    • A substantial excess risk of lymphomas and nonmelanoma skin cancer has been demonstrated following organ transplantation. Large sample size and long follow-up time may, however, allow more accurate risk estimates and detailed understanding of long-term cancer risk. The objective of the study was to assess the risk of cancer following organ transplantation. A nationwide cohort study comprising 5931 patients who underwent transplantation of kidney, liver or other organs during 1970-1997 in Sweden was conducted. Complete follow-up was accomplished through linkage to nationwide databases. We used comparisons with the entire Swedish population to calculate standardised incidence ratios (SIRs), and Poisson regression for multivariate internal analyses of relative risks (RRs) with 95% confidence intervals (CI). Overall, we observed 692 incident first cancers vs 171 expected (SIR 4.0; 95% CI 3.7-4.4). We confirmed marked excesses of nonmelanoma skin cancer (SIR 56.2; 95% CI 49.8-63.2), lip cancer (SIR 53.3; 95% CI 38.0-72.5) and of non-Hodgkin's lymphoma (NHL) (SIR 6.0; 95% CI 4.4-8.0). Compared with patients who underwent kidney transplantation, those who received other organs were at substantially higher risk of NHL (RR 8.4; 95% CI 4.3-16). Besides, we found, significantly, about 20-fold excess risk of cancer of the vulva and vagina, 10-fold of anal cancer, and five-fold of oral cavity and kidney cancer, as well as two- to four-fold excesses of cancer in the oesophagus, stomach, large bowel, urinary bladder, lung and thyroid gland. In conclusion, organ transplantation entails a persistent, about four-fold increased overall cancer risk. The complex pattern of excess risk at many sites challenges current understanding of oncogenic infections that might become activated by immunologic alterations.

    • ObjectiveGuided Internet-based cognitive behaviour therapy (ICBT) for panic disorder has been shown to be efficacious in several randomized controlled trials. However, the effectiveness of the treatment when delivered within routine psychiatric care has not been studied. The aim of this study was to investigate the effectiveness of ICBT for panic disorder within the context of routine psychiatric care. MethodWe conducted a cohort study investigating all patients (n=570) who had received guided ICBT for panic disorder between 2007 and 2012 in a routine care setting at an out-patient psychiatric clinic providing Internet-based treatment. The primary outcome measure was the Panic Disorder Severity Scale-Self-report (PDSS-SR). ResultsParticipants made large improvements from screening and pretreatment assessments to posttreatment (Cohen's d range on the PDSS-SR=1.07-1.55). Improvements were sustained at 6-month follow-up. ConclusionThis study suggests that ICBT for panic disorder is as effective when delivered in a routine care context as in the previously published randomized controlled trials.

    • Familial amyloid polyneuropathy (FAP) associated with transthyretin (TTR) mutations is the commonest type of hereditary amyloidosis. Plasma TTR is produced almost exclusively in the liver and orthotopic liver transplantation is the only available treatment, although the clinical outcome varies. Serum amyloid P component (SAP) scintigraphy is a method for identifying and quantitatively monitoring amyloid deposits in vivo, but it has not previously been used to study the outcome of visceral amyloid deposits in FAP following liver transplantation. Whole body scintigraphy following injection of iodine-123 labelled SAP was performed in 17 patients with FAP associated with TTR Met30 and in five asymptomatic gene carriers. Follow-up studies were performed in ten patients, eight of whom had undergone orthotopic liver transplantation 1-5 years beforehand. There was abnormal uptake of 123I-SAP in all FAP patients, including the kidneys in each case, the spleen in five cases and the adrenal glands in three cases. Renal amyloid deposits were also present in three of the asymptomatic carriers. Follow-up studies 1-5 years after liver transplantation showed that there had been substantial regression of the visceral amyloid deposits in two patients and modest improvement in three cases. The amyloid deposits were unchanged in two patients. In conclusion, 123I-SAP scintigraphy identified unsuspected visceral amyloid in each patient with FAP due to TTR Met30. The universal presence of renal amyloid probably underlies the high frequency of renal failure that occurs in FAP following liver transplantation. The variable capacity of patients to mobilise amyloid deposits following liver transplantation may contribute to their long-term clinical outcome.

    • The most well-known example of an ordered quantum state—superconductivity—is caused by the formation and condensation of pairs of electrons. Fundamentally, what distinguishes a superconducting state from a normal state is a spontaneously broken symmetry corresponding to the long-range coherence of pairs of electrons, leading to zero resistivity and diamagnetism. Here we report a set of experimental observations in hole-doped Ba1−xKxFe2As2. Our specific-heat measurements indicate the formation of fermionic bound states when the temperature is lowered from the normal state. However, when the doping level is x ≈ 0.8, instead of the characteristic onset of diamagnetic screening and zero resistance expected below the superconducting phase transition, we observe the opposite effect: the generation of self-induced magnetic fields in the resistive state, measured by spontaneous Nernst effect and muon spin rotation experiments. This combined evidence indicates the existence of a bosonic metal state in which Cooper pairs of electrons lack coherence, but the system spontaneously breaks time-reversal symmetry. The observations are consistent with the theory of a state with fermionic quadrupling, in which long-range order exists not between Cooper pairs but only between pairs of pairs.

  • Unusual Interplay between Superconductivity and Field-Induced Charge Order in YBa2Cu3Oy

    • We present a detailed study of the temperature (T) and magnetic field (H) dependence of the electronic density of states (DOS) at the Fermi level, as deduced from specific heat and Knight shift measurements in underdoped YBa2Cu3Oy. We find that the DOS becomes field independent above a characteristic field H-DOS, and that the H-DOS (T) line displays an unusual inflection near the onset of the long-range 3D charge-density wave order. The unusual S shape of H-DOS (T) is suggestive of two mutually exclusive orders that eventually establish a form of cooperation in order to coexist at low T. On theoretical grounds, such a collaboration could result from the stabilization of a pair-density wave state, which calls for further investigation in this region of the phase diagram.

    • We study Fiske steps in small Bi2Sr2CaCu2O8+x mesa structures, containing only a few stacked intrinsic Josephson junctions. Careful alignment of magnetic field prevents penetration of Abrikosov vortices and facilitates observation of a large variety of high-quality geometrical resonances, including superluminal with velocities larger than the slowest velocity of electromagnetic waves. A small number of junctions limits the number of resonant modes and allows accurate identification of modes and velocities. It is shown that superluminal geometrical resonances can be excited by subluminal fluxon motion and that flux flow itself becomes superluminal at high magnetic fields. We argue that observation of high-quality superluminal geometrical resonances is crucial for realization of the coherent flux-flow oscillator in the terahertz frequency range.

    • We perform a detailed comparison of magnetotunneling in conventional low-Tc Nb/Al-AlOx/Nb junctions with that in slightly overdoped Bi2−yPbySr2CaCu2O8+δ [Bi(Pb)-2212] intrinsic Josephson junctions and with microscopic calculations. It is found that both types of junctions behave in a qualitatively similar way. Both magnetic field and temperature suppress superconductivity in the state-conserving manner. This leads to the characteristic sign change of tunneling magnetoresistance from the negative at the subgap to the positive at the sum-gap bias. We derived theoretically and verified experimentally scaling laws of magnetotunneling characteristics and employ them for accurate extraction of the upper critical field Hc2. For Nb an extended region of surface superconductivity at Hc2c3 is observed. The parameters of Bi(Pb)-2212 were obtained from self-consistent analysis of magnetotunneling data at different levels of bias, dissipation powers, and for different mesa sizes, which precludes the influence of self-heating. It is found that Hc2(0) for Bi(Pb)-2212 is ≃T→Tc T and decreases significantly at T→Tc. The amplitude of subgap magnetoresistance is suppressed exponentially at T>Tc/2, but remains negative, although very small, above Tc. This may indicate the existence of an extended fluctuation region, which, however, does not destroy the general second-order type of the phase transition at Tc.
