    • We consider the homogenization of the linear parabolic problem rho(x/epsilon(2))partial derivative(t)u(epsilon)(x,t) - del . (a(x/epsilon(1), t/epsilon(2)(1))del u(epsilon) (x,t)) = f(x,t) which exhibits a mismatch between the spatial scales in the sense that the coefficient a(x/epsilon(1), t/epsilon(2)(1)) of the elliptic part has one frequency of fast spatial oscillations, whereas the coefficient rho(x/epsilon(2)) of the time derivative contains a faster spatial scale. It is shown that the faster spatialmicroscale does not give rise to any corrector termand that there is only one local problemneeded to characterize the homogenized problem. Hence, the problem is not of a reiterated type even though two rapid scales of spatial oscillation appear.

  • High revision rate following artificial urethral sphincter implantation.

    • Abstract Objective. In severe cases of stress urinary incontinence (SUI), with sphincter dysfunction, the artificial urinary sphincter AMS 800™ may be the last solution. The purpose of this study was to evaluate the outcome of surgical intervention for SUI with the AMS 800 in patients who were treated at Sahlgrenska University Hospital, Gothenburg. The primary aim of the study was to determine the complications related to the operation. Material and methods. A retrospective follow-up was done by reviewing medical records. The material comprised 97 men, who underwent their first AMS 800 implantation between May 1997 and June 2010 at Sahlgrenska University Hospital. Results. The revision rate was 28%, including an infection rate of 3% and an erosion rate of 7%. The mean follow-up for revised patients was 3 years. The median time until revision was 1 year. Seventy-five per cent of all patients were satisfied with the operation at 6 months' follow-up. Radical prostatectomy was the reason behind incontinence in 84% of patients in this series. Conclusion. The results clearly demonstrate a need for revision procedures in a considerable proportion of patients implanted with an AMS 800 device. Patient satisfaction was high, but although this operation has extremely low mortality it has its complications and the system will need to be replaced in time.

  • Early Clinical Indicators of Addison’s Disease in Adults with Type 1 Diabetes: a Nationwide, Observational, Cohort Study

    • Context Patients with type 1 diabetes mellitus (T1DM) have an increased risk of Addison disease (AD) development, but prediction of those at risk is not possible. Objective To determine whether there are early clinical indicators that may denote the development of AD in adults with T1DM. Design Observational, matched-cohort study. Setting Patient data from Swedish national registries [National Diabetes Register (NDR), Inpatient Register, and Prescription Drug Register]. Participants All patients with T1DM diagnosed with concomitant AD (n = 66) among the 36,514 adult patients with T1DM in the NDR between 1998 and 2013. Each case was matched to five controls with T1DM alone (n = 330). Main Outcome Measures Clinical data and drug prescriptions were assessed prior to baseline (inclusion into the study) and prior to AD diagnosis. Analysis of covariance and estimated group proportions were used for comparisons. Results Prior to baseline, cases had a higher frequency of thyroid/antithyroid drug prescription than controls (9.1% vs 1.8%). Prior to AD diagnosis, cases had higher frequencies of diabetic retinopathy (12.1% vs 2.1%), infections requiring hospital admission (16.7% vs 2.1%), thyroid/antithyroid drug prescription (28.8% vs 7.0%), and glucagon prescription (18.2% vs 6.4%). There was no difference in glycated Hb between the groups prior to baseline or prior to AD diagnosis. Conclusions These data suggest that medical treatment of thyroid disease, a severe infection, and glucagon prescription for severe hypoglycemia should raise the suspicion of AD development in adults with T1DM. Development of diabetic retinopathy might also be associated with glucocorticoid deficiency and the development of AD among patients with T1DM.

    • We study the homogenization of a certain linear hyperbolic-parabolic problem exhibiting two rapid spatial scales {ε; ε2}. The homogenization is performed by means of evolution multiscale convergence, a generalization of the concept of two-scale convergence to include any number of scales in both space and time. In particular we apply a compactness result for gradients. The outcome of the homogenization procedure is that we obtain a homogenized problem of hyperbolic-parabolic type together with two elliptic local problems, one for each rapid scale, for the correctors.
