  • Second international consensus report on gaps and opportunities for the clinical translation of precision diabetes medicine

    • Precision medicine is part of the logical evolution of contemporary evidence-based medicine that seeks to reduce errors and optimize outcomes when making medical decisions and health recommendations. Diabetes affects hundreds of millions of people worldwide, many of whom will develop life-threatening complications and die prematurely. Precision medicine can potentially address this enormous problem by accounting for heterogeneity in the etiology, clinical presentation and pathogenesis of common forms of diabetes and risks of complications. This second international consensus report on precision diabetes medicine summarizes the findings from a systematic evidence review across the key pillars of precision medicine (prevention, diagnosis, treatment, prognosis) in four recognized forms of diabetes (monogenic, gestational, type 1, type 2). These reviews address key questions about the translation of precision medicine research into practice. Although not complete, owing to the vast literature on this topic, they revealed opportunities for the immediate or near-term clinical implementation of precision diabetes medicine; furthermore, we expose important gaps in knowledge, focusing on the need to obtain new clinically relevant evidence. Gaps include the need for common standards for clinical readiness, including consideration of cost-effectiveness, health equity, predictive accuracy, liability and accessibility. Key milestones are outlined for the broad clinical implementation of precision diabetes medicine.

    • Stars form by accreting material from their surrounding disks. There is a consensus that matter flowing through the disk is channelled onto the stellar surface by the stellar magnetic field. This is thought to be strong enough to truncate the disk close to the corotation radius, at which the disk rotates at the same rate as the star. Spectro-interferometric studies in young stellar objects show that hydrogen emission (a well known tracer of accretion activity) mostly comes from a region a few milliarcseconds across, usually located within the dust sublimation radius1–3. The origin of the hydrogen emission could be the stellar magnetosphere, a rotating wind or a disk. In the case of intermediate-mass Herbig AeBe stars, the fact that Brackett γ (Brγ) emission is spatially resolved rules out the possibility that most of the emission comes from the magnetosphere4–6 because the weak magnetic fields (some tenths of a gauss) detected in these sources7,8 result in very compact magnetospheres. In the case of T Tauri sources, their larger magnetospheres should make them easier to resolve. The small angular size of the magnetosphere (a few tenths of a milliarcsecond), however, along with the presence of winds9,10 make the interpretation of the observations challenging. Here we report optical long-baseline interferometric observations that spatially resolve the inner disk of the T Tauri star TW Hydrae. We find that the near-infrared hydrogen emission comes from a region approximately 3.5 stellar radii across. This region is within the continuum dusty disk emitting region (7 stellar radii across) and also within the corotation radius, which is twice as big. This indicates that the hydrogen emission originates in the accretion columns (funnel flows of matter accreting onto the star), as expected in magnetospheric accretion models, rather than in a wind emitted at much larger distance (more than one astronomical unit).

  • The GRAVITY young stellar object survey: IV. The CO overtone emission in 51 Oph at sub-au scales

    • Context. 51 Oph is a Herbig Ae/Be star that exhibits strong near-infrared CO ro-vibrational emission at 2.3 μm, most likely originating in the innermost regions of a circumstellar disc. Aims. We aim to obtain the physical and geometrical properties of the system by spatially resolving the circumstellar environment of the inner gaseous disc. Methods. We used the second-generation Very Large Telescope Interferometer instrument GRAVITY to spatially resolve the continuum and the CO overtone emission. We obtained data over 12 baselines with the auxiliary telescopes and derive visibilities, and the differential and closure phases as a function of wavelength. We used a simple local thermal equilibrium ring model of the CO emission to reproduce the spectrum and CO line displacements. Results. Our interferometric data show that the star is marginally resolved at our spatial resolution, with a radius of ∼10.58 ± 2.65R·. The K-band continuum emission from the disc is inclined by 63° ± 1°, with a position angle of 116° ± 1°, and 4 ± 0.8 mas (0.5 ± 0.1 au) across. The visibilities increase within the CO line emission, indicating that the CO is emitted within the dust-sublimation radius. By modelling the CO bandhead spectrum, we derive that the CO is emitted from a hot (T = 1900-2800 K) and dense (NCO = (0.9-9) × 1021 cm-2) gas. The analysis of the CO line displacement with respect to the continuum allows us to infer that the CO is emitted from a region 0.10 ± 0.02 au across, well within the dust-sublimation radius. The inclination and position angle of the CO line emitting region is consistent with that of the dusty disc. Conclusions. Our spatially resolved interferometric observations confirm the CO ro-vibrational emission within the dust-free region of the inner disc. Conventional disc models exclude the presence of CO in the dust-depleted regions of Herbig AeBe stars. Ad hoc models of the innermost disc regions, that can compute the properties of the dust-free inner disc, are therefore required.

  • Clinical Manifestations, Immunological Characteristics and Genetic Analysis of Patients with Hyper-Immunoglobulin M Syndrome in Iran

    • <b><i>Background:</i></b> Hyper-immunoglobulin M (HIGM) syndrome is a rare heterogeneous group of primary immunodeficiency disorders characterized by low or absent serum levels of IgG and IgA along with normal or elevated serum levels of IgM. <b><i>Methods:</i></b> Clinical and immunological data were collected from the 75 patients’ medical records diagnosed in Children’s Medical Center affiliated to Tehran University Medical Sciences and other Universities of Medical Sciences in Iran. Among 75 selected patients, 48 patients (64%) were analyzed genetically using targeted and whole-exome sequencing. <b><i>Results:</i></b> The ratio of male to female was 2.9:1. The median age at the onset of the disease, time of diagnosis, and diagnostic delay were 10.5, 50, and 24 months, respectively. Pneumonia and lower respiratory tract infections (61.3%) were the most common complications. Responsible genes were identified in 35 patients (72.9%) out 48 ge<i>netically</i> analyzed patients. <i>Cluster of differentiation 40 ligand</i> gene was the most mutated gene observed in 24 patients (68.5%) followed by <i>activation-induced cytidine deaminase</i> gene in 7 patients, <i>lipopolysaccharide-responsive and beige-like anchor</i> (1 patient), <i>nuclear factor-kappa-B essential modulator</i> (1 patient), <i>phosphoinositide-3-kinase regulatory subunit 1</i> (1 patient), and <i>nuclear factor kappa B subunit 1</i> (1 patient) genes. Nineteen (25.3%) patients died during the study period, and pneumonia was the major cause of death occurred in 6 (31.6%) patients. <b><i>Conclusion:</i></b> Physicians in our country should carefully pay attention to respiratory tract infections and pneumonia, particularly in patients with a positive family history. Further investigations are required for detection of new genes and pathways resulting in HIGM phenotype.

  • Microstructure, fractal geometry and corrosion properties of CrN thin films : The effect of shot number and angular position

    • The effect of different plasma focus shots and angular positions (0° and 30°) on the properties of chromium nitride (CrN) coatings, deposited by a plasma focus (PF) device on stainless steel substrates, have been systematically investigated in this paper. The structural and morphological properties of CrN thin films were characterized by X-ray diffraction (XRD) and atomic force microscopy (AFM). Moreover, the corrosion behavior of the CrN thin films was investigated using the ‘c’ method. The XRD patterns demonstrated the growth of the polycrystalline structure composed of CrN/Cr2N nanograins and the enhanced crystallinity of the CrN coatings upon increasing the shot numbers. In addition, AFM results showed enhanced multifractal properties of the sample prepared at 0° angular position and a reducing trend in these properties for the layers prepared at 30° angular position. Moreover, they exhibited sharp hillock-like features on the surface, corresponding to the columnar growth of the CrN coatings, which further protruded as the number of shots increased. The results of the corrosion test showed that the resistance of stainless-steel substrate was improved by depositing the CrN coatings due to the formation of a passive and protective layer on its surface. Notably, ceramic CrN film, prepared through 10 shots at 30° angular position, showed the best corrosion resistance. Our strategy is advantageous for designing and manufacturing novel devices and instruments based on CrN corrosion resistant coating.

  • A Preliminary Assessment of Size-Fractionated Microplastics in Indoor Aerosol—Kuwait’s Baseline

    • The omnipresence of microplastic (MP) in various environmental samples, including aer-osols, has raised public health concerns; however, there is presently very limited information on MPs in indoor aerosol. This paper presents a unique dataset where smaller MPs have been sampled using a six-stage cascade impactor from indoor environments in Kuwait. The MP concentration in the indoor air varied between 3.2 and 27.1 particles m−3, and the relative MP concentration decreased linearly from the lowest to the highest size fraction. A significant effect of location was observed for the total number of MPs (F2,14 = 5.80, p = 0.02) and the inhalable fraction (F2,14 = 8.38, p = 0.005), while location had no effect on the respirable fraction (F2,14 = 0.54, p = 0.60). A significant effect of the type of air conditioning used was also observed for the total number of MPs (F2,19 = 5.58, p = 0.01) and the inhalable fraction (F2,19 = 6.45, p = 0.008), while location had no effect on the respirable fraction (F2,19 = 1.30, p = 0.30). For the total number of MPs and the inhalable fraction, the concentration was sig-nificantly higher for the split unit air-conditioning as compared to the central air-conditioning plants. The presence/absence of carpets had no significant effect on the MP concentrations (total: F1,19 = 4.08, p = 0.06; inhalable: F1,19 = 3.03, p = 0.10; respirable: F1,19 = 4.27, p = 0.05). The shape was dominantly fibers, with few fragments in lower size fractions. These datasets represent the first baseline information for Kuwait, and the smaller MPs in all the samples further underscore the need to develop standardized protocols of MP collection in the ≤2.5 µm fraction that can have more con-spicuous health implications. © 2022 by the authors.
