    • GOALS OF WORK: Staff members in palliative home care play an important role in supporting bereaved family members. The aim of this study was to explore staff members' perspectives on providing such support. MATERIAL AND METHODS: Staff members in six units responded (n = 120; response rate 58%) to a postal questionnaire with Likert-type and open-ended questions. The responses were analyzed using statistics and manifest content analysis. MAIN RESULTS: None of the respondents stated that bereavement follow-up was "most often difficult," 23% "most often rather difficult," 52.5% "most often rather easy," and 12.5% "most often easy." Apart from a tendency for age to be linked to perceived difficulty, there were no apparent patterns. Bereavement follow-up was a positive opportunity to support the family member's coping with their bereavement and to get feedback on the palliative care provided. Critical aspects concerned the question of whose needs actually were being met at bereavement follow-up, i.e., the staff members' needs for getting feedback on the care provided versus the risk of burdening the family members' by reminding them of the deceased's dying trajectory. Aspects that negatively influenced the staff members' experiences were complex and related, e.g., to the family member's dissatisfaction with the care provided, to the staff member's perceived lack of competence, and to the staff member's relationship to the family member. CONCLUSIONS: Bereavement follow-up was perceived as a rewarding conclusion to the relationship with the family member. The findings suggest that meaning-based coping might be an appropriate framework when understanding staff members' experiences with providing bereavement follow-up.

  • Exercise radionuclide ventriculography for predicting postoperative left ventricular function in chronic aortic regurgitation

    • Objectives: Ejection fraction (EF) reaction upon exercise by radionuclide ventriculography and standard echocardiographic parameters was evaluated as predictors for post-operative left ventricular (LV) function in chronic aortic regurgitation (AR). Background: The optimal timing of surgery for chronic AR is when the left ventricle is still compensating for the volume and pressure overload without irreversible dysfunction. For asymptomatic patients when EF is normal and LV diameters are borderline, exercise testing is recommended by present guidelines. However, only a limited number of studies have been performed, and data are scarce on this subject. Methods: Radionuclide ventriculography with multiple gated acquisition at rest and during exercise was performed in 29 consecutive patients with severe chronic aortic regurgitation pre-operatively and 6 months post-operatively. Patient subgroups were formed based on pre-operative EF exercise response (ΔEF) and were categorized as decreasing (ΔEF <−5%), unaltered (−5% ≤ ΔEF ≤ 5%), and increasing (ΔEF > 5%). A 5% or higher increase was considered normal. The LV diameters and mass were measured by echocardiography. Results: Pre-operative LV diameters were markedly elevated before surgery and diminished significantly after surgery. Left ventricular diameters, LV mass, EF at rest (EFrest), and EF change from rest to exercise (ΔEF) were independent of New York Heart Association functional class. Pre-operative end-diastolic diameter proved to be a predictor for pre- and post-operative ΔEF (p = 0.003; p = 0.04) but not for the nature of the exercise response post-operatively. Patients with decreasing and unaltered EF pre-operatively presented a significantly higher but still abnormal ΔEF post-operatively. Those with increasing EF pre-operatively had a similar response and a normal ΔEF post-operatively. Pre-operative ΔEF was not only a predictor for post-operative ΔEF (p = 0.02) but also classified patients into post-operative subgroups (EF decreasing, p = 0.03; unaltered, p = 0.02; increasing, p = 0.0008). Conclusions: An abnormal EF response to exercise may also occur in patients who do not fulfill criteria for surgery based on LV dimensions or EF. A follow-up of exercise LV function and adjusting the timing of surgery according to the nature of exercise response could, therefore, be beneficial.

  • The effect gas composition during thermal aging on the dispersion and NO oxidation activity over Pt/Al2O3 catalysts

    • The aging of a model 1 wt.% Pt/Al2O3 catalyst was performed stepwise under different reactive atmosphere to study the evolution of metal dispersion and NO oxidation activity. After each aging step the dispersion was evaluated by CO chemisorptions and the activity of the catalyst for NO oxidation was measured using 500 ppm NO and 8%O-2 diluted in Ar. After a degreening step at 500 degrees C, aging was performed at 600, 700,800 and 900 degrees C. Five wash-coated cordierite monoliths were aged in Ar, 10% O-2, 1% H-2 30 ppm SO2 and 30 ppm SO2 + 10% O-2, respectively. The general trend showed a linear decrease in dispersion when increasing the aging temperature for the lower aging temperatures and for the highest ones the dispersion levels off. When the platinum dispersion decreased the NO oxidation activity increased, due to that the reaction is structure sensitive. H-2 seemed to hinder sintering at low aging temperature. Interestingly, after aging in 10% oxygen at 600 degrees C the NO oxidation activity was significantly higher compared to the Ar aged sample, although the dispersions were similar. Aging in oxygen at higher temperatures resulted in a decrease of dispersion and a slightly decreasing NO oxidation activity. Moreover lower dispersion limit was reached with oxygen aging. Aging in SO2 provoked a severe dispersion drop at low aging temperature meanwhile the activity increased only moderately. However, activity kept increasing with further treatments at higher temperature. The combination of O-2 and SO2 enabled to decrease rapidly the dispersion and to greatly enhance the catalytic NO oxidation activity after the first aging step at only 600 degrees C. The best overall conversion was obtained for the catalyst treated with this mixture after aging at 800 degrees C. A 22-h aging at 250 degrees C in a mixture containing 500 ppm NO, 10% O-2 and 30 ppm SO2 led to a significant decrease of Pt dispersion, which shows the ability of SO2 to promote platinum sintering already 250 degrees C. The low temperature sintering was confirmed with STEM measurements. Several larger particles were observed, but also many small particles remained. Thus the SO2 + O-2 induced low temperature sintering results in a large variation of particle sizes. This treatment resulted in an increase of the maximum NO conversion (after reduction of the sample) from 45% to 76%. The different aging experiments show that it is beneficial to add SO2 during aging and the reason is the increased particle size, but also a clear chemical effect was observed.

  • Extrusion of Porous Protein-Based Polymers and Their Liquid Absorption Characteristics

    • The production of porous wheat gluten (WG) absorbent materials by means of extrusion processing is presented for the future development of sustainable superabsorbent polymers (SAPs). Different temperatures, formulations, and WG compositions were used to determine a useful protocol that provides the best combination of porosity and water swelling properties. The most optimal formulation was based on 50 wt.% WG in water that was processed at 80 degrees C as a mixture, which provided a porous core structure with a denser outer shell. As a green foaming agent, food-grade sodium bicarbonate was added during the processing, which allowed the formation of a more open porous material. This extruded WG material was able to swell 280% in water and, due to the open-cell structure, 28% with non-polar limonene. The results are paving the way towards production of porous bio macromolecular structures with high polar/non-polar liquid uptake, using extrusion as a solvent free and energy efficient production technique without toxic reagents.

    • Orosanmälningar har på senare år ökat dramatiskt. Bland de viktiga yrkesgrupper som har skyldighet att anmäla till socialtjänsten när de hyser oro för ett barns välbefinnande, återfinns personal inom barnhälsovård (BHV) och elevhälsa. I detta kapitel beskrivs sjuksköterskors erfarenheter av att göra orosanmälningar såväl inom barnhälsovård som inom elevhälsans medicinska insats (EMI). Vi har särskilt närmat oss skolsköterskor inom skolors elevhälsoteam då deras anmälningar inte specifikt framgår i Socialstyrelsens statistik. Skolsköterskor besvarade en fråga om vad som avgjort, när de hyst oro för barn, att de beslutat sig för att göra orosanmälan till socialtjänsten.

    • Long-term diseases are today the leading cause of mortality worldwide and are estimated to be the leading cause of disability by 2020. Person-centered care (PCC) has been shown to advance concordance between care provider and patient on treatment plans, improve health outcomes and increase patient satisfaction. Yet, despite these and other documented benefits, there are a variety of significant challenges to putting PCC into clinical practice. Although care providers today broadly acknowledge PCC to be an important part of care, in our experience we must establish routines that initiate, integrate, and safeguard PCC in daily clinical practice to ensure that PCC is systematically and consistently practiced, i.e. not just when we feel we have time for it. In this paper, we propose a few simple routines to facilitate and safeguard the transition to PCC. We believe that if conscientiously and systematically applied, they will help to make PCC the focus and mainstay of care in long-term illness.

    • A porous network acts as transport paths for drugs through films for controlled drug release. The interconnectivity of the network strongly influences the transport properties. It is therefore important to quantify the interconnectivity and correlate it to transport properties for control and design of new films. This work presents a novel method for 3D visualisation and analysis of interconnectivity. High spatial resolution 3D data on porous polymer films for controlled drug release has been acquired using a focused ion beam (FIB) combined with a scanning electron microscope (SEM). The data analysis method enables visualisation of pore paths starting at a chosen inlet pore, dividing them into groups by length, enabling a more detailed quantification and visualisation. The method also enables identification of central features of the porous network by quantification of channels where pore paths coincide. The method was applied to FIB-SEM data of three leached ethyl cellulose (EC)/hydroxypropyl cellulose (HPC) films with different weight percentages. The results from the analysis were consistent with the experimentally measured release properties of the films. The interconnectivity and porosity increase with increasing amount of HPC. The bottleneck effect was strong in the leached film with lowest porosity.
