    • OBJECTIVES: A healthy diet is associated with reduced risk for premature death; however, data on the association between food expenditure and life expectancy are scarce. The aim of this study was to determine the association of expenditure on food groups with life expectancy in men and women.METHODS: This study used data from 1548 counties, representing >90% of the population of the United States from 2001 through the end of 2014. Multivariable adjusted Bayesian generalized linear models were used to assess the association of county-level expenditure on nine groups of individual food and combined healthy and unhealthy foods, and a constructed healthy diet score with life expectancy at 40 y of age by sex.RESULTS: A 1-unit increase in the healthy diet score based on food expenditure was associated with a 0.07 y (95% confidence interval [CI], 0.05-0.10), 0.04 y (95% CI, 0.02-0.07), and 0.06 y (95% CI, 0.04-0.08) increase in county-level life expectancy among men alone, women alone, and men and women combined, respectively. Increasing expenditure on whole grains (estimate of per 1% increase 0.07; 95% CI, 0.03-0.11), fresh fruit and vegetables (0.06; 95% CI, 0.02-0.09), and dairy products (0.05; 95% CI, 0.03-0.07), as well as reducing expenditure on sugar-sweetened beverages (-0.02; 95% CI, -0.04 to -0.01) and processed red meat (-0.05; 95% CI, -0.08 to -0.02) showed a positive association with increased county-level life expectancy.CONCLUSIONS: These findings may inform a nutritional measure against premature death and stagnation of increase in life expectancy.

    • There is no consensus on how to estimate energy requirements after liver transplantation (LT). The aim of this study was to compare measured resting energy expenditure (REE) with predictive equations and fixed factors, and evaluate whether clinical variables were associated with REE.During the period of 2011 through 2018, REE measured with indirect calorimetry and predicted by the Harris and Benedict (HB) equation was compared in patients during the first 30 postoperative days after LT. The fixed factors 25 kcal/kg, 30 kcal/kg, or 35 kcal/kg were used to calculate energy requirements. The accuracy of HB and fixed factors were evaluated with a Bland-Altman analysis and Lin's concordance correlation coefficient. The associations of pre- and postoperative clinical variables with REE were evaluated in a multivariate regression analysis.A total of 143 patients were evaluated and had indirect calorimetry performed on postoperative day 6 (interquartile range: 3) in median. The mean measured REE was 1950 ± 461 kcal (range, 720-3309 kcal) or 24.5 ± 6.1 kcal/kg body weight. Large limits of agreements were observed in the Bland-Altman analyses for both HB and fixed factors. HB was closer than fixed factors with a positive concordance (concordance correlation: 0.350; 95% confidence interval, 0.248-0.445) and Pearsons r2 = 0.261. Measured REE was significantly associated (P < 0.05) with age, sex, Model for End-Stage Liver Disease score before LT, surgery time, and graft cold ischemia time according to the multiple regression analysis.The low accuracy of HB and fixed factors suggests risks of both under- and overfeeding of individual patients if energy requirement is only based on calculation. REE measurement is recommended after LT to secure accurate and safe nutritional therapy.

    • ObjectivesThe lifestyle of postindustrial society has undergone major shifts characterized by changes in demographic and epidemiologic profiles, eating habits, and job structures, with irregular working hours, particularly night shifts. The investigation of dietary patterns is of great importance for the discussion and devising of effective dietary strategies for shift and night workers in general, particularly in view of the increased sleepiness reported during night work.ObjectiveWe aimed to determine the association between dietary patterns of Brazilian truck drivers and sleepiness levels, according to work shift.MethodsA cross-sectional study of 52 drivers (25 long haul and 27 short haul) at a freight company was carried out. This study entailed application of a structured questionnaire collecting sociodemographic, lifestyle, and nutritional status data. Assessment of dietary intake using a 24-h dietary recall and an evaluation of sleepiness by the Karolinska Sleepiness Scale were performed. The principal components of the diet were analyzed by factor analysis to derive dietary patterns. A linear mixed model was then applied to determine a model for sleepiness levels of the drivers as a function of dietary pattern, time of day, and work shift.ResultsThree intake patterns were derived: traditional, prudent, and Western. Associations of time of day (F = 23.629, P < 0.01) and shift type (F = 42.218, P < 0.01) on sleepiness were found. An association between diet and sleepiness was also evident, where the prudent pattern was associated with low sleepiness among short-haul truck drivers (F = 3.865, P = 0.02).ConclusionsThe results of the present study revealed an association between dietary patterns and short-haul driving, in which the healthy pattern produced low sleepiness during the day. The sleepiness curve of long-haul drivers appears to have a flattening pattern, probably because of irregular working times.
