  • Establishment and utility of SwedAD : a nationwide Swedish registry for patients with atopic dermatitis receiving systemic pharmacotherapy

    • SwedAD, a Swedish nationwide registry for patients with atopic dermatitis receiving systemic pharmacotherapy, was launched on 1 September 2019. We describe here the establishment of a user-friendly registry to the benefit of patients with atopic dermatitis. By 5 November 2022, 38 clinics had recorded 931 treatment episodes in 850 patients with an approximate national coverage rate of 40%. Characteristics at enrolment included median Eczema Area and Severity Index (EASI) 10.2 (interquartile range 4.0, 19.4), Patient-Oriented Eczema Measure (POEM) 18.0 (10.0, 24.0), Dermatology Life Quality Index (DLQI) 11.0 (5.0, 19.0) and Peak Itch Numerical Rating Scale-11 (NRS-11) 6.0 (3.0, 8.0). At 3 months, median EASI was 3.2 (1.0, 7.3) and POEM, DLQI, and NRS-11 were improved. Regional coverage varied, reflecting the distribution of dermatologists, the ratio of public to private healthcare, and difficulties in recruiting certain clinics. This study highlights the importance of a nationwide registry when managing systemic pharmacotherapy of atopic dermatitis.

    • There is a need for a cheaper method of producing bleached TMP and CTMP without impairing the properties of the pulp. This is made possible by the present invention, which relates to a method of producing said pulps in which finely divided lignocellulosic material is defibrated after pretreatment and the resultant pulp is bleached in the form of a suspension with bleaching agent that increases the anionic charge of the pulp fibres, whereafter the pulp suspension is cleaned and then possibly further treated, for instance bleached, characterised in that the defibration is effected with the aid of a) a refiner that has counter-rotating refining discs while using an extra high temperature (HTDD), or b) a refiner that has a single rotating refining disc(s) having a single refining zone or two parallel refining zones, while using an extra high temperature and while rotating the disc(s) at an extra high speed (RTS), and in that the cleaning treatment comprises first screeming the pulp suspension and them hydrocyclone cleaning said suspension, wherewith the reject obtained with each cleaning process is treated individually and the treated reject is returned to the advancing pulp suspension.

    • Backgroundp63 proteins are important in formation of the oral mucosa. Normal oral mucosa shows a balance between the six protein isoforms, whereas an imbalance between them is seen in squamous cell carcinomas (SCC). There is controversy over the clinical impact of p63 in SCC, which may relate to different expression in different areas. In addition, p63 isoforms can act as p53-like molecules (TAp63) or can inhibit p53 functions (Np63) and expression of these isoforms varies in different tumours. Here, we chose to concentrate on the most common intra-oral sub-site, SCC of the mobile tongue. MethodsTotal p63, Np63 and TAp63 were analysed separately using immunohistochemistry. The percentage of cells and intensity of expression of different isoforms of p63 was evaluated using a quick score method and correlated with clinical data in a group of 87 patients with tongue SCC. ResultsAll tumours expressed p63 in at least 60% of the cells when using two different antibodies detecting all 6 isoforms. p63 expression correlated significantly with 2-year survival (P=0.018), with fewer patients surviving 2years if their tumours expressed p63 with strong intensity in at least 80% of the cells (quick score 18). Looking at 5-year survival, this was even more emphasized. Np63 was expressed in all tumours, whereas expression of TAp63 was seen only in 59/87 patients, usually at very low levels. ConclusionsBased on the present data, we recommend using expression of p63 as an additional factor contributing prognostic information in analysis of SCC in the tongue.

  • Utilization of the natural variation in wood properties–a comparison between pilot plant and mill scale trials

    • Four different pulpwood classes of Norway spruce (Picea abies (L.) Karst). were tested in mill scale production of TMP at Hallsta Papermill. The objectives of the trial were to evaluate both rather extreme and more traditional roundwood classes in a full-scale TMP-plant with respect to pulp properties and energy requirements. The experiment was partly based on the results from the proceeding pilot plant trial within the EuroFiber project. The results generally confirmed the findings in the pilot plant trial. Young wood with a high proportion of juvenile wood gave a pulp with attractive optical properties, but somewhat lower tear index and higher energy consumption. The other extreme, sawmill chips from old trees of heavy dimensions, required lower energy input and gave higher tear index, but inferior optical properties. The two more traditional classes gave pulps with properties between the two extremes.

    • Despite the promise of RNA interference (RNAi) and its potential, e.g. for use in cancer therapy, several technical obstacles must first be overcome. The major hurdle of RNAi-based therapeutics is to deliver nucleic acids across the cell's plasma membrane. This study demonstrates that exosome vesicles derived from humans can deliver short interfering RNA (siRNA) to human mononuclear blood cells. Exosomes are nano-sized vesicles of endocytic origin that are involved in cell-to-cell communication, i.e. antigen presentation, tolerance development and shuttle RNA (mainly mRNA and microRNA). Having tested different strategies, an optimized method (electroporation) was used to introduce siRNA into human exosomes of various origins. Plasma exosomes (exosomes from peripheral blood) were used as gene delivery vector (GDV) to transport exogenous siRNA to human blood cells. The vesicles effectively delivered the administered siRNA into monocytes and lymphocytes, causing selective gene silencing of mitogen-activated protein kinase 1. These data suggest that human exosomes can be used as a GDV to provide cells with heterologous nucleic acids such as therapeutic siRNAs.
