    • ObjectiveThe current study examined whether bullying victimization was associated with bringing a weapon to school, fully or partly mediated by feeling unsafe in school, negative future education orientation, and skipping school.MethodData from 6199 students (12–18 years old), who had filled out the 2017 School Crime Supplement to the National Crime Victimization Survey, were analyzed.ResultsThe results showed a positive association between bullying victimization and bringing a weapon to school. The link was both direct and indirect through the mediating roles of feeling unsafe in school, having a negative future education orientation, and skipping school.ConclusionThus, victims of bullying tended to feel unsafe in school, have a negative future education orientation, and skip school—all of which were positively associated with bringing a weapon to school.

  • Longitudinal associations between psychedelic use and psychotic symptoms in the United States and the United Kingdom

    • It has long been speculated that psychedelic use could provoke the onset of psychosis, but there is little evidence to support this conjecture. Using a longitudinal research design with samples representative of the US and UK adult populations with regard to sex, age, and ethnicity (n = 9732), we investigated associations between psychedelic use and change in the number of psychotic symptoms during the two-month study period. In covariate-adjusted regression models, psychedelic use during the study period was not associated with a change in the number of psychotic symptoms unless it interacted with a personal or family history of bipolar disorder, in which case the number of symptoms increased, or with a personal (but not family) history of psychotic disorders, in which case the number of symptoms decreased. Taken together, these findings indicate that psychedelic use may affect psychotic symptoms in individuals with a personal or family history of certain disorders characterized by psychotic symptomatology.

  • Association of Vitamin B-12, Folate, and Sulfur Amino Acids With Brain Magnetic Resonance Imaging Measures in Older Adults A Longitudinal Population-Based Study

    • IMPORTANCE Vitamin B-12, folate, and sulfur amino acids may be modifiable risk factors for structural brain changes that precede clinical dementia. OBJECTIVE To investigate the association of circulating levels of vitamin B-12, red blood cell folate, and sulfur amino acids with the rate of total brain volume loss and the change in white matter hyperintensity volume as measured by fluid-attenuated inversion recovery in older adults. DESIGN, SETTING, AND PARTICIPANTS The magnetic resonance imaging subsample of the Swedish National Study on Aging and Care in Kungsholmen, a population-based longitudinal study in Stockholm, Sweden, was conducted in 501 participants aged 60 years or older who were free of dementia at baseline. A total of 299 participants underwent repeated structural brain magnetic resonance imaging scans from September 17, 2001, to December 17, 2009. MAIN OUTCOMES AND MEASURES The rate of brain tissue volume loss and the progression of total white matter hyperintensity volume. RESULTS In the multi-adjusted linear mixed models, among 501 participants (300 women [59.9%]; mean [SD] age, 70.9 [9.1] years), higher baseline vitamin B-12 and holotranscobalamin levels were associated with a decreased rate of total brain volume loss during the study period: for each increase of 1 SD, beta (SE) was 0.048 (0.013) for vitamin B-12 (P < .001) and 0.040 (0.013) for holotranscobalamin (P = .002). Increased total homocysteine levels were associated with faster rates of total brain volume loss in the whole sample (beta [SE] per 1-SD increase, -0.035 [0.015]; P = .02) and with the progression of white matter hyperintensity among participants with systolic blood pressure greater than 140mmHg (beta [SE] per 1-SD increase, 0.000019 [0.00001]; P = .047). No longitudinal associations were found for red blood cell folate and other sulfur amino acids. CONCLUSIONS AND RELEVANCE This study suggests that both vitamin B-12 and total homocysteine concentrations may be related to accelerated aging of the brain. Randomized clinical trials are needed to determine the importance of vitamin B-12 supplementation on slowing brain aging in older adults.

  • Emotional mental imagery and the reduction of fear within the mind’s eye

    • Mental imagery refers to sensory-perceptual experiences in the absence of external sensory input. Emotional mental imagery (i.e., imagery with emotional content) is a key feature in many mental disorders, such as the image-based intrusive memories of trauma in posttraumatic stress disorder (PTSD). However, mental imagery can also be a vehicle for emotional change. In imaginal exposure, fear-provoking events are revisited using mental imagery. Imaginal exposure is a core component in evidence-based therapies for anxiety and PTSD. Treatment development is needed, as effects are many times insufficient, accessibility is low, and the treatment is not well-tolerated by some patients. The aim of this thesis was to increase knowledge of underlying mechanisms of imaginal exposure and improve our understanding of emotional mental imagery. The thesis explored the neural underpinnings of imaginal exposure and investigated mechanisms that could enhance its effectiveness, accessibility and tolerability. To further our knowledge of intrusive memories in PTSD (i.e., involuntary mental imagery), the characteristics of trauma memory hotspots (worst moments) collected within the first hours after trauma were explored. Study I demonstrated that imaginal exposure to mental imagery of phobic (vs. neutral) stimuli robustly activated emotion-processing brain areas. Study I also revealed that a brief 10-minute session of imaginal exposure was associated with reduced fear one week later. Study II investigated the link between vividness (clarity and liveliness) of mental imagery during imaginal exposure and reduction of fear using an experimental analogue of imaginal exposure (imaginal extinction). No evidence was found that high imagery vividness during imaginal extinction was associated with better long-term reduction in physiological fear responses than lower vividness. Study III revealed that hotspots collected soon after trauma are expressed as motion-rich sensory-perceptual experiences (mental imagery) with little detail on emotion/cognition. The contributions of this thesis involve demonstrating that mental imagery has the power to elicit emotional responses at subjective, physiological and neural levels and suggesting new avenues for treatment development. Future studies should explore the benefits of briefer imaginal exposure sessions to improve the effectiveness and accessibility of imaginal exposure. Future studies should also examine if fear reduction can be obtained with less vivid imaginal exposure, which could help attenuate distress and thereby make imaginal exposure tolerable for more patients. Lastly, the dynamic and visuospatial nature of newly formed trauma memory hotspots may help elucidate mechanisms through which tasks conducted posttrauma can prevent intrusive memories.

  • Reducing intrusive traumatic memories after emergency caesarean section : A proof-of-principle randomized controlled study

    • Preventative psychological interventions to aid women after traumatic childbirth are needed. This proof of-principle randomized controlled study evaluated whether the number of intrusive traumatic memories mothers experience after emergency caesarean section (ECS) could be reduced by a brief cognitive intervention. 56 women after ECS were randomized to one of two parallel groups in a 1:1 ratio: intervention (usual care plus cognitive task procedure) or control (usual care). The intervention group engaged in a visuospatial task (computer-game 'Tetris' via a handheld gaming device) for 15 min within six hours following their ECS. The primary outcome was the number of intrusive traumatic memories related to the ECS recorded in a diary for the week post-ECS. As predicted, compared with controls, the intervention group reported fewer intrusive traumatic memories (M = 4.77, SD = 10.71 vs. M = 9.22, SD = 10.69, d = 0.647 [95% CI: 0.106,1.182]) over 1 week (intention-to-treat analyses, primary outcome). There was a trend towards reduced acute stress re-experiencing symptoms (d = 0.503 [95% CI: -0.032, 1.033]) after 1 week (intention-to-treat analyses). Times series analysis on daily intrusions data confirmed the predicted difference between groups. 72% of women rated the intervention "rather" to "extremely" acceptable. This represents a first step in the development of an early (and potentially universal) intervention to prevent postnatal posttraumatic stress symptoms that may benefit both mother and child.     • IMPORTANCE Subsyndromal hypomanic symptoms are relatively common in the general population and are linked to the onset of bipolar disorder. Little is known about their etiology and whether this is shared with the etiology of bipolar disorder or other mental illnesses. OBJECTIVE To examine the genetic and environmental architecture of hypomanic symptoms in a nonclinical youth sample and compare estimates at varying severity levels and their association with diagnosed bipolar disorder. DESIGN, SETTING, AND PARTICIPANTS This cohort study used phenotypic and genetic data from the Child and Adolescent Twin Study in Sweden and included individuals with International Statistical Classification of Diseases and Related Health Problems, Tenth Revision diagnosis of psychiatric disorders from national registries for residents of Sweden. Associations between hypomania and polygenic risk scores for bipolar disorder, major depressive disorder and schizophrenia were also investigated. Analysis began November 2018 and ended October 2021. MAIN OUTCOMES AND MEASURES Hypomanic symptomswere assessed using the parent-rated Mood Disorders Questionnaire when the twins were aged 18 years. Bipolar disorder diagnosis and/or lithium prescription were ascertained from national registries for residents of Sweden. Polygenic risk scores for psychiatric disorders were calculated using independent discovery genetic data. RESULTS A total of 8568 twin pairs aged 18 years (9381 [54.7%] female) were included in the study. The hypomania heritability estimate was 59% (95% CI, 52%-64%) for male individuals and 29% (95% CI, 16%-44%) for female individuals. Unique environmental factors accounted for 41% (95% CI, 36%-47%) of the hypomania variance in male individuals and 45%(95% CI, 40%-50%) in female individuals. Shared environmental factors were only detected for female individuals and explained 26%(95% CI, 13%-38%) of the variance. The heritability estimates were fairly consistent across different hypomania severity groups. Moderate genetic (0.40; 95% CI, 0.21-0.58) and shared environmental (0.41; 95% CI, 0.03-0.75) correlations between hypomania and diagnosed bipolar disorder were found. Hypomania was significantly associated with the polygenic risk scores for schizophrenia (beta = 0.08; SE = 0.026; P =.002) and major depressive disorder (beta = 0.09; SE = 0.027; P =.001) but not bipolar disorder (beta = 0.017; SE = 0.03; P = 0.57) (bipolar disorder I [beta = 0.014; SE = 0.029; P =.64] or bipolar disorder II [beta = 0.045; SE = 0.027; P =.10]). CONCLUSIONS AND RELEVANCE Higher heritability for hypomania was found for male compared with female individuals. The results highlight the shared etiologies between hypomanic symptoms, bipolar disorder, major depression, and schizophrenia in youths. Future research should focus on identifying specific shared genetic and environmental factors. These findings support a possible dimensional model of bipolar disorder, with hypomania representing a continuous trait underlying the disorder.

  • Migration experience, resilience and depression : a study of Iranian immigrants living in Australia

    • The challenges of migration can jeopardize mental health, but also can provide opportunities for growth. This study investigated the association between levels of depression and socio-demographic and migration variables, and the role of resilience as a mediator between these features. A cross-sectional study was conducted of 182 Iranian immigrants living across Australia who completed an online questionnaire. Higher levels of depression were found in participants who were unemployed, experienced high levels of discrimination, had an incomplete tertiary education, and were younger and unmarried (p < 0.05). Higher levels of education, experience of moderate levels of discrimination and being married were associated with lower levels of depression mediated by resilience. Higher levels of depression were correlated with lower levels of resilience (r = −0.50, p < 0.001). The results of this study suggest several possible approaches to prevention of depression in immigrants. These may include improving employment opportunities, reducing discrimination, approaches that reduce the stress of the settlement process and interventions that aim to strengthen resilience.

We need to understand the scale of any such impact in different sections of the population, who is most affected and how best to mitigate, prevent and treat any excess morbidity. We propose a coordinated and interdisciplinary mental health science response.
