    • The American Thoracic Society has previously published statements on what constitutes an adverse effect on health of air pollution in 1985 and 2000. We set out to update and broaden these past statements that focused primarily on effects on the respiratory system. Since then, many studies have documented effects of air pollution on other organ systems, such as on the cardiovascular and central nervous systems. In addition, many new biomarkers of effects have been developed and applied in air pollution studies.This current report seeks to integrate the latest science into a general framework for interpreting the adversity of the human health effects of air pollution. Rather than trying to provide a catalogue of what is and what is not an adverse effect of air pollution, we propose a set of considerations that can be applied in forming judgments of the adversity of not only currently documented, but also emerging and future effects of air pollution on human health. These considerations are illustrated by the inclusion of examples for different types of health effects of air pollution.

  • Immunohistochemical differences between hyaluronan- and non-hyaluronan-producing malignant mesothelioma

    • In many but not all cases, malignant mesothelioma is associated with an elevated content of hyaluronan in pleural fluid. The hyaluronan-producing mesothelioma has not yet been immunohistochemically characterized; therefore, the purpose of this study was to compare the immunohistochemical reactivity patterns in relation to the ability of this tumour to produce hyaluronan. Pleural fluid samples from 33 patients with malignant mesothelioma were analysed for content hyaluronan using a quantitative high performance liquid chromatographic method. Biopsy specimens from the patients were studied immunohistochemically, using monoclonal antibodies against carcinoembryonic antigen (CEA), epithelial membrane antigen (EMA), a low molecular weight cytokeratin antigen (CAM 5.2) and vimentin. An elevated hyaluronan content, i.e. > 100 mg.L-1, was noted in 23 patients (70%). There was no reactivity to the monoclonal antibody raised against CEA in any case. There was a significantly higher reactivity to EMA (p = 0.026), a higher reactivity to CAM 5.2 (p = 0.053) and a lower reactivity to vimentin (p = 0.057) in the hyaluronan-producing mesotheliomas as compared to those with normal levels of hyaluronan. Mesotheliomas that produced hyaluronan differed immunohistochemically from those that did not. The connection between the ability to produce different antigens and hyaluronan may relate to the degree of differentiation of the tumour. Both of these characteristics (immunophenotype and ability to produce hyaluronan) may, therefore, be of importance in studies concerning the prognosis and treatment of the malignant mesothelioma.

  • Does parental or grandparental farm upbringing influence risk of asthma in offspring?

    • Background: Farm upbringing has been associated with lower risk of asthma, and also with methylation of asthma-related genes. As such, farm upbringing has the potential to transfer less asthma risk across generations. We aimed to study generational effects from parental farm upbringing on offspring asthma.Methods: Our study involved three generations: 5,759 participants from the ECRHS study (born 1945-71, denoted G1), their 9,991 parents (G0) and their 8,260 offspring (G2) participating in RHINESSA. Questionnaire data on upbringing and asthma were available for all generations; direct information for G1 and G2, and via G2 for G0. Parental and grandparental place of upbringing was categorised as (1) both parents from farm (2) mother from farm, father from village/city (3) father from farm, mother from village/city (4) both parents from village or one parent from village and one from city (5) both parents from city (ref.). Data was analysed in Cox regression with G2 age as time scale.Results: Parental farm upbringing was not related to offspring asthma when compared to city upbringing (HR 1.12, 95 % CI 0.74-1.69) Findings remained similar when stratified by offspring upbringing and asthma phenotypes. Quantitative bias analyses showed similar estimates for alternative data sources. Grandparental farm upbringing was not associated with offspring asthma in either the maternal (HR 1.05, 95% CI 0.67-1.65) or paternal line (HR 1.02, 95% CI 0.62-1.68).Conclusion: This multi-generation analysis suggests no evidence of an association between parental or grandparental farm upbringing and offspring asthma.

  • Associations between markers of respiratory morbidity in European children

    • School-aged children often experience acute respiratory symptoms. In a multicentre European study, the association between chronic respiratory symptoms (reported in a questionnaire), skin-prick test results, and lung function, and the occurrence of acute respiratory morbidity, was examined among children aged 6-12 yrs with chronic respiratory symptoms. Children with chronic respiratory symptoms, living in 10 European countries, were selected from a parent-completed questionnaire (n=4,307). Atopy was measured with skin-prick tests, and lung function with spirometry. A total of 1,854 (86%, of those in the initial cohort) children kept a successful daily diary regarding their respiratory symptoms for 2-3 months. In multivariate logistic regression analyses, children with asthmatic symptoms, particularly those with doctor-diagnosed asthma, had a greater risk of occurrence of lower respiratory symptoms (odds ratio (OR): 6.12; 95%, confidence interval (CI): 4.99-8.35) than children with a dry nocturnal cough as their only symptom. Atopy, particularly a positive reaction to indoor allergens, was significantly associated with occurrence of lower respiratory symptoms. For atopy the OR was 1.62 (95%, CI: 1.34-1.96). A reduced level of maximal mid-expiratory How was associated with an increased risk of lower respiratory symptoms, cough and phlegm. The associations were similar in Scandinavia, Central Eastern, Western and Southern Europe. To conclude, asthmatic symptoms reported in a questionnaire, atopic status and a reduced level of maximal mid-expiratory flow were associated with the occurrence of acute respiratory symptoms, especially those of lower respiratory symptoms.

  • Maternal preconception exposure to cleaning agents and disinfectants and offspring asthma

    • Background: There is growing concern about health effects of cleaning agents. Emerging research suggests adverse health effects of exposures prior to conception.Aim: To study childhood asthma in relation to mother’s occupational exposure to indoor cleaning before conception and around pregnancy.Methods: The multi-centre two-generation RHINE/RHINESSA study investigated asthma and wheeze with onset <10 years in 3318 adults. Exposure of their mothers to indoor cleaning agents and disinfectants starting before conception, in the 2-year period around conception and pregnancy, or after birth, was defined from mothers’ occupational history in combination with an asthma-specific job exposure matrix. Examples of exposed groups are cleaners, cooks, and nurses. Ordinary and mixed effects logistic regression analyses accounted for family clustering and study centre, and adjusted for maternal education.Results: Mother’s exposure to indoor cleaning starting before conception and continuing (n=610;47%) was associated with offspring’s childhood asthma: adjusted Odds Ratio (OR)=1.6 (95%CI 1.1-2.3), childhood asthma with nasal allergies: 1.8 (1.1-2.8), and childhood wheeze: 2.0 (1.3-3.1). Exposure starting around the time of conception and pregnancy (n=77;6%) was associated with childhood asthma: 2.3 (1.0-5.3) and childhood asthma with nasal allergies: 2.5 (1.0-6.4).Conclusions: Maternal occupational exposure to indoor cleaning pre-conception, or around the time of conception and pregnancy, may be a risk factor for childhood asthma in offspring. Considering potential implications for women in childbearing age using cleaning agents, and their children, mechanistic research and replication in cohort studies are needed.

    • The most recent European guidelines and task force reports on hospital-acquired pneumonia (HAP) and ventilator-associated pneumonia (VAP) were published almost 10 years ago. Since then, further randomised clinical trials of HAP and VAP have been conducted and new information has become available. Studies of epidemiology, diagnosis, empiric treatment, response to treatment, new antibiotics or new forms of antibiotic administration and disease prevention have changed old paradigms. In addition, important differences between approaches in Europe and the USA have become apparent. The European Respiratory Society launched a project to develop new international guidelines for HAP and VAP. Other European societies, including the European Society of Intensive Care Medicine and the European Society of Clinical Microbiology and Infectious Diseases, were invited to participate and appointed their representatives. The Latin American Thoracic Association was also invited. A total of 15 experts and two methodologists made up the panel. Three experts from the USA were also invited (Michael S. Niederman, Marin Kollef and Richard Wunderink). Applying the GRADE (Grading of Recommendations, Assessment, Development and Evaluation) methodology, the panel selected seven PICO (population-intervention-comparison-outcome) questions that generated a series of recommendations for HAP/VAP diagnosis, treatment and prevention.
