  • Surgical Replacement of Implantable Pulse Generator in Deep Brain Stimulation : Adverse Events and Risk Factors in a Multicenter Cohort

    • Background: Deep brain stimulation (DBS) is a growing treatment modality, and most DBS systems require replacement of the implantable pulse generator (IPG) every few years. The literature regarding the potential impact of adverse events of IPG replacement on the longevity of DBS treatments is rather scarce. Objective: To investigate the incidence of adverse events, including postoperative infections, associated with IPG replacements in a multicenter cohort. Methods: The medical records of 808 patients from one Australian and five Swedish DBS centers with a total of 1,293 IPG replacements were audited. A logistic regression model was used to ascertain the influence of possible predictors on the incidence of adverse events. Results: The overall incidence of major infections was 2.3% per procedure, 3.7% per patient and 1.7% per replaced IPG. For 28 of 30 patients this resulted in partial or complete DBS system removal. There was an increased risk of infection for males (OR 3.6, p = 0.026), and the risk of infection increased with the number of prior IPG replacements (OR 1.6, p < 0.005). Conclusions: The risk of postoperative infection with DBS IPG replacement increases with the number of previous procedures. There is a need to reduce the frequency of IPG replacements.

    • Although sled dogs are one of the most specialized groups of dogs, their origin and evolution has received much less attention than many other dog groups. We applied a genomic approach to investigate their spatiotemporal emergence by sequencing the genomes of 10 modern Greenland sled dogs, an similar to 9500-year-old Siberian dog associated with archaeological evidence for sled technology, and an similar to 33,000-year-old Siberian wolf. We found noteworthy genetic similarity between the ancient dog and modern sled dogs. We detected gene flow from Pleistocene Siberian wolves, but not modern American wolves, to present-day sled dogs. The results indicate that the major ancestry of modern sled dogs traces back to Siberia, where sled dog-specific haplotypes of genes that potentially relate to Arctic adaptation were established by 9500 years ago.

    • No previous study has evaluated patients attitudes towards inclusion in an ongoing cardiac arrest clinical trial. The aim of this study was to assess patientś willingness and motives to participate in the ongoing randomized controlled drug trial "Vasopressin and Steroids in addition to Adrenaline in cardiac arrest" (VAST-A trial) in case of an in-hospital cardiac arrest (IHCA).Hospitalized patients, men≥18 and women≥50years, were asked for informed consent for inclusion in the VAST-A trial in case of an IHCA, the reason for approving or declining inclusion in the trial and baseline characteristics.Patients admitted to hospital were asked to give informed consent of inclusion in VAST-A in case of an IHCA during their hospital stay. Patients were also asked why they approved or declined inclusion as well as baseline characteristics questions.1,064 patients were asked about willingness to participate in the VAST-A trial, of these 902 (84.8%) patients approved inclusion. A subgroup of 411 patients were, except willingness, also asked about motives to participate or not and basic characteristics. The main reason for approving inclusion was to contribute to research (n=328, 83.9%). The main reason for declining inclusion was concerns regarding testing the drug treatment (n=6, 30%).Among hospitalized patients the vast majority gave informed consent to inclusion in an ongoing randomized cardiac arrest drug trial. The main reason for approving inclusion was to contribute to research.

    • Fast analog to digital conversion with only one bit per sample does not only make high sampling rates possible but also reduces the required hardware complexity. For short data buffers or block lengths, it has been shown that tone frequency estimators can be implemented by a simple table look-up. In this paper we present an analysis of such tables using the Hadamard transform. As an outcome of the analysis, we propose a class of nonlinear estimators of low complexity. Their performance is evaluated using numerical simulations. Comparisons are made with the proper Cramer-Rao bound and with the table look-up approach.

    • The paper compares the Bronze Age ship settings of Gotland with the vessels portrayed in rock carvings on the Scandinavian mainland. It also makes comparisons with the drawings of vessels on decorated metalwork of the same period. It considers their interpretation in relation to two approaches taken to the depictions of ships in other media. One concerns the use of boats to transport the sun, while the other emphasises the close relationship between seagoing vessels and the dead. A third possibility concerns the distinctive organisation of prehistoric communities on Gotland. It seems possible that the largest of the ship settings were equivalent to the Bronze Age cult houses found on the mainland and that they may even have represented the island itself.

    • The rock art of southern Scandinavia is characterized by depictions of watercraft. The majority are closeto the coast, and they have been the primary focus of research. Less attention has been paid to similarrepresentations associated with two large inland lakes in southern Sweden. In this article we present theresults of fieldwork around Lake Vänern and Lake Vättern and consider the relationship of this rock artto the better-known images on the coast. We explore the practicalities of navigating between the sea andthe interior and suggest that there was an important contrast between an early eastern sphere extendingto Lake Vättern from the Baltic and a later western sphere connecting Lake Vänern with the Atlantic.

    • Palaeoecological analysis of peat deposits from a small bog at Lingarden, southern Sweden, have been used to examine the nature and timing of vegetation changes and anthropogenic activity associated with a nearby rock carving located close to the edge of the wetland. This study is the first of its type to investigate the environmental context of rock carvings in southern Sweden. Debate has tended to focus on chronology and iconography, with little consideration of the environmental relationships of rock carvings and how vegetation may help construct a site within its surrounding landscape. The pollen evidence from Lingarden demonstrates that the rock carving was located in an isolated semi-wooded setting during the late Bronze Age. This is in stark contrast to several other pollen studies from the Bjare Peninsula that record widespread woodland clearance and agricultural activity from the late Neolithic Bronze Age transition. The results of this study support hypotheses that suggest complex rock carvings, such as Lingarden, were separated from settled areas. This sense of separation and isolation is reinforced by the vegetation surrounding the rock carving. This paper also discusses the relationship between charcoal in the pollen sequence and evidence that the decorated outcrop had been burnt. (c) 2010 Elsevier Ltd. All rights reserved.
