  • What matters more for the decision to move: jobs versus amenities

    • Highly skilled workers are increasingly recognised as a key competitive asset for regional development, and claims have been made that emphasise the importance of certain amenities for the prospects of attracting this particular group of workers. We use a recent large-scale survey to investigate the relative importance of jobs versus amenities for the decision to migrate, as perceived by the migrants themselves. The paper thereby adds important insights to the existing literature that has hitherto mainly focused on analysing the extent to which aggregate migration flows correlate with employment-related or amenity-related factors. The results show that jobs are considerably more important for the decision to move among highly educated migrants compared with migrants with lower education.

  • Metabolic enzyme expression highlights a key role for MTHFD2 and the mitochondrial folate pathway in cancer

    • Metabolic remodeling is now widely regarded as a hallmark of cancer, but it is not clear whether individual metabolic strategies are frequently exploited by many tumours. Here we compare messenger RNA profiles of 1,454 metabolic enzymes across 1,981 tumours spanning 19 cancer types to identify enzymes that are consistently differentially expressed. Our meta-analysis recovers established targets of some of the most widely used chemotherapeutics, including dihydrofolate reductase, thymidylate synthase and ribonucleotide reductase, while also spotlighting new enzymes, such as the mitochondrial proline biosynthetic enzyme PYCR1. The highest scoring pathway is mitochondrial one-carbon metabolism and is centred on MTHFD2. MTHFD2 RNA and protein are markedly elevated in many cancers and correlated with poor survival in breast cancer. MTHFD2 is expressed in the developing embryo, but is absent in most healthy adult tissues, even those that are proliferating. Our study highlights the importance of mitochondrial compartmentalization of one-carbon metabolism in cancer and raises important therapeutic hypotheses.

  • Effects of a Swedish master’s programme on quality improvement and leadership  - A qualitative study on micro, meso, and macro levels of the welfare sector

    • Improvement knowledge is recognised as an important competence for healthcare professionals in many countries. Researchers have studied the effects of improvement knowledge education but there is a lack of knowledge educations’ operational effects. Research also lacks a system perspective. To address these gaps we use a model making it possible to study the welfare sector as a complex system. In this way, we intend to provide a deeper understanding of operational effects of a improvement knowledge curricula. We examine the effects of a master’s programme on quality improvement and leadership in the Swedish welfare sector on micro, meso and macro levels. Twelve telephone interviews were performed with alumni exploring the experienced effects of the programme. On micro level, respondents reported that the engagement for improvement work increased and that patient/client/customer focus increased. Several personal effects were also described. On meso level, respondents described how the role as quality improvement facilitator developed and how measures were central for monitoring improvement. Several respondents believed the education contributed to improved health and welfare on macro level. To capture all effects of an improvement knowledge curricula in higher education qualitative effects on several levels of the welfare system are important to acknowledge.

    • Medicines improve health and the chances of survival in a wide variety of conditions. At the same time, no substance with pharmacological effects is without hazard. Adverse drug reactions (ADRs) can be associated with the intended pharmacological effect of the medicine (eg bleeding from warfarin), mediated by other mechanisms (eg anticholinergic effects of tricyclic antidepressants) or can be altogether unexpected (eg hypersensitivity reactions to abacavir). Some ADRs can be identified early in the development of a medicine, but knowledge of the adverse effects profile is provisional when the medicine is first marketed and usually changes over time. Premarketing clinical trials include too few patients and are too short to detect every outcome that will affect public health and individual patient safety. In addition, clinical trials are carried out in controlled settings that differ from real-world practice. This reduces their power to detect ADRs, for example those that are due to drug-drug interactions or that affect only susceptible subgroups (eg phocomelia due to thalidomide). Safety needs to be evaluated continuously throughout the life-cycle of a medicinal product.1,2 A key challenge is to identify emerging problems as early as possible, without generating false alarms.

  • Characterization of the circulating transcriptomic landscape in inflammatory bowel disease provides evidence for dysregulation of multiple transcription factors including NFE2, SPI1, CEBPB, and IRF2

    • AIM: To assess the pathobiological and translational importance of whole blood transcriptomic analysis in inflammatory bowel disease (IBD).METHODS: We analyzed whole blood expression profiles from paired-end sequencing in a discovery cohort of 590 Europeans recruited across six countries in the IBD Character initiative (newly diagnosed patients with Crohn's disease [CD, n = 156], ulcerative colitis [UC, n = 167], and controls [n = 267]), exploring differential expression (DESeq2), co-expression networks (WGCNA), and transcription factor involvement (EPEE, ChEA, DoRothEA). Findings were validated by analysis of an independent replication cohort (99 CD, 100 UC, and 95 controls). In the discovery cohort, we also defined baseline expression correlates of future treatment escalation using cross-validated elastic-net and random forest modelling, along with a pragmatic ratio detection procedure.RESULTS: Disease-specific transcriptomes were defined in IBD (8697 transcripts), CD (7152), and UC (8521), with the most highly significant changes in single genes, including CD177 (log2-fold change [LFC] = 4.63, p = 4.05 × 10 -118), MCEMP1 (LFC = 2.45, p = 7.37 × 10 -109), and S100A12 (LFC = 2.31, p = 2.15 × 10 -93). Significantly over-represented pathways included IL-1 (p = 1.58 × 10 -11), IL-4, and IL-13 (p = 8.96 × 10 -9). Highly concordant results were obtained using multiple regulatory activity inference tools applied to the discovery and replication cohorts. These analyses demonstrated central roles in IBD for the transcription factors NFE2, SPI1 (PU.1), CEBPB, and IRF2, all regulators of cytokine signaling, based on a consistent signal across cohorts and transcription factor ranking methods. A number of simple transcriptome-based models were associated with the need for treatment escalation, including the binary CLEC5A/CDH2 expression ratio in UC (hazard ratio = 23.4, 95% CI 5.3-102.0).CONCLUSION: Transcriptomic analysis has allowed for a detailed characterization of IBD pathobiology, with important potential translational implications.

    • This work presents a latchable valve microactuator for use in high-pressure environments, for instance deep-sea sampling in missions of long duration. Mounted on a minisubmersible, it can be used in confined spaces, earlier virtually unreachable. However, the device can be used in any high-pressure application where long open and/or closed times are required, and power supply is an issue. The actuator is fabricated using standard batch-processes as photochemical machining, wet etching and lithography. Focus of this work is on the endurance of the actuator to facilitate a bistable valve. The actuator managed to keep a deflected position for almost 50 hours at 1.8 MPa applied pressure, after which the experiment was aborted.
