  • Symptoms during pregnancy in primiparous women with congenital heart disease.

    • Background: As more women with congenital heart disease (CHD) are reaching childbearing age, it becomes more common for their symptoms to be evaluated during pregnancy. However, pregnancy-related symptoms are similar to those caused by heart disease. This study investigated the prevalence of factors associated with symptoms during pregnancy in women with CHD.Methods: The national birth register was searched for primiparous women with CHD who were registered in the national quality register for patients with CHD.Results: Symptoms during the third trimester were reported in 104 of 465 evaluated women. The most common symptom was palpitations followed by dyspnea. Factors associated with symptoms were tested in a univariable model; higher NYHA classification (>1) (OR 11.3, 95%CI 5.5-23.2), low physical activity (≤3 h/week) (OR 2.1 95%CI 1.3-3.6) and educational level ≤ 12 years (OR 1.9 95%CI 1.2-3.0) were associated with having symptoms. In multivariable analysis, low physical activity level (OR 2.4 95%CI 1.2-5.0) and higher NYHA class (OR 11.3 95%CI 5.0-25.6) remained associated with symptoms during pregnancy. There were no cases with new onset of impaired systemic ventricular function during pregnancy.Conclusion: Symptoms during pregnancy are common in women with CHD but are often already present before pregnancy. Because ordinary symptoms during pregnancy often overlap with symptoms of heart disease, it is important to know if symptoms were present before pregnancy and if they became worse during pregnancy. These results should be included in pre-pregnancy counselling and considered in the monitoring during pregnancy.

    • The aim of this article is to describe how experince-based knowledge can be made visible by giving some examples of how this has been done in Swedish social welfare services, in collaborations between social services agencies and research and development (R&D) units. These examples will be linked to theories and discussed in relation to different research utilisation models. By using one of these models, it is argued that R&D activities can broaden the concept of evidence-based practice and help brigde the gap between research and practice.

    • Elucidating patients´ experiences of living with chronic progressive hereditary ataxia and the symptomatic treatment with intrathecal baclofen (ITB) is the objective of the current study. A multicenter qualitative study with four patients included due to the rare combination of hereditary ataxia and ITB therapy was designed to elucidate participants' experiences through semi-structured interviews. The transcribed text was analyzed according to content analysis guidelines. Overall we identified living in the present/ taking one day at a time as the main theme covering the following categories: 1) Uncertainty about the future as a consequence of living with a hereditary disease; The disease; 2) Impact on life as a whole, 3) Influence on personal life in terms of feeling forced to terminate employment, 4) Limiting daily activities, and 5) ITB therapy, advantages, and disadvantages. Uncertainty about the future was the category that affected participants' personal life, employment, and daily activities. The participants' experience of receiving ITB therapy was expressed in terms of improved quality of life due to better body position and movement as well as better sleep and pain relief.

    • OBJECTIVE: To assess physical activity and energy expenditure and to identify their correlates during adolescence and early adulthood. DESIGN: In a cohort study, total energy expenditure (TEE) and physical activity level (PAL) were assessed at 15 and 21 y from a 7-day activity diary and predicted BMR. The influences on TEE and PAL of body size, gender, sexual maturity, physical activity habits, sitting time, region, social conditions, employment, food habits, smoking and alcohol intake were examined in multivariate analyses.Subjects:71 male and 89 female subjects, living in two socioeconomically different regions of Sweden, a university region and an industrial region. SETTING: The university city of Uppsala and the industrial town of Trollhättan. RESULTS: At both 15 and 21 y, PAL and TEE were high, with gender, sitting time and physical activity habits as main correlates. At age 21 y, employment and the mother's educational level also appeared as significant correlates. The relations between the main variables and their correlates were more complex than at age 15 y, and the sitting time was reduced and the activity habits were changed. CONCLUSIONS: A reduction of daily sitting appears to be a major reason why high levels of physical activity and energy expenditure were maintained from 15 to 21 y of age in spite of changed and less frequent activity habits during this interval.

  • Numerical analysis of an Uppsala University WEC deployment by a barge for different sea states

    • Wave energy converters (WECs) have been deployed onshore, nearshore, and offshore to convert ocean wave movement into electricity. The exploitation of renewable energy sources has restrictions; in the case of wave energy, high installation, maintenance, and decommissioning costs have limited their commercial use. Moreover, these offshore operations can be compromised by safety issues. This paper draws attention to offshore operation safety of a WEC developed by Uppsala University. Specifically, this paper investigates what sea states are suitable for the safe deployment of a WEC from a barge. This study follows recommendations in DNV-RP-H103 for analysis of offshore operations, namely lifting through the wave zone. ANSYS Aqwa is used to find hydrodynamic forces acting on a typical barge, using frequency domain analysis. Based on these hydrodynamic simulation results and methodology given in DNV-RP-H103, tables are created to show the sea states that would allow for the safe installation of a WEC using a typical barge. Considered sea states have significant wave heights varying between 0 m and 3 m and the wave zero crossing periods varying between 3 s and 13 s. The WEC submersions are considered between 0 m and 7 m, i.e. when the WEC is in the air until it is fully submerged. © 2020 Elsevier Ltd

  • Unscripted sexual practices : Sexual health of men having sex with men after prostate cancer treatment

    • Purpose: The purpose of the study was to describe the sexual health of men having sex with men after prostate cancer treatment.Methods: A snowball sampling was conducted via clinicians and gatekeepers to the community. After consenting to participate, a semi-structured interview was conducted based on a questionnaire related to symptoms and issues and the help asked for and received. The interviews were approximately one hour and took place via digital platforms. The interviews were transcribed verbatim and then analyzed using content analysis. The following themes emerged during the analysis: A predominant heteronormative care, A situation of suffering and possibilities, A window of peer support system emerged.Results: The men described how their sexuality had been severely affected by their treatments. Bodily changes with decreased penis size, difficulties maintaining an erection, and less pleasure from anal sex. However, they also described the benefit of being men who had sex with men. They described being accustomed to exploring new ways of expressing and exploring their sexualities due to a lack of sexual scripts. The prostate cancer treatment had given them a new reason to explore new sexualities and sexual practices. In contacts with urology and oncology departments, both physicians and nurses, the men described a heteronormative environment. Albeit same-sex partners could be asked for in intake notes, no one reflected on what non-penovaginal sexuality could look like. Therefore, getting the appropriate care for the men after prostate cancer treatment did not involve erectile dysfunction medications was challenging. Instead, their friends and ability to find alternative ways to valid information became important.Conclusion: The result shows that predominantly heteronormative care exists for men who have sex with men. The new insight of the treatment is regarded as a situation of suffering and new possibilities. To conclude, men who have sex with men describe care that was well-meaning but heteronormative. The men described being men having sex with men as something that had made them explore unscripted sexual practices and had given them a broader sexual repertoire that was helpful for their sexual health. Furthermore, the support they received emerged from a peer support system in their private life.

    • Young delinquents are known to be sensation seekers. Not all sensation seekers become delinquents: many engage in socially accepted activities, such as mountaineering or parachute jumping. The present study compares 47 juvenile delinquents (mean age 17 years) with 18 Swedish air force pilot recruits (mean age 23 years) and 19 conscripts (mean age 18 years) as a control group. Sensation-seeking behaviour, impulsiveness, and psychiatric/psychological vulnerability were measured by the Zuckerman Sensation-Seeking Scales (SSS), the Karolinska Scales of Personality, and the Eysenck Personality Questionnaire. Two separate multivariate analyses of variance were performed, followed up by stepdown analyses to identify those personality scale scores that contributed uniquely. In order to clarify the relationships, the pooled within-group correlations among scales were computed. Juvenile delinquents and pilot recruits were both high in sensation seeking, but on different subscales. Delinquents were high in impulsiveness, somatic anxiety, and extraversion–sociability, and low in socialization, suggesting psychiatric/psychological vulnerability.     • BACKGROUND: An intact umbilical cord allows the physiological transfusion of blood from the placenta to the neonate, which reduces infant iron deficiency and is associated with improved development during early childhood. The implementation of delayed cord clamping practice varies depending on mode of delivery, as well as gestational age and neonatal compromise. Emerging evidence shows that infants requiring resuscitation would benefit if respiratory support were provided with the umbilical cord intact. Common barriers to providing intact cord resuscitation is the availability of neonatal resuscitation equipment close to the mother, organizational readiness for change as well as attitudes and beliefs about placental transfusion within the multidisciplinary team. Hence, clinical evaluations of cord clamping practice should include implementation outcomes in order to develop strategies for optimal cord management practice.METHODS: The Sustained cord circulation And Ventilation (SAVE) study is a hybrid type I randomized controlled study combining the evaluation of clinical outcomes with implementation and health service outcomes. In phase I of the study, a method for providing in-bed intact cord resuscitation was developed, in phase II of the study the intervention was adapted to be used in multiple settings. In phase III of the study, a full-scale multicenter study will be initiated with concurrent evaluation of clinical, implementation and health service outcomes. Clinical data on neonatal outcomes will be recorded at the labor and neonatal units. Implementation outcomes will be collected from electronic surveys sent to parents as well as staff and managers within the birth and neonatal units. Descriptive and comparative statistics and regression modelling will be used for analysis. Quantitative data will be supplemented by qualitative methods using a thematic analysis with an inductive approach.DISCUSSION: The SAVE study enables the safe development and evaluation of a method for intact cord resuscitation in a multicenter trial. The study identifies barriers and facilitators for intact cord resuscitation. The knowledge provided from the study will be of benefit for the development of cord clamping practice in different challenging clinical settings and provide evidence for development of clinical guidelines regarding optimal cord clamping.TRIAL REGISTRATION: Clinicaltrials.gov, NCT04070560 .     • Even though the magnetic circuit of a linear electric machine is very similar to a rotating electric machine, they diverge in one fundamental property. The linear generator is open in both ends, i.e., the magnetic circuit is non-symmetric. This paper investigates and discusses the drawbacks of this non-symmetric design in a linear permanent magnet generator, installed in a wave energy conversion system. A two-dimensional geometry has been utilized for the numerical calculations in a finite element method simulation tool. The results present an increased cogging force and significant core losses in the translator as consequences of the longitudinal ends in the machine.

    • Background The aim was to explore the thalamo-striato-cortical theory of central fatigue in multiple sclerosis (MS) patients with self-reported fatigue. If the theory correctly predicted fatigue based on disruptions of the thalamo-striato-cortical network, we expected altered brain activation in this network in MS participants while performing a complex cognitive task that challenged fatigue.
