  • A1Meighted Composite of Endodontic Inflammatory Disease is Linked to a First Myocardial Infarction

    • Purpose: To explore a weighted composite of endodontic inflammatory disease (ED) as a risk factor for suffering a first myocardial infarction (MI).Materials and Methods: Seven tooth-specific conditions related to EID were assessed radiographically in 797 patients suffering a first MI and 796 controls. A weighted composite of EID was calculated as the sum of all teeth, excluding third molars. Using maximum likelihood estimation, each condition was assigned a specific weight. Wth multivariable condi-tional regression, EIDvariables, periodontal disease, and missing teeth were assessed as predictors of a first MLResults: Periodontal disease (OR 1.38; 95% CI 1.13-1.69, p =0.0016) and missing teeth (OR 1.03; 95% CI 1.002-1.05, p =0.034) were related to the risk of a first MI, while none of the ED-related conditions individually were. However, when assessed as an aggregate, a weighted composite of ED (OR 1.97; 95% CI 1.23-3.17, p =0.0050) and periodontal disease (OR 1.34; 95% CI 1.09-1.63, p =0.0046) was associated with the risk of ML Missing teeth did not remain a statistically sig-nificant predictor of MI in the final model.Condusions: A weighted composite of ED was associated with the risk of MI and strengthens the evidence for a direst connection between oral inflammatory diseases and cardiovascular disorders.

  • Reduced Use of Antibiotics and Nasal Decongestants During Treatment with a Mouthwash Containing Delmopinol.

    • PURPOSE: To evaluate the use of concomitant medication in combination with a mouthwash of delmopinol HCl 0.2% based on data from 8 phase III efficacy studies on the mouthwash.MATERIALS AND METHODS: Clinical data obtained from 8 previously performed phase III studies, carried out to document the clinical efficacy of a mouthwash of delmopinol HCl 0.2% with respect plaque and gingivitis, were used to analyse the use of concomitant medication. In these 8 randomised double-blind clinical phase III studies the patients were - in addition to their normal oral hygiene measures - treated for 2-6 months with mouthwashes containing delmopinol HCl 0.2%, delmopinol HCl 0.1%, chlorhexidine digluconate 0.2% or placebo. The number of visits in each study was three. Each time the patients visited the dentist for efficacy determinations, other data were also recorded. One of these was whether the patient had used any other medication during the study. In this paper, the number of treatments of different types of concomitant medication (antibiotics, nasal decongestants and others) was used as a basis for statistical comparisons between the different test groups.RESULTS: For antibiotics (all indications), a 27% lower number of treatments was obtained in the delmopinol 0.2% group in comparison with the placebo group, and a 41% decrease was observed for treatments with antibiotics for respiratory infections. For nasal decongestants, the number of treatments was 53% lower in the delmopinol 0.2% group.CONCLUSIONS: The delmopinol HCl 0.2% solution in patients with gingivitis provided a statistically significant reduction of concomitant use of antibiotics and nasal decongestants.

  • The Importance of Psychological Factors in the Maintenance of Oral Health: A Study of Swedish University Students

    • Purpose: The aim of this study was to investigate the predictive value of the traditional multidimensional health locus of control (MHLC) scale, a corresponding dental health locus of control (DHLC) scale and dental health values (DHVs) regarding students' dental health before and after a video programme that presented information about dental diseases and instructions on oral hygiene. Materials and Methods: A group of 217 university students participated in this study Before the video programme was presented, assessments were made of MHLC, DHLC and DHVs Plaque and gingival indices were obtained in a clinical examination conducted before (plaque index [PLI] 1 and gingival index [GI] 1) and 10 weeks after (PLI 2 and GI 2) the presentation of the programme. Results: The percentage of tooth surfaces exhibiting plaque and of sites involving gingivitis decreased and was statistically significant between baseline and re-examination for both males and females. The females exhibited statistically significant better PLI 1 and GI 1 values and stronger DHVs than males Also, better GI 1 values were found to be statistically significant and related to stronger DHVs for females. A stronger trust in the dental health personnel regarding the dental health was related to more gingivitis at the initial examination (GI 1). For males, stronger internal DHLC was related to more plaque at the final examination (PLI 2). Conclusion: The only psychological scales that showed some relationship to the measures of dental health were DHLC and DHVs Gender was the strongest related variable to dental health.

    • PURPOSE: The aim of the present study was to evaluate the recommendations relating to the use of approximal cleaning aids given by dental hygienists and dentists, the self-care practices in a Swedish population and the ability to remove dental plaque. MATERIALS AND METHODS: A structured questionnaire was randomly distributed to 500 dental hygienists and 500 dentists and a similar questionnaire was distributed to 1000 randomly selected individuals, divided equally into the following age groups: 15 to 20, 21 to 40, 41 to 60 and > 60 years. A clinical examination evaluating the ability to remove approximal dental plaque was also carried out in a total of 60 regular users of approximal cleaning aids. Plaque was scored before and after cleaning with a toothpick, dental floss or an interdental brush. RESULTS: The response rate was 82%, 79% and 68% for the three groups. The results reveal that dental hygienists give more detailed information about a majority of the aspects that are related to the use of approximal cleaning aids compared with dentists (P < 0.01 or P < 0.001). The majority of the dental staff give recommendations to children and adolescents firstly to prevent dental caries and to older individuals to improve periodontal health. The use of different approximal cleaning aids on a daily basis varied with respect to age group (2% to 42%); dental floss dominated in the younger age groups and interdental brushes in the two oldest groups. In the clinical study, the largest plaque reduction was produced by the interdental brush (83%), followed by toothpicks (74%) and dental floss (73%). CONCLUSIONS: The present study indicated the importance of individual recommendations related to the use of approximal cleaning aids.

  • Caries risk profiles of 12-13-year-old children in Laos and Sweden

    • PURPOSE: To analyse caries risk factors of 12-13-year-old children living in Laos, using the computer program Cariogram to illustrate the caries risk profile. In addition, to compare the results with a study performed in Sweden. MATERIALS AND METHODS: One hundred Laotian and 392 Swedish children were included. Interviews were performed to obtain information on diet intake and fluoride use. Saliva was analysed for mutans streptococci, lactobacilli and secretion rate/buffering capacity. Oral hygiene was assessed using the Silness and Löe criteria. Caries prevalence was recorded according to WHO. The data were entered into the Cariogram to determine each child's caries risk, expressed as 'the chance of avoiding caries'. The children were divided into five risk groups. RESULTS: Mean DMFT level of the Laotian children was 4.61 +/- 2.95 and 1.38 +/- 1.97 in the Swedish group. For the risk factors plaque amount, frequency of food intake, saliva secretion rate, buffering capacity and fluoride, the Laotian children had significantly less favourable values compared to the Swedes. Only 6% of Laotian children belonged to the Cariogram low risk group versus 40% of the Swedish children. The mean DMFT for the five Cariogram groups was (from low to high risk) 0.00, 3.00, 3.56, 5.66, 6.11 for the Lao children and 0.31, 1.39, 2.56, 3.03, 2.91 for the Swedish ones. The mean chance of avoiding caries was 37.3% for the Laotians and 69.2% for the Swedish children (p < 0.001). CONCLUSION: According to the 'opinion' of the Cariogram, the Laotian children demonstrated significantly higher caries risk than Swedish children.

  • Urinary Fluoride Excretion after a Single Application of Fluoride Varnish in Preschool Children

    • PURPOSE: To assess urinary fluoride excretion after topical application of a commercial fluoride varnish in preschool children.MATERIALS AND METHODS: Twelve healthy children, 2.5-6.0 years of age, were enrolled in a placebo-controlled cross-over trial. After a 5-day run-in period, the morning urine was collected as baseline. One hour after breakfast, 0.1 ml of the active (Fluor Protector S; 7,700 ppm F) or the placebo varnish was applied with a microbrush on the buccal surfaces of the primary teeth. Thereafter, a 6-h urine sample was collected and the fluoride content was determined with an ion-sensitive electrode. The parents supervised toothbrushing with a small-fingernail amount of fluoride toothpaste (1000 ppm) twice daily during the entire experiment.RESULTS: One boy failed to comply with the urinary samplings and was excluded. The mean fluoride concentration in the 6-h urine samples was slightly higher after the active varnish compared with the placebo varnish, but the difference was not statistically significant. Likewise, no statistically significant differences were obtained when the post-treatment concentrations were compared with baseline for the two varnishes. No side-effects or adverse events were reported.CONCLUSION: A single topical treatment with the investigated varnish did not significantly increase the urinary fluoride excretion compared with placebo in preschool children with parallel use of fluoride toothpaste.

  • Human Leucocyte Antigen Profile and Transmission of Mutans Streptococci in Mother-Child Pairs

    • Purpose: To investigate possible association between the transmission of mutans streptococci and sharing the immune system component Human Leucocyte Antigen (HLA) class II in mother-child pairs. Material and Methods: Plaque samples from 43 mother-child pairs were cultivated and screened for mutans streptococci. In 14 pairs where both mother and child harboured the bacteria, the strains were genotyped by Random Amplified Polymorphic DNA and samples were run on PAGE gels. Analysis of genetic identity between mother and child strains was performed with help of software and Dice similarity index. The distribution of HLA of serogroup DR4 (HLA DR4) was studied in relation to maternal transmission and mutans streptococci colonisation in children. The study hypothesis was that in pairs where both mother and child were HLA DR4 positive, transmission of mutans streptococci was more likely. Results: No correlation between the presence of HLA DR4 in mother and child and maternal transmission of mutans streptococci was established. However, the results showed no linkage between mutans streptococci colonisation and HLA DR 4. Of 15 children with mutans streptococci, 12 were HLA DR4 positive. Conclusion: The result suggests that presence of HLA DR4 could be a predisposing factor for colonisation with mutans streptococci in children.
