariable logistic regression were used to assess associations with mortality.Results: Compared patients admitted after acute surgery, patients admitted after elective surgery suffered less often from frailty as defined as CFS (28% vs 46%; p < 0.001), evidenced lower SOFA scores (4 +/- 5 vs 7 +/- 7; p < 0.001). Presence of frailty (CFS >4) was associated with significantly increased mortality both in elective surgery patients (7% vs 12%; p = 0.01), in acute surgery (7% vs 12%; p = 0.02).Conclusions: VIPs admitted to ICU after elective surgery evidenced favorable outcome over patients after acute surgery even after correction for relevant confounders. Frailty might be used to guide clinicians in risk stratification in both patients admitted after elective and acute surgery.

  • Between Political Party and Armed Group : Understanding Renamo as a Hybrid Party

    • This thesis explores the concept of hybridity within the context of rebel-to-party transformation, with a particular focus on Renamo during the period of 2015-2017 in Mozambique. This timeframe was marked by a resurgence of organised systematic violence, disrupting the relative peace that had prevailed in Mozambique since 1992. The central conflict revolved around the issue of local self-governance, with Renamo asserting its claim to govern in six out of Mozambique's ten provinces. During this period, Renamo operated both as a political party with a significant presence in parliament and as an armed group engaged in systematic violence.The rebel-to-party literature has sparked discussions about groups that seem to retain elements of their violent past while transitioning into political parties, leading to the emergence of the concept of hybrid parties. However, this concept remains relatively undefined and underexplored. This thesis seeks to contribute to this discussion through an in-depth qualitative study of Renamo's elite-level politicians, aiming to enhance our understanding of hybrid parties.The study, conducted during the conflict years, closely examines how elite politicians in Renamo navigate their dual roles. It encompasses 14 months of fieldwork from February 2015 to January 2017, drawing on elite interviews, elite-level public statements, and elite observations of parliament, enriched by ethnographic sensibility. The analysis is grounded in a theoretical framework that allows for an exploration of Renamo's behaviour and perceptions as both an armed group and a political party, particularly concerning the issue of local self-governance. Through this analysis, the study aims to elucidate the intersections, thereby advancing our comprehension of how hybridity manifests.The main findings suggest that Renamo's hybridity predominantly manifests through processes related to contemporaneity in behaviour, elite-level legitimisation, and one key political issue. Furthermore, I argue that the hybridity is maintained through narratives of democracy and a charismatic leader. The thesis advocates for a deeper exploration of these processes to enhance both empirical understanding and the theoretical discussion surrounding hybrid parties.

  • Hållbarhetsanalys av utvecklingsvägen ”starka sektorskopplingar” för att uppnå ett hundra procent förnybart elsystem : Delrapport i projektet "Hundra procent förnybart - hur många procent hållbart?" i samarbete med Mistra Carbon Exit

    • Rapporten är ett samarbete mellan projekten ”Hundra procent förnybart – hur många procent hållbart?” och Mistra Carbon Exit (MCE). Denna inledande analys utgör ett första steg att identifiera de mest kritiska hållbarhetsaspekterna (ekonomiska, ekologiska och sociala) som den fortsatta hållbarhetsanalysen i projektet ”Hundra procent förnybart – hur många procent hållbart” ska fokusera på.Fokus i rapporten är framtagande av en analys av det preliminära scenariot ”Starka sektorskopplingar” som en väg att nå ett hundra procent förnybart elsystem. Med ”Starka sektorskopplingar” avses mellan elproduktion och transport- och industrisektorerna i form av värmelager, vätgaslager, uppvärmningssystem samt flexibel laddning av elfordon.Tre fallstudier (värmelager, vätgaslager och flexibel laddning av elfordon) har valts ut och studerats på en workshop. På workshopen fick expertgrupperna för de tre fallstudierna resonera kring påverkan på de 17 globala hållbarhetsmålen med det digitala verktyget SDG Impact Assessment Tool. Slutsatser från workshopen, både utifrån medskick till den kommande fortsatta hållbarhetsanalysen samt erfarenheter av metodiken, har sedan sammanställts.Verktyget SDG Impact Assessment Tool ger möjlighet att systematiskt gå igenom samtliga 17 hållbarhetsmål för en fallstudie och med det undvika att handplocka de SDG:er som vid första anblick skulle kunna ses som mest relevanta. Metoden ger en djup förståelse kring motiv och argument till den utförda analysen, där resultaten avspeglar sammansättning av de deltagande experterna. Resultaten är strategiska i att guida det fortsatta arbetet för kvantifiering av hållbarhetspåverkan. Det är nödvändigt att på förhand tydligt definiera fallstudien för att ge gruppen samsyn om vad som ska analyseras, samt att möjliggöra för gruppen på plats att ytterligare precisera definitionen vid behov.Generellt har expertgrupperna bedömt att målen som riktar sig mer mot hållbarhet kopplat till sociala aspekter inte påverkas i samma utsträckning som de som är tydligare riktade mot miljö- och ekonomisk hållbarhet. Hållbarhetsmål som i sin beskrivning är mer direkt kopplade till omställningen av energisystemet (så som energi, klimat, infrastruktur och samhällsaspekter) har även i stort bedömts ha tydligare påverkan. Gemensamt visar gruppernas utförda bedömningar att de positiva effekterna överlag har bedömts vara fler, och i två av fallstudierna gjordes ingen sammanvägd bedömning om negativ påverkan för något av de 17 SDG:erna.Det är däremot inte sagt att det inte finns eller diskuterats några negativa effekter i grupperna. Exempelvis diskuterades att materialåtgång och ingrepp vid uppförande av de olika teknikerna kan påverka hållbarhetsmålen negativt. Dessutom kan till exempel en nationellt sett positiv påverkan på ett mål samtidigt ha en lokalt negativ påverkan.Genom den utförda workshopen har en första indikation och identifiering gjorts av vilka SDG:er som påverkas för respektive fallstudie.Även om fallstudierna har olika karaktär framgår i flertalet fall liknande tendenser och riktningar vid SDG-bedömningarna. Denna inledande analys ger ett värdefullt underlag för projektets nästa steg där indikatorer och analytiskt ramverk definieras, som bas för hållbarhetsanalysen där även målkonflikter ska analyseras.

    • Background We have developed genomic tools to allow the genus Populus (aspens and cottonwoods) to be exploited as a full-featured model for investigating fundamental aspects of tree biology. We have undertaken large-scale expressed sequence tag (EST) sequencing programs and created Populus microarrays with significant gene coverage. One of the important aspects of plant biology that cannot be studied in annual plants is the gene activity involved in the induction of autumn leaf senescence. Results On the basis of 36,354 Populus ESTs, obtained from seven cDNA libraries, we have created a DNA microarray consisting of 13,490 clones, spotted in duplicate. Of these clones, 12,376 (92%) were confirmed by resequencing and all sequences were annotated and functionally classified. Here we have used the microarray to study transcript abundance in leaves of a free-growing aspen tree (Populus tremula) in northern Sweden during natural autumn senescence. Of the 13,490 spotted clones, 3,792 represented genes with significant expression in all leaf samples from the seven studied dates. Conclusions We observed a major shift in gene expression, coinciding with massive chlorophyll degradation, that reflected a shift from photosynthetic competence to energy generation by mitochondrial respiration, oxidation of fatty acids and nutrient mobilization. Autumn senescence had much in common with senescence in annual plants; for example many proteases were induced. We also found evidence for increased transcriptional activity before the appearance of visible signs of senescence, presumably preparing the leaf for degradation of its components.

    • Dizziness is a common and often untreated symptom in the general population. The aim of this study was to investigate the effects of a combined cognitive-behavioral/vestibular rehabilitation (VR) program, using a randomized control design. A total of 29 participants were randomized to treatment consisting of psychoeducation, vestibular exercises, relaxation and cognitive interventions, or to serve as waiting list controls. Measures of dizziness-related handicap, dizziness-provoking movements, and daily diary registrations of dizziness symptoms at pre- and post-treatment showed statistically significant improvements in many domains, which translated to moderate effect sizes. These findings provide preliminary support for the combination of Cognitive-behavioral therapy (CBT) and VR methods in the treatment of dizziness.

    • Background. Gastrointestinal stromal tumors (GISTs) can be effectively treated with tyrosine kinase inhibitors (TKIs). However, some patients with GIST develop drug resistance, and alternative treatment strategies are therefore needed. The aim of this study was to analyze the expression of somatostatin receptors (SSTR) in GIST as a target for peptide receptor-mediated radiotherapy (PRRT). Material and methods. Expression profiling of SSTR1-5 was performed on biopsies from 34 GISTs (16 gastric tumors, 15 small intestinal tumors, and three rectal tumors). SSTR scintigraphy ((111)In-octreotide) and measurement of (111)In activity in tumor specimens was performed in seven patients. Uptake and internalization of (177)Lu- octreotate was studied in primary cell cultures from two patients. Results. Quantitative PCR analysis showed expression of SSTR1 and SSTR2 in the majority of tumors, while SSTR3-5 were expressed at low levels. Immunohistochemical analysis confirmed the presence of SSTR1 and SSTR2 proteins in all GISTs, and SSTR3-5 in a subset of tumors. Diagnostic imaging by SSTR scintigraphy, using (111)In-octreotide, demonstrated tumor uptake of (111)In in three of six GIST patients. Measurement of (111)In activity in excised tumor specimens from five patients gave tumor-to-blood (T/B) activity ratios of between eight and 96. Tumor cells in primary culture (gastric and small intestinal GIST) specifically bound and internalized (177)Lu when incubated with the therapeutic compound (177)Lu-octreotate for 4-48 hours (p < 0.05). Conclusion. Peptide receptor-mediated radiotherapy via SSTR may provide a novel treatment strategy in carefully selected GIST patients with TKI-resistant tumors.

    • The choice of tree species used in production forests matters for biodiversity and ecosystem services. In Sweden, damage to young production forests by large browsing herbivores is helping to drive a development where sites traditionally regenerated with Scots pine (Pinus sylvestris) are instead being regenerated with Norway spruce (Picea abies). We provide a condensed synthesis of the available evidence regarding the likely resultant implications for forest biodiversity and ecosystem services from this change in tree species. Apart from some benefits (e.g. reduced stand-level browsing damage), we identified a range of negative outcomes for biodiversity, production, esthetic and recreational values, as well as increased stand vulnerability to storm, frost, and drought damage, and potentially higher risks of pest and pathogen outbreak. Our results are directly relevant to forest owners and policy-makers seeking information regarding the uncertainties, risks, and trade-offs likely to result from changing the tree species in production forests.
