    • Quantitative analysis of electrocardiographic ST-segment/heart rate relationship (ST/HR loop) during early recovery phase of exercise stress test provides a sensitive tool for the detection of coronary artery disease (CAD). This study evaluates the effect of data sampling frequency on the diagnostic performance of ST/HR loop in 1876 patients undergoing a routine exercise test on a bicycle ergometer. CAD was verified angiographically in 668 patients and excluded by coronary angiography (n = 119), myocardial scintigraphy (n = 250) and on clinical grounds (n = 839) in 1208 patients. The normalized ST/HR loop area was calculated in all cases by integration of ST-segment amplitude difference from the end of exercise to the end of the first 3 min of recovery period over HR and dividing the integral by the HR difference over the integration period. The effect of different sampling rates (one, two and five samples per minute) on the CAD discrimination ability of ST/HR loop area was subsequently evaluated using receiver operating characteristic (ROC) curves. Reduction in ST/HR data sampling frequency from two to one sample per minute resulted in a significantly decreased diagnostic performance of the ST/HR loop whereas no differences in CAD discrimination capacity were observed between sampling frequencies of two and five samples per minute. The choice of ST/HR data sampling frequency may have a significant impact on the CAD diagnostic ability of the ST/HR loop. The use of sampling frequency below two samples per minute results in a significantly diminished diagnostic performance, a fact that should be taken into consideration when employing ST/HR diagnostic procedures.

    • The temporal distribution of the diagnostic information for the detection of coronary artery disease (CAD) provided by exercise-induced electrocardiographic (ECG) ST-segment amplitude changes in different ECG leads in men and women has not been fully investigated. To shed further light in this area, 1877 electrocardiograms selected from 8322 patients undergoing a routine exercise test on a bicycle ergometer were evaluated. ST-segment amplitude and the difference between heart rate-matched recovery and exercise ST-segment amplitudes (ST/HR difference) were measured. Coronary artery disease was verified angiographically in 669 patients and excluded in 1208 patients by angiography (n = 119), by myocardial scintigraphy (n = 250), or on clinical grounds (n = 839). The diagnostic performance of the 2 ECG methods used was assessed by constructing receiver operating characteristic curves for each sampling point every 12 seconds during 10 minutes of recovery as well as the last 4 minutes of exercise for the ST-segment amplitude. ST-segment amplitude performed better after exercise than during exercise and best within the first 2 minutes of recovery. Its diagnostic ability did not differ from the ST-amplitude hysteresis assessed by the difference between recovery ST-segment amplitude and exercise ST-segment amplitude at matched heart rate. Both methods performed better in men and the diagnostic information appeared mainly in leads I, -aVR, II, V-4, V-5, and V-6. The best discrimination of CAD is provided by analysis of ST-segment amplitude changes in 6 specific leads early during the recovery phase. This information should be targeted by exercise ECG diagnostic methods.

  • Brain activation while forming memories of fearful and neutral faces in women and men

    • Event-related functional MRI (fMRI) was used to assess brain activity during encoding of fearful and neutral faces in 12 women and 12 men. In a subsequent memory analysis, the authors separated successful from unsuccessful encoding of both types of faces, based on whether they were remembered or forgotten in a later recognition memory test. Overall, women and men recruited overlapping neural circuitries. Both sexes activated right-sided medial-temporal regions during successful encoding of fearful faces. Successful encoding of neutral faces was associated with left-sided lateral prefrontal and right-sided superior frontal activation in both sexes. In women, relatively greater encoding related activity for neutral faces was seen in the superior parietal and parahippocampal cortices. By contrast, men activated the left and right superior/middle frontal cortex more than women during successful encoding of the same neutral faces. These findings suggest that women and men use similar neural networks to encode facial information, with only subtle sex differences observed for neutral faces.

  • Age differences in arousal, perception of affective pictures, and emotional memory enhancement : Appraisal, Electrodermal activity, and Imaging data

    • In contrast to effortful cognitive functions, emotional functioning may remain stable or even be enhanced in older adults. It is unclear how affective functions in aging correspond to subjective experiences and physiological changes. In Study I, ratings of emotional intensity and neural activity to facial expressions using functional magnetic resonance imaging (fMRI) were analyzed in younger and older adults. Negative expressions resulted in increased neural activity in the right amygdala and hippocampus in younger adults, and increased activation in the right insular cortex in older adults. There were no age differences in subjective ratings. In Study II, subjective ratings of, and skin conductance response (SCR) to, neutral and negative pictures were studied. The ratings of negative pictures were higher for older adults compared to younger adults. SCRs increased in both age groups for the negative pictures, but magnitude of SCRs was significantly larger in younger adults. Finally, in Study III, emotional memory after a one-year retention interval was tested. The memory performance of both age groups was higher in response to negative pictures compared to neutral ones, although the performance was generally higher for younger adults. SCR at encoding was the better arousal predictor for memory, but only in younger adults. The results indicate age-related changes in affective processing. Age differences may involve a gradual shift from bottom-up processes, to more top-down processes. The results are discussed in a wider lifespan perspective taking into consideration the accumulated life experience of older adults.

  • Modulation of striatal dopamine D1 binding by cognitive processing

    • There is strong evidence that dopamine (DA) is implicated in higher-order cognitive functioning, but it remains controversial whether D1 receptor binding can be modified by cognitive activity. We examined striatal D1 binding potential (BP) in 20 younger (22-30 years) and 20 older (65-75 years) persons who underwent two [(11)C] SCH 23390 PET measurements, one while resting and one while performing a cognitive task taxing inhibitory functioning. The younger persons showed significant task-related BP reductions in sensorimotor, limbic, and associative striatum during cognitive activity compared to rest. Older persons showed no reliable BP reductions in any striatal subregion. These findings demonstrate that D1 receptor binding can be modified by cognitive activity in younger persons, but also provide novel evidence for the notion that human aging is associated not only with lower DA receptor density but also with altered modifiability of the DA system.
