Supported Visitation in Cases of Violence : Political Intentions and Local Practice in Sweden

- In Sweden, supervised visitation has been replaced with a new measure called supported visitation. In the reform process, it was emphasized that if face to face visitation cannot be organized without risk for the child, indirect visitation or no visitation are to be considered better options. The aim of this article is to explore social work practice regarding supported visitation in cases involving violence. It draws on a study of a local visitation centre and the data consists of case files from the social services regarding 37 children where a court ordered visitation support, interviews with seven members of staff, ten parents and three children, and local documents and guidelines. For 18 of the 37 children, case files contained credible information about a history of violence. The study shows that district courts sometimes order visitation support in cases where there is a risk for the child and where in the near future normalization of visitation is unlikely. Thus, the measure of visitation support is sometimes used in a way that was not intended. Regarding social work practice, the analysis indicates that, although the guidelines developed at the local support centre under study adhere to the national policy intentions, both professionals’ validation and invalidation of violence can be seen. For service users previously subjected to violence, the documented court and social services’ practices may actively contribute to children’s and residential parents’ continued vulnerability.

- Utifrån egna erfarenheter av deltagarnära forskning resonerar vi i kapitlet om forskningsetik och forskaretik baserat på olika etiska dilemman och överväganden vi mött, olika betydelser av god forskningsetik och god forskning. Vi reflekterar över hur vi som forskare kan utveckla ett särskilt etiskt förhållningssätt som präglas av närhet och samarbete i en strävan att skapa kunskap som är praktiknära och samhällsrelevant. Vi har valt att utgå från tre forskningsprojekt. Två av projekten kretsar kring barn och unga. Det tredje fokuserar på hyresgästers upplevelser av bostadsförnyelse och hur boende organiserar sig i frågor som rör hemmet och boendet. Mot slutet av kapitlet resonerar vi om hur forskare ständigt bör reflektera över etik, i förhållande till sin roll som forskare och sina forskningspraktiker. Här diskuterar vi även fördelar med deltagarnära forskning som förespråka rhandling, teori, reflektion och praktik i forskningen.

- Purpose: Being a family member to someone who has amyotrophic lateral sclerosis (ALS) is demanding and often requires sacrificing a lot. Family members can experience fatigue, anxiety, guilt and need support. The aim was to explore registered nurses’ (RNs’) experiences of providing support to the families of patients with ALS within specialized palliative home care (SPHC).Methods: A qualitative explorative design. Interviews were conducted with RNs (n = 11) from five SPHCs in Sweden and analysed using qualitative content analysis.Results: The results emerged in the following categories:”To support in an increasingly difficult everyday life”, based on the sub-categories: “Creating a trusting relationship”, “Balancing between the needs of patients and their families”, and “Sharing knowledge about dying to the families”;”To support in emotionally challenging situations”, based on the sub-categories: “Harbouring family members’ difficult feelings”, “Providing support even though the situation is unpleasant” and “Being able to give support by receiving confirmation and support from others”.Conclusions: RNs working in SPHC have an important role in providing support in several ways to the families of patients with ALS, through facilitating their everyday life and giving emotional support when needed, based on the needs of both patients and the families.

Collaborative Interpretation of Researcher-Generated Photo-Elicitation Findings : Insights From Women With Lived Experience of Homelessness

- Interviews with individuals experiencing homelessness can be challenging for various reasons, including mental and physical health issues, substance use, and negative experiences with authority figures. Researchers have used photos to facilitate communication and empower participants during data collection. We analyzed data from a previous study to explore the use of researcher-generated photos during interviews about health with 13 women experiencing homelessness. Conversation analysis revealed clear patterns regarding the use of the photos during the interviews. The photos were referred to 118 times over the total interview length, 6 hours and 23 minutes, with the interviewer making 62% of the referrals and the women accounting for 38%. Fifty-nine percent of the referrals occurred within the first 5 minutes of the interviews. The women used the photos to trigger associations and emotions, to describe photo content, or in a minor role during the interview. Interpretations from an advisory board of six women with lived experiences of homelessness suggested that the photos did not engage participants as intended, highlighting the importance of considering participants’ perspectives when designing photo-elicitation methods. The feedback also provided valuable insights into interview locations and incentives in research that may have influenced the women’s willingness to use the photos. This study emphasizes the importance of understanding the complexity of choosing researcher-generated photos in interviews with underserved, hard-to-reach populations.

Psychometric Properties of the PTSD Checklist for DSM-5 (PCL-5) in parents to children with burns.

- Introduction Symptoms of post-traumatic stress disorder (PTSD) is a common problem among parents of children with burns. However, there is a paucity of evaluated screening tools for this population. The aim was to evaluate the psychometric properties of the PTSD Checklist (PCL), which is recently revised in accordance to the Diagnostic and statistical manual of mental disorders, 5th ed. Methods The participating parents (N= 62, mean age= 38) completed self-report questionnaires 0.8-5.6 years after their child’s burn. Measures were the PCL-5 (consisting of four subscales; Intrusion, Avoidance, Negative alterations in cognitions and mood, and Arousal and reactivity), the Impact of Event Scale-Revised (with three subscales; Intrusion, Avoidance and Hyperarousal) and the Perceived Stress Scale. The children had a TBSA burned ranging from 1 to 31 % and were 0.1-15.0 years of age at injury.Results The average PCL-5 scores were low to moderate and indicated that no parent was above the recommended preliminary cut off of 38 for PTSD. Cronbach’s alpha values were acceptable and varied between 0.56 and 0.77 for the four PCL-5 subscales and mean inter-item correlations ranged from 0.22 to 0.73. The PCL-5 subscales were positively correlated with the corresponding IES-R subscales as well as the total PSS score (p<.05). There were no associations between the PCL-5 and burn severity (TBSA, TBSA-FT, and LOS), time since injury, child age or gender, or parent gender. Conclusion In conclusion, the PCL-5 had high internal consistency and evaluation of concurrent validity suggested moderate associations with other measures of traumatic stress and perceived stress as expected. The moderate associations with other measures of stress is to be expected, taking into account the slightly different constructs targeted by the three measures in this study. This first study suggests that the PCL-5 is a psychometrically sound instrument that deserves further evaluation as a screening tool for parents of children with burns.

Factors associated with student learning processes in primary health care units : a questionnaire study

- BACKGROUND: Clinical placement plays a key role in education intended to develop nursing and caregiving skills. Studies of nursing students' clinical learning experiences show that these dimensions affect learning processes: (i) supervisory relationship, (ii) pedagogical atmosphere, (iii) management leadership style, (iv) premises of nursing care on the ward, and (v) nursing teachers' roles. Few empirical studies address the probability of an association between these dimensions and factors such as student (a) motivation, (b) satisfaction with clinical placement, and (c) experiences with professional role models.OBJECTIVE: The study aimed to investigate factors associated with the five dimensions in clinical learning environments within primary health care units.DESIGN AND METHODS: The Swedish version of Clinical Learning Environment, Supervision and Teacher, a validated evaluation scale, was administered to 356 graduating nursing students after four or five weeks clinical placement in primary health care units. Response rate was 84%. Multivariate analysis of variance is determined if the five dimensions are associated with factors a, b, and c above.RESULTS: The analysis revealed a statistically significant association with the five dimensions and two factors: students' motivation and experiences with professional role models. The satisfaction factor had a statistically significant association (effect size was high) with all dimensions; this clearly indicates that students experienced satisfaction.CONCLUSIONS: These questionnaire results show that a good clinical learning experience constitutes a complex whole (totality) that involves several interacting factors. Supervisory relationship and pedagogical atmosphere particularly influenced students' satisfaction and motivation. These results provide valuable decision-support material for clinical education planning, implementation, and management.
