    • <p>The purpose of this research was to psychometrically evaluate the Dyadic Adjustment Scale (DAS), modified for use with new, first-time parents by extending the items of communication, sensuality, and sexuality. A total of 820 Swedish respondents, 6 months after the birth of their first child, participated in the study. Psychometric evaluation was conducted with factor analysis. The obtained factor structure was tested with multitrait analysis program. Thirty-three itemswere found to fit into a five-factor solution, explaining 50% of the total variance. Descriptive data revealed thatmost new parentswere satisfied with their intimate relationship in general, but dissatisfied with their sexual lives. Themodified DAS, now called the Quality of Dyadic Relationship Instrument includes 33 items and seems to be a useful, updated measurement for assessing quality of the intimate relationship in new first-time parents.</p>

    • <p>This study aimed to describe the quality of the intimate relationship among parents six months after the birth of their first child. The Dyadic Adjustment Scale (DAS) has been modified and used on 820 responding first-time parents, of which 768 were couples. The results reveal that most parents were happy in their relationship, but both mothers and fathers were discontented with the dyadic sexuality. "Being too tired for sexual activity" was a problem, especially for the mothers, and the most common frequency of intercourse was once or twice per month. The result does not support the assumption that the couples compensate the lacking sexuality with sensuality. Good communication within the couple was associated with higher levels of several dimensions of the intimate relationship, especially dyadic consensus and satisfaction. Thus, one way to stabilize and strengthen a relationship when dyadic sexual activity is low would be to emphasize dyadic communication and sensual activity.</p>

  • Akademiskt entreprenörskap ger samhällsekonomisk vinst : Entreprenörskap och företagande bland akademiker under perioden 2004–2009

    • <p>I denna rapport analyseras samhällseffekter av satsningar på akademisk utbildning genom att se på hur akademikers involvering i kunskapsintensivt företagande under perioden 2004-2009 bidrar till nya arbetstillfällen och ekonomisk tillväxt. I rapporten fokuseras särskilt fyra huvudsakliga grupper av akademiker: civilekonomer, civilingenjörer, jurister samt läkare/ tandläkare (medicinare). Rapporten är författad av Lina Ahlin; verksam vid CIRCLE och Nationalekonomiska institutionen/Lunds Universitet, Jonas Gabrielsson; verksam vid CIRCLE/Lunds Universitet och Sektionen för Ekonomi och Teknik/Högskolan i Halmstad, samt Karl Wennberg; verksam vid Centrum för Entreprenörskap och Affärsskapande/ Handelshögskolan i Stockholm och Ratio - Näringslivets Forskningsinstitut.</p><p>Rapportens resultat visar genomgående att akademikers företag bidrar till nya arbetstillfällen och ekonomisk tillväxt i betydligt högre utsträckning än andra företag. Det finns även stora skillnader mellan företag som startas och drivs av individer med olika akademiska utbildningsinriktningar. Civilekonomers och civilingenjörers företag skapar betydligt fler nya arbetstillfällen än juristers och medicinares företag. Samtidigt är bidraget från civilekonomers företag till nya arbetstillfällen cirka 20 procent högre än civilingenjörernas företag.</p><p>Utöver sammanställningen av akademikerföretagens bidrag till nya arbetstillfällen och ekonomisk tillväxt presenterar vi också ett antal pedagogiska och användbara nyckeltal för att mäta den <em>entreprenöriella avkastningen </em>från investeringar i akademisk utbildning. Denna avkastning baseras på akademikerföretagens bidrag till nya arbetstillfällen och ekonomisk tillväxt i förhållande till statens kostnader för olika typer av akademiska utbildningar. I analysen finner vi att om myndigheterna satsar 10 000 kronor på en akademisk utbildning så har detta generellt sett en god entreprenöriell avkastning för samtliga utbildningsgrupper.</p><p>Vi finner även att resultaten skiljer sig markant åt mellan olika akademiska utbildningar. Våra resultat visar att juristers och framförallt civilekonomers företag genererar betydligt fler nya arbetstillfällen per statlig utbildningskrona jämfört med civilingenjörers och läkares/ tandläkares företag, framförallt eftersom kostnaden för att utbilda de senare grupperna är så pass mycket högre. Skillnaderna i entreprenöriell avkastning mellan de olika akademiska utbildningsgrupperna i termer av löneutbetalningar och skatteintäkter baserat på företagens omsättning är mycket snarlika de skillnader som vi finner för nya arbetstillfällen.</p><p>För att driva på tillväxten och den industriella utvecklingen i ekonomin behöver Sverige individer och företag som kan omsätta teknologi och ny kunskap till produkter och tjänster. Förhoppningen är att rapporten skall bidra till den fortsatta debatten kring hur akademiker bidrar till den samhällsekonomiska utvecklingen genom involvering i kunskapsintensivt företagande och entreprenörskap.</p>

  • Differences in pain and nausea in children operated on by Tonsillectomy or Tonsillotomy – a prospective follow-up study

    • <p><strong>Aim: </strong>To evaluate the differences in postoperative pain, nausea and time of discharge in children 3–12 years old after Tonsillectomy or Tonsillotomy at the postanaesthetic care unit, children’s ward and at home. </p><p><strong>Background:</strong> Tonsillectomy involves risk of bleeding, severe postoperative pain and nausea. Tonsillotomy is a less invasive method with lower risk of bleeding, postoperative pain and nausea according to previous studies.</p><p><strong>Design:</strong> A prospective, comparative follow-up study design.</p><p><strong>Method:</strong> From December 2008–April 2009 following parental agreement, 87 children in the ages 3–12 undergoing Tonsillectomy or Tonsillotomy participated. Visual analogue scale was used for children’s pain and nausea reports.</p><p><strong>Result:</strong> Significantly, fewer children operated on by the Tonsillotomy reported postoperative pain ‡ 3 according to the visual analogue scale than children operated on by the Tonsillectomy at the postanaesthetic care unit and the children’s ward. A statistically significant difference of postoperative nausea was only present during the care at the postanaesthetic care unit and children’s ward with fewer Tonsillotomy children reporting nausea ‡ 3. The time of postoperative care was shorter among the Tonsillotomy children in both the postanaesthetic care unit and the children’s ward. Postoperative pain and pain related difficulties in eating after discharge was significantly more present among the Tonsillectomy children compared with the Tonsillotomy children.</p><p><strong>Conclusion: </strong>The results of our study showed duration of postoperative pain and nausea in both groups, but indicated that Tonsillotomy is a more favourable alternative than Tonsillectomy in children. © 2012 Blackwell Publishing Ltd.</p>

    • <p>The present article analyses why and how Ericsson Telecom assumeda greater responsibility than was legally required when it dismissedmore than 23,000 employees in Sweden at the beginning of the21st century. The analysis starts from neoinstitutional theoryand is based on case studies of the company’s closuresin Norrköping and Linköping. The article focuses,in particular, on the interaction between Ericsson, the tradeunions, the County Administrative Board, the County Labour Board,the Public Employment Service, the Swedish Employment SecurityCouncil, the government and the respective municipalities. Itis shown that the greater responsibility taken by Ericsson wasbased on its desire to maintain legitimacy by taking into considerationprevailing societal expectations regarding the company’sbehaviour.</p>

    • <p>Hur gör ett företag som gör mer än det tidigare gjort för sin personal vid en nedläggning? Varför gör det överhuvudtaget mer än det tidigare gjort, eller ens behöver? I denna bok analyseras telekomföretaget Ericssons nedläggningar i Norrköping och Linköping under åren mellan 1999 och 2002. Författaren visar hur nedläggningarna utvecklades i samspel med sociala institutioner och i interaktion med fackliga organisationer såväl som bemanningsföretag, Trygghetsrådet och kommunala och statliga myndigheter. Här förfäktas idén att företagets agerande hängde samman med att etablerade organisationer genom sin tröghet till förändring hade skapat utrymme för nya organisatoriska lösningar i samband med rationaliseringar: bemanningsföretagen kunde nu ta över personalansvaret.</p><p>Författaren gör också en genomgång av tidigare forskning om nedläggningar och personalinskränkningar, varvid modeller redovisas som förknippats med företag som ansetts ha tagit socialt ansvar när de lagt ner eller minskat på sin personal.</p>

    • <p>The aim of this study was to describe the ways of living with major depression in families. Seven families with an adult member who suffered major depression participated, in all 18 participants. Data were collected from seven narrative group interviews, one with each family. A qualitative thematic content analysis was used and the text was coded according to its content and further interpreted into themes. Findings consist of five themes: 'Being forced to relinquish control of everyday life'; during depression the family members lost their energy and could not manage everyday life. 'Uncertainty and instability are affecting life'; everyday life in the families was unstable and emotions influenced the atmosphere. 'Living on the edge of the community': the families periodically lived in seclusion. 'Everyday life becomes hard'; everyday life was demanding for everyone in the family and the responsibility shifted between family members. 'Despite everything a way out can be found'; the families as a unit as well as individually had their own ways of coping and finding some kind of satisfaction within the bounds of possibility. The families' experiences were demanding. Children were aware of their parent's depression and were involved in managing everyday life and emotionally affected by the situation. This legitimates the need for nurses and other healthcare professionals to have a family's perspective on the whole situation and to include partners as well as children and take their experiences seriously when supporting and guiding a family. It is important to develop strategies which in particular include children in the planning, provision and receiving of care as well as unburden them with the responsibility. It is also important to support the family in their own coping strategies with a major depressive episode.</p>

    • <p>Major depression challenges the ways of living for both individuals and families. The aim of this study was to describe what happens and how to manage major depression in a family. The case in this paper is a family with a mother who is suffering major depression and her son and daughter. Narrative interviews and qualitative content analysis were conducted. The findings revealed six themes: "a stealthy intruder," "moving slowly to helplessness," "saving the situation," "protecting oneself and others," "conveying things that are beyond words," and the "dispersal of shadows." These themes elucidated the family members' varying views of depression and the unique ways they managed the situation.</p>

    • <p><strong>AIM AND OBJECTIVE:</strong> The aim was to elucidate the meaning of major depression in family life from the viewpoint of an ill parent. Background. Major depression according to Diagnostic and Statistical Manual of Mental Disorders is common and may appear repeatedly over several years, and affects family life. Depression in parents has a negative impact on family function and children's health; however, studies regarding the deeper understanding of major depression in family life are lacking.</p><p><strong>DESIGN:</strong> A qualitative explorative study using narrative interviews with eight parents who were identified with major depression.</p><p><strong>METHODS:</strong> A phenomenological-hermeneutic method of interpretation was used for analysing interview texts and included naïve understanding, a structural analysis where text was divided into meaning units, which were condensed and abstracted, and finally a comprehensive understanding.</p><p><strong>RESULT:</strong> Two themes were extracted: 'to be afflicted in an almost unmanageable situation' with sub-themes 'feeling hopelessly bad', 'being worthless', 'being unsatisfied' and the theme 'to reconcile oneself to the situation' with sub-themes 'being active', 'being satisfied' and 'maintaining parenthood'.</p><p><strong>CONCLUSION:</strong> Comprehensive understanding revealed the parents' simultaneous suffering and dignity in family life; suffering with serious lack of well-being and health, destroyed self-confidence and unhappiness, and dignity with strength, confidence and joy in children. The movement between suffering and dignity complicated family life. Dignity was threatened by the awareness that suffering in major depression was recurrent. Dignity had to be repeatedly restored for self and the family, and family dignity has to be restored before others outside the family circle.</p><p><strong>RELEVANCE TO CLINICAL PRACTICE:</strong> A deeper understanding of the meaning of major depression in family life is helpful and for healthcare professionals to prevent individual and family suffering by assisting and preserving dignity.</p>

    • <p><em>Objective: </em>Breathlessness is a subjective symptom, which makes it difficult to define and understand. The aim of the present study was to illuminate how patients suffering from breathlessness experience their everyday life. <em>Method: </em>The study was a qualitative study, and the focus of the analysis was the patients' descriptions of their experiences of breathlessness using a diary with two unstructured questions for a period of 7 consecutive days. Sixteen participants: 7 men, mean age 65 ± 7 (range 55-73 years old), and 9 women, mean age 65 ± 9 (range 50-72 years old) participated in the study. <em>Results: </em>Two themes emerged from the analysis: 1) Impaired quality of life and 2) symptom tolerance and adaptation. The theme "impaired quality of life" included the categories limited physical ability, psychological burdens, and social life barriers. The theme "symptom tolerance and adaptation" included importance of health care, social support, hobbies and leisure activities, and coping strategies. <em>Significance of results:</em> The findings in our study showed that patients, in spite of considerable difficulties with shortness of breath, found relief in several types of activities, in addition to drug therapy. The result indicates that the "biopsychosocial model" is an appealing approach that should be discussed further to gain a better understanding of breathlessness.
