- Neuroendocrine neoplasms (NENs) are a heterogeneous family of uncommon tumours with challenging diagnosis, clinical management and unique needs that almost always requires a multidisciplinary approach. In the absence of guidance from the scientific literature, along with the rapidly changing data available on the effect of COVID-19, we report how 12 high-volume NEN centres of expertise in 10 countries at different stages of the evolving COVID-19 global pandemic along with members of international neuroendocrine cancer patient societies have suggested to preserve high standards of care for patients with NENs. We review the multidisciplinary management of neuroendocrine neoplasms during the COVID-19 pandemic, and we suggest potential strategies to reduce risk and aid multidisciplinary treatment decision-making. By sharing our joint experiences, we aim to generate recommendations for proceeding to other institutions facing the same challenges. (C) 2020 Elsevier Ltd. All rights reserved.

In the Sea of Influence : A World System Perspective of the Nicobar Islands

- From a “world system” perspective, this monograph describes the processes by which the Nicobar Islands became integrated into the global economy. Situated some 1,200 km off the east coast of India in the Bay of Bengal, the islands are home to an indigenous population of approximately 40,000 inhabitants who draw their sustenance from horticulture, pig rearing, fishing and, more recently, copra production. The location of the Nicobar Islands on an ancient trade route to southeast Asia has had inevitable consequences for both the inhabitants and the islands’ ecology; the pace of change increasing with the European colonisation of Asia. The islands are no longer a source of food and shelter to passing vessels as they used to be in former times, but are positioned, despite present protectionist measures, on the disadvantaged side of an exploitative relationship through an unequal exchange of resources within the modern world system. In drawing linkages between the world system perspective and environmental change, this monograph emphasises a shift from monetary indicators to biophysical indicators. Based on archival material and prolonged fieldwork, the monograph is a first detailed account of the colonial and economic history of the Nicobar Islands, and hence of interest not only to the scientific community, but to the general reader as well.

Patient-Reported Burden of a Neuroendocrine Tumor (NET) Diagnosis : Results From the First Global Survey of Patients With NETs

- PURPOSE: Despite the considerable impact of neuroendocrine tumors (NETs) on patients' daily lives, the journey of the patient with a NET has rarely been documented, with published data to date being limited to small qualitative studies. NETs are heterogeneous malignancies with nonspecific symptomology, leading to extensive health care use and diagnostic delays that affect survival. A large, international patient survey was conducted to increase understanding of the experience of the patient with a NET and identify unmet needs, with the aim of improving disease awareness and care worldwide.METHODS: An anonymous, self-reported survey was conducted (online or on paper) from February to May 2014, recruiting patients with NETs from > 12 countries as a collaboration between the International Neuroendocrine Cancer Alliance and Novartis Pharmaceuticals. Survey questions captured information on sociodemographics, clinical characteristics, NET diagnostic experience, disease impact/management, interaction with medical teams, NET knowledge/awareness, and sources of information. This article reports the most relevant findings on patient experience with NETs and the impact of NETs on health care system resources.RESULTS: A total of 1,928 patients with NETs participated. A NET diagnosis had a substantially negative impact on patients' personal and work lives. Patients reported delayed diagnosis and extensive NET-related health care resource use. Patients desired improvement in many aspects of NET care, including availability of a wider range of NET-specific treatment options, better access to NET experts or specialist centers, and a more knowledgeable, better-coordinated/-aligned NET medical team.CONCLUSION: This global patient-reported survey demonstrates the considerable burden of NETs with regard to symptoms, work and daily life, and health care resource use, and highlights considerable unmet needs. Further intervention is required to improve the patient experience among those with NETs.

Regenerative economics at the service of islands: Assessing the socio-economic metabolism of Samothraki in Greece

- For many islands, the answer to the question "why a locally, self-sustaining, and regenerative economy is needed?" is clear. The struggle often lies in the "how". Here, we argue that tools from regenerative economics, which follow an island economy-as-an-organism analogy, offer valuable and complementary insights to socio-metabolic research. Indicators from flow-based and information-based ecological network analysis can quantify proper- ties of an islands socio-economic metabolism (SEM) which are related to cycling, resilience, and degree of mutualism, among others. To illustrate the applicability of these methods, we select Samothraki in Greece as a case study. Results show that over the years the island became very efficient in streamlining imported resources, experiencing physical growth as indicated by a substantial increase in its total material throughput. This growth was attributed to a high degree of order (i.e., network efficiency) endowed by the constraining (ordered) part of the linear structure of the islands SEM. The disordered part of its SEM which is related to resilience, played a much smaller role which became progressively more important over the years, albeit to a limited degree. While the island exhibits an increasing trend in its robustness, its value over the years studied was well below what is typically observed for healthy natural ecosystems, and its current SEM has a very low ability to generate internal flow activity and cycling of resources per unit input. This limited robustness is due to the islands dependency on imports but also due to its linear SEM which had a very small number of feedback loops in its network. A scenario analysis showed that a reticulated network structure would theoretically endow the island with increased resilience, and hence robustness, by allowing for more internal resource flow activity to be circulated as regen- erative re-investment. This article highlights that methods from regenerative economics can be used as diagnostic tools to assess and monitor the impact of strategies related to circular economy interventions on network properties, and to illuminate their effect on the regenerative potential of islands.
