    • Den idéburna organisationen A Non Smoking Generation utvecklade under 2010–2017 ett kunskaps- och teoribaserat tobaksförebyggande flerkomponentsprogram för grund- och gymnasieskola tillsammans med en tvärvetenskaplig rådgivande forskargrupp. Här beskrivs programutvecklingsarbetet och genomförandet i fyra svenska kommuner, graden av engagemang och stöd för genomförandet samt elevernas syn på policy och lagar rörande tobak. Programmets teoretiska ramverk utgick från teorier och kunskap om förändringsberedskap, risk- och skyddsfaktorer, implementering, samt den processmodell som utvecklades och tillämpades i projektet. Alla skolor i kommunerna gick under projekttiden från att ha en lägre förändringsberedskap att arbeta tobaksförebyggande till en högre. I slutet av projektet var de tobaksspecifika skyddsfaktorerna inkluderade i tobakspolicys som tillämpades och andelen elever som kände till tobakspolicyn hade ökat. Resultaten visade också att kommunernas skolor hade kunnat tillämpa och kommit långt i arbetet med processmodellen. Av intervjuerna framkom att de var mycket nöjda med modellen och ansåg att den var lätt att arbeta utifrån och hålla fast vid i framtida arbete.

    • A survey was undertaken of the policies and practices concerning organisation and implementation of health education and health promotion in schools in 151 of the total 213 Local Educational Areas (LEAs) in Stockholm county in 1990. Health education was included in the workplan of 49% of the responding LEAs, while 39% of respondents had a local action programme or guidelines for health education. Topic areas taught to all pupils and considered most important included alcohol, drug abuse, smoking, sex education, bullying, nutrition and physical exercise. Most senior level schools (55–83%) had written policies concerning pupils using alcohol, drugs or smoking in school, and 68% of LEAs had restrictions on staff smoking in school. Continuing health education was desired by 87% of the respondents. A written programme/plan regardings health education was identified as an important indicator of interest and commitment in health education and health policy issues by the local school.

    • The Stockholm Cancer Prevention Program is to our knowledge the first community intervention program in the world aiming of a reduction in the incidence of cancer. A community diagnosis model has been used in the planning. A thorough description of different organizations in the local communities of Stockholm has been made. A model for describing the organizations or collaborators has been worked out, including description of the organizations and its environment, objectives of activities, possible activities to reach the objectives, resources needed, incentives for collaborative partners, formative evaluation criteria and anticipated problems and solutions. The main method of description has been by use of keyperson interviews. This was found to be a good method to reach commitment to the program.

    • Background: Multiple control areas and time-series analyses have been recommended for effect evaluations of community-based health promotion. Large fluctuations, maybe due to chance, among the areas and over the years might obscure the intervention effect. Methods: A quasi-experimental time-series analysis with several control areas was performed as an effect evaluation of a community-based elderly safety promotion program. The program was implemented during 199599 in a community in the Stockholm Metropolitan area (population 65 years: 5500; number of first hip fractures in 1995: 60). Four control areas were selected based on similar hip fracture-related characteristics as the intervention community, complemented with two larger control areas. The time series covered 6 years pre-intervention (199095) and 6 years post-intervention (19962001). The study population was divided into two age groups and gender, resulting in 28 panels. The first hip fracture incidence was obtained from the Swedish national in-patient register. Results: The time series revealed no discernible pattern, and conventional analyses showed no conclusive results. A multivariate analysis, examining the time trends by employing the intra-annual and intra-panel variance, revealed the underlying trends in hip fracture rates. Comparisons between predicted numbers of hip fractures in the intervention and control areas was enabled, which resulted in 14 less hip fractures in the intervention community than expected from the control communities. If one extreme value was altered, the result changed considerably. Conclusion: Effect evaluations of community-based health promotion programs using time-series from small communities might give faulty results, if statistical modelling is not employed.
