  • Early phase clinical trials in pediatric oncology : Swedish pediatric oncologists’ experiences of balancing hope and expectations in life-threatening illnesses

    • Aim: To study Swedish pediatric oncologists’ practical and emotional experiences of referring, including and/or treating children in early-phase clinical trials.Methods: A nationwide study was conducted using a mixed-method approach. Structured interviews based on a study-specific questionnaire and participants’ personal reflections were utilized. Survey responses were analyzed using descriptive statistics, while participants’ comments were analyzed using thematic analysis. All interviews were recorded and transcribed verbatim.Results: In total, 29 physicians with 4 to 32 years of experience in pediatric oncology participated, with 19 (66%) having > 10 years of experience. Three themes appeared: 1) Optimization-based approach focused on finding the most suitable treatment and care for every child with a refractory/relapsed cancer eligible for an early-phase clinical trial; 2) Team-based approach aimed at establishing local and national consensus in decision-making for treatment options, including early-phase clinical trials and palliative care; 3) Family-based approach in which the physicians provided families with actionable information, listened to their desires, and endeavored to maintain hope in challenging circumstances. Several participants (40% with ≤ 10 years of experience and 58% with > 10 years of experience) viewed the early-phase clinical trial as a potential “chance of cure”. A majority (80%) of physicians with ≤ 10 years of experience, reported that they often or always felt personally and emotionally affected by communication regarding early-phase clinical trials. Delivering difficult news in cases of uncertain prognosis was identified as the major challenge. None of the study participants felt adequately prepared in terms of sufficient knowledge and experience regarding early-phase clinical trials. The physicians expressed a need for guidance and training in communication to address these challenges.Conclusions: Working with early-phase clinical trials highlight a field where physicians cannot solely rely on their expertise or past experiences, and where they are likely to be deeply emotionally involved. Physicians who care for children eligible for such studies require targeted educational initiatives and supervision.

  • Achieving balance in everyday life : Insights from women with stress-related disorders

    • There is growing interest in developing a conceptual understanding of the experience of balance in everyday life, from an occupational perspective. The purpose of this study was to gain insights about balance in the everyday lives of women with stress-related disorders. Data were gathered from 19 women who were past the first phase of recovery from a stress-related disorder and participated in one of five focus groups. Analysis revealed that the participants experienced a continuum between imbalance and balance in everyday life. The themes that emerged were image of occupational self, strategies to manage and control everyday life, occupational repertoire, and occupational experience. Balance in everyday life was achieved through a dynamic interaction between these themes, which the women characterised as respecting their own values, needs, and resources; having strategies to manage and control everyday life; having a harmonious occupational repertoire; and engaging in personally meaningful occupation. Engagement in personally meaningful occupation appears to be a mechanism that enables people to achieve balance in everyday life by enabling them to develop a successful occupational selfimage, manageability, control, and a harmonious occupational repertoire. Well-being seems to be the outcome of balance in everyday life, and lack of balance is experienced as overload.

  • Aktivitetsbalans – ett instrument för att mäta självskattad aktivitetsbalans (OBQ11)

    • Bakgrund/SyfteAktivitetsbalans definieras som att: ”uppfatta livet som innehållande rätt mängd och rätt variation mellan olika aktiviteter i relation till: aktivitetskategorier, aktiviteter med olika karaktäristika samt tidsanvändning”. Aktivitetsbalans ses alltså som personens egna subjektiva upplevelse av sin mix av aktiviteter i aktivitetsmönstret oberoende av vilka aktiviteter som ingår. Baserat på denna definition utvecklades ett instrument för självskattad aktivitetsbalans (OBQ11) som är reliabilitets- och validitetstestat och avsett att användas både i praxis och forskning.Metod/TillvägagångssättInstrumentet har utvecklats stegvis. Först identifierades ett antal indikatorer på aktivitetsbalans baserat på forskning samt erfarenheter från praxis. Utifrån dessa indikatorer utvecklades ett antal påståenden som granskades av experter inom området. Baserat på deras synpunkter omformulerades och reducerades antalet påståenden. Denna första version testades med klassisk testteori. Därefter gjordes en Raschanalys och antalet påståenden reducerades till nuvarande version, OBQ11.Resultat/Preliminärt resultatInstrumentet består av 11 påståenden där svaren kan användas som en screening för att få en bild av hur aktivitetsbalansen ser ut. I praxis behöver OBQ11 kompletteras med en dialog för att få en större förståelse för personens aktivitetsbalans och vad som eventuellt behöver förändras för att personen ska uppleva en bättre aktivitetsbalans. Instrumentet kan också användas som uppföljning och utvärdering av interventioner i både praxis och forskning. Instrumentet självskattad aktivitetsbalans (OBQ11) ges from hösten 2022 ut av Sveriges Arbetsterapeuter.Slutsats/Praktisk tillämpningInstrumentet självskattad aktivitetsbalans (OBQ11) är testat i olika grupper, både i normalpopulationer och i olika diagnosgrupper och är validerat och reliabelt för dessa grupper. Det är ännu inte testat på barn och ungdomar.

    • Background ⁄ aim: More knowledge is needed about the role of perceived control in the associations between different perceptions of daily occupations and positive health outcomes. The aim was to explore the associations between different subjective perceptions of daily occupations, in terms of occupational balance, occupational meaning, occupational value and satisfaction with occupations, and life satisfaction, and the role of perceived control in those associations. Methods: A questionnaire including questions about perceptions of daily occupations, perceived control and life satisfaction were answered by a random sample of 488 middle-aged Swedish women. Multiple logistic regression analysis was used to test the associations between perceptions of daily occupations and life satisfaction, and the role of perceived control. Results: After adjustments for perceived control women who perceived a high level of satisfaction with work and leisure, occupational balance, occupational meaning and occupational value perceived greater life satisfaction than the other women. Perceived control was not significant in the model. Conclusions: It seems that occupational balance and occupational meaning were pivotal for the women’s life satisfaction, but satisfaction with work and leisure, as well as perceived occupational value, was also of importance. The assumption that perceived control would have a role in the association between perceptions of occupations and life satisfaction was not confirmed. Significance of the study: The results indicate that occupational therapists need to focus on occupational balance, occupational meaning, occupational value and satisfaction with work and leisure to promote positive health outcomes, in terms of life satisfaction, when working with middle-aged female clients.

    • Background: Occupational balance is fundamental to occupational therapy and occupational science. Therefore, the Occupational Balance Questionnaire (OBQ) was developed and has previously been found psychometrically valid according to classical test theory. Aim: To investigate the internal construct validity of the OBQ using Rasch measurement theory. Material and methods: Data from two general population samples were used to investigate the psychometric properties of the OBQ according to Rasch measurement theory. Results: The analyses identified problems with the current response scale and multidimensionality of two items. As a result, a revised version, the OBQ11, was suggested and exhibited response categories that worked properly, good reliability (0.92), model fit and measurement invariance across age and gender groups. The hierarchical item ordering was in agreement with previous research. Conclusion: The new OBQ11 satisfies the measurement criteria defined by the Rasch model. However, further studies of additional samples are needed to validate its generic properties. Significance: The purpose of the OBQ11 is to measure occupational balance of individuals or groups, and to identify aspects of occupational balance in need of improvement. Our observations suggest that the OBQ11 is a valid and promising complement to other instruments.

    • The aim of the present cross-sectional study was to examine how subjective experiences of engagement in patterns of daily occupations (gainful employment, domestic work, enjoyable and recreational occupations) were associated with perceived health among women of working age. The sample (n_488) was drawn from a longitudinal cohort study of women of working age in Gothenburg, Sweden. Participants were women 38 (n_202) and 50 (n_286) years of age. They completed a questionnaire including questions about occupational experiences in relation to their patterns of daily occupations, perceived health, and socioeconomic factors. The results of the present study showed that a combination of different experience dimensions of patterns of daily occupations was associated with perceived health among women of working age, even when adjusted for socioeconomic factors and age. The results provided occupational pattern-related health indicators, i.e. manageability, personally meaningful occupations, and occupational balance. To combine these health indicators can be a way for occupational therapists to enable women to develop strategies to promote health and to prevent stress and sick leave.
