Long term risk stratification of patients with acute coronary syndromes: characteristics of troponin T testing and continuous ST segment monitoring

- OBJECTIVE: To examine the long term prognostic characteristics of troponin T testing and continuous multi-lead ST segment monitoring in combination with clinical and 12 lead ECG risk indicators in patients with acute coronary syndromes (ACS). PATIENTS AND DESIGN: Patients with suspected ACS (n = 213) were studied. Troponin T was analysed in blood samples collected during the first 12 hours after admission. Continuous vectorcardiography ST segment monitoring was performed for 24 hours and the number of ST vector magnitude episodes was registered. Patients were followed up for a median of 28 months. The end point was a composite of cardiac death and acute myocardial infarction. RESULTS: Thirty eight (18%) patients reached the composite end point. The median (interquartile range) time from study inclusion to the time of the composite end point was longer for patients predicted to be at risk by troponin T testing (n = 27) than for those predicted to be at risk by ST segment monitoring (n = 20) (8.4 (0.2-15) months v 0.3 (0.1-4.3) months, p = 0.04). Significant univariate predictors of the composite end point were age > or = 65 years, diabetes, previous myocardial infarction, congestive heart failure, use of beta blockers or diuretics at admission, 12 lead ECG ST segment depression at admission, troponin T concentration > or = 0.10 microg/l, and > or = 1 ST vector magnitude episodes. Age > or = 65 years, previous myocardial infarction, and troponin T concentration > or = 0.10 microg/l provided independent prognostic information after multivariate analysis of potential risk variables. The prognostic value of transient ischaemic episodes in ACS seems to be confined to the short term. CONCLUSIONS: Both biochemical and continuous ECG markers reflect an increased risk for patients with ACS; however, the methods exhibit different temporal risk characteristics.

- The poorly known, so called widely distributed and highly varying species of the European Rhyacophila fasciata species complex are revised by fine phenomics of the paramere organisation. In this species complex paramere is the most diverse organ. It is the speciation trait integrating the initial split of speciation by its stimulatory and titillating function involved in the early processes of reproductive isolation. Based on paramere organisation and on the character state of distribution three lineages have been delineated in the Rhyacophila fasciata species complex: the European R. fasciata lineage, the Caucasian R. aliena lineage and the R. mysica lineage distributed from Albania to Pakistan. In the R. fasciata lineage we have distinguished three clades of species: R. fasciata with five species, R. matrensis with seven species, and R. denticulata with seven species. We have re-diagnosed three known species: R. fasciata Hagen, 1859, R. denticulataMcLachlan, 1879, R. sociata Navas, 1916 and described 15 species new to science: R. biharensis Oláh sp. nov., R. bulgarica Oláh sp. nov., R. coppai Oláh sp. nov., R. csornahorensis Oláh & Szczęsny sp. nov., R. ferda Oláh sp. nov., R. kopasa Oláh & Coppa sp. nov., R. matrensis Oláh, & Szczęsny sp. nov., R. retezatensis Oláh sp. nov., R. rova Oláh & Coppa sp. nov. R. ruda Oláh & Johanson sp. nov., R. salfa Oláh sp. nov., R. soreda Coppa & Oláh sp. nov. R. suna Oláh sp. nov., R. tuhega Oláh sp. nov., R. zemplenensis Oláh sp. nov. The species status of Rhyacophila gemella Navas, 1923 was reinstated.

Carcinoma in situ of the breast: correlation of histopathology to immunohistochemical markers and DNA ploidy

- In a consecutive and unselected series of 178 cases of carcinoma in situ of the breast (CIS), comprising both ductal (DCIS) and lobular type (LCIS), and a series of 48 cases of invasive carcinoma (IC) with predominance of DCIS, the association between histopathology, immunohistochemical markers (ER, PgR, MIB-1, c-erbB-2, and p53), and DNA ploidy was investigated, in order to discriminate biologically different groups. In DCIS, significant correlation was shown between large nuclear size and comedonecrosis, both of which showed also strong association to DNA aneuploidy, high proliferation activity, low steroid receptor content, and overexpression of c-erbB-2 and p53 factors that may indicate an aggressive behavior. Small nuclear CIS, whether LCIS or DCIS, on the contrary, were DNA diploid with low proliferation, and no cases showed overexpression of c-erbB-2 and p53. Heterogeneity with respect to the investigated parameters was also a frequent finding that may reflect a development complexity. In IC, comparison of the DCIS and the invasive component showed similar patterns. No significant differences were shown between DCIS without and with invasion. This may indicate that none of the investigated parameters on its own are essential for the event of invasion.

- High-pressure Raman and mid-infrared spectroscopic studies were carried out on ZrP2O7 to 23.2 and 13 GPa respectively. In the pressure range 0.7-4.3 GPa the lattice mode at 248 cm(-1) disappears, new modes appear around 380 and 1111 cm(-1) and the strong symmetric stretching mode at 476 cm(-1) softens, possibly indicating a subtle phase transition. Above 8 GPa all the modes broaden, and all of the Raman modes disappear beyond 18 GPa. On decompression from the highest pressure, 23.2, to 0 GPa all of the modes reappear but with larger full width at half maximum. Lattice dynamics of the high temperature phase of ZrP2O7 were studied using first principles method and compared with experimental values.

- Supervision enhances professional functioning, helps ensure quality services, and fills a gatekeeping function for the profession. This International Society of Sport Psychology (ISSP) Position Stand synthesises the most pertinent literature on supervision practices relevant to sport psychology (SP), builds on the collective supervision experience of the authors and the present ISSP Managing Council, and offers recommendations for competent, ethical, and culturally safe supervision. Specifically, after defining supervision and describing supervision models and their relational features, we review the scholarly contributions in the areas of supervision content and methods (including tele-supervision), along with cultural, linguistic, ethical, and legal considerations. We conclude with a set of nine postulates that are further operationalised through recommendations for competent supervision practices. © 2023 International Society of Sport Psychology.
