Efficacy of dapagliflozin in heart failure with reduced ejection fraction according to body mass index.

- In heart failure with reduced ejection fraction (HFrEF), there is an 'obesity paradox', where survival is better in patients with a higher body mass index (BMI) and weight loss is associated with worse outcomes. We examined the effect of a sodium-glucose co-transporter 2 inhibitor according to baseline BMI in the Dapagliflozin And Prevention of Adverse-outcomes in Heart Failure trial (DAPA-HF).Body mass index was examined using standard categories, i.e. underweight (<18.5 kg/m2 ); normal weight (18.5-24.9 kg/m2 ); overweight (25.0-29.9 kg/m2 ); obesity class I (30.0-34.9 kg/m2 ); obesity class II (35.0-39.9 kg/m2 ); and obesity class III (≥40 kg/m2 ). The primary outcome in DAPA-HF was the composite of worsening heart failure or cardiovascular death. Overall, 1348 patients (28.4%) were under/normal-weight, 1722 (36.3%) overweight, 1013 (21.4%) obesity class I and 659 (13.9%) obesity class II/III. The unadjusted hazard ratio (95% confidence interval) for the primary outcome with obesity class 1, the lowest risk group, as reference was: under/normal-weight 1.41 (1.16-1.71), overweight 1.18 (0.97-1.42), obesity class II/III 1.37 (1.10-1.72). Patients with class I obesity were also at lowest risk of death. The effect of dapagliflozin on the primary outcome and other outcomes did not vary by baseline BMI, e.g. hazard ratio for primary outcome: under/normal-weight 0.74 (0.58-0.94), overweight 0.81 (0.65-1.02), obesity class I 0.68 (0.50-0.92), obesity class II/III 0.71 (0.51-1.00) (P-value for interaction = 0.79). The mean decrease in weight at 8 months with dapagliflozin was 0.9 (0.7-1.1) kg (P < 0.001).We confirmed an 'obesity survival paradox' in HFrEF. We showed that dapagliflozin was beneficial across the wide range of BMI studied.ClinicalTrials.gov NCT03036124.

- Next-Generation Sequencing (NGS) technologies are expected to play a crucial role in the surveillance of infectious diseases, with their unprecedented capabilities for the characterisation of genetic information underlying the virulence and antimicrobial resistance (AMR) properties of microorganisms. In the implementation of any novel technology for regulatory purposes, important considerations such as harmonisation, validation and quality assurance need to be addressed. NGS technologies pose unique challenges in these regards, in part due to their reliance on bioinformatics for the processing and proper interpretation of the data produced. Well-designed benchmark resources are thus needed to evaluate, validate and ensure continued quality control over the bioinformatics component of the process. This concept was explored as part of a workshop on "Next-generation sequencing technologies and antimicrobial resistance" held October 4-5 2017. Challenges involved in the development of such a benchmark resource, with a specific focus on identifying the molecular determinants of AMR, were identified. For each of the challenges, sets of unsolved questions that will need to be tackled for them to be properly addressed were compiled. These take into consideration the requirement for monitoring of AMR bacteria in humans, animals, food and the environment, which is aligned with the principles of a "One Health" approach.

- Identification of host volatile compounds attractive to codling moth Cydia pomonella, a most important insect of apple, will contribute to the development of safe control techniques. Synthetic apple volatiles in two doses were tested for antennal and behavioural activity in codling moth. Female antennae strongly responded to (Z)3-hexenol, (Z)3-hexenyl benzoate, (Z)3-hexenyl hexanoate, (+/-)-linalool and E,E-alpha-farnesene. Two other compounds eliciting a strong antennal response were the pear ester, ethyl (E,Z)-2,4-decadienoate, and its corresponding aldehyde, E,E-2,4-decadienal, which is a component of the larval defence secretion of the European apple sawfly. Attraction of codling moth to compounds eliciting a strong antennal response was tested in a wind tunnel. Male moths were best attracted to a blend of (E,E)-alpha-farnesene, (E)-beta-farnesene and ethyl (E,Z)-2,4-decadienoate. The aldehyde E,E-2,4-decadienal had an antagonistic effect when added to the above mixture.

- A Galois unitary is a generalization of the notion of anti-unitary operators. They act only on those vectors in Hilbert space whose entries belong to some chosen number field. For Mutually Unbiased Bases the relevant number field is a cyclotomic field. By including Galois unitaries we are able to remove a mismatch between the finite projective group acting on the bases on the one hand, and the set of those permutations of the bases that can be implemented as transformations in Hilbert space on the other hand. In particular we show that there exist transformations that cycle through all the bases in all dimensions d - p(n) where p is an odd prime and the exponent n is odd. (For even primes unitary MUB-cyclers exist.) These transformations have eigenvectors, which are MUB-balanced states (i.e. rotationally symmetric states in the original terminology of Wootters and Sussman) if and only if d = 3 modulo 4. We conjecture that this construction yields all such states in odd prime power dimension.

- It is known that if the dimension is a perfect square the Clifford group can be represented by monomial matrices. Another way of expressing this result is to say that when the dimension is a perfect square the standard representation of the Clifford group has a system of imprimitivity consisting of one dimensional subspaces. We generalize this result to the case of an arbitrary dimension. Let k be the square-free part of the dimension. Then we show that the standard representation of the Clifford group has a system of imprimitivity consisting of k-dimensional subspaces. To illustrate the use of this result we apply it to the calculation of SIC-POVMs (symmetric informationally complete positive operator valued measures), constructing exact solutions in dimensions 8 (hand-calculation) as well as 12 and 28 (machine-calculation).

- We show that the Clifford group-the normaliser of the Weyl-Heisenberg group-can be represented by monomial phase-permutation matrices if and only if the dimension is a square number. This simplifies expressions for SIC vectors, and has other applications to SICs and to Mutually Unbiased Bases. Exact solutions for SICs in dimension 16 are presented for the first time.
