    • The Macrocyclic Lactone and Lactide derivative (ML) group of fragrance ingredients was critically evaluated for safety following a complete literature search. For high end users, calculated maximum dermal exposures vary from 0.47% to 11.15%; systemic exposures vary from 0.0008 to 0.25 mg/kg/day. The MLs had low acute toxicity and no significant toxicity in repeat dose oral ordermal toxicity studies. Effects on blood biochemistry were reversible after 2 weeks of no treatment. No mutagenic or genotoxic activity in bacteria and mammalian cell line assays was observed. Reproductive and developmental toxicity was not observed. Human dermatological studies show MLs are generally not irritating after one application. Minor irritation was observed in a few individuals following multiple applications. At rates consistent with reported levels for current human exposure, no phototoxicity or photosensitization was observed. In animal studies, the MLs are not sensitizers at lower exposures from consumer products. Eleven ML materials were evaluated for human sensitization. Of these, only ethylene brassylate showed evidence of sensitization in 2/27 studies (sensitization frequency 4/2059 total). Based on these findings, the Panel is of the opinion that there are no safety concerns for the MLs at reported levels of use and exposure as fragrance ingredients. (C) 2011 Published by Elsevier Ltd.

  • Occupational allergic contact dermatitis to gloves, follow-up of investigated patients

    • Background: In the Department of Occupational and Environmental Dermatology there has been an increase in the inflow of patients, working as surgical nurses or surgeons. They have been referred due to presumed occupational contact dermatitis. Results from investigated patients have in part been presented previously, where the investigation has shownmultiple contact allergies caused by their occupational use of gloves. Allergens such as diphenylguanidine were found in the gloves and contact allergies to their rubber gloves were found. Many of the patients have been working for decades in their present occupation, but the hand dermatitis had often been present for less than a year. Objectives: To present a follow-up of patientswith hand dermatitis due to gloves. Methods: A follow-up of the patients and a structure for the investigation of the individual patient are given. Results: The cases will be presented with their contact allergies, howtheinvestigation isperformedandtheoutcomeof theprevention by avoiding gloves containing the allergens diagnosed in the individual patients. Conclusions: For health care personnel hand hygiene and the wearing of protective gloves are mandatory. Our findings indicate that the increased need to use rubber gloves and at the same time the exclusion of latex gloves give rise to new contact allergies which are clinically relevant. In the investigation, and the advice given to the patients, several factors have to be taken into consideration, including the fast change of glove material. The cases indicate the importance of close cooperation between chemists and dermatologists and the possibility to perform chemical analysis. Furthermore, and just as important for this patient group, is that with correct diagnosis and advice the contact dermatitis can actually heal completely and the patients continue in their work.

  • The reactivity of the back revisited. Are there differences in reactivity in different parts of the back?

    • Background: In the contact dermatitis literature, it is regularly stated that the patch test reactivity on various areas of the back differs; this might have a large impact on the reproducibility of patch testing. Objectives: To investigate the reproducibility of patch testing on the upper back with regard to the left as opposed to the right side and the medial as opposed to the lateral part of the upper back. The reproducibility over time and with regard to the reactivity pattern was also investigated. Methods: Thirty-one subjects with contact allergy to the metals gold (n=19) or nickel (n=12) were patch tested with serial dilutions, in triplicate applications, on different locations on the upper back. The Friedman test was used for statistical calculations. Results: No significant differences in the reactivity of the back were found. In all gold-allergic patients and 11 of 12 nickel-allergic patients, the allergy could be reproduced with regard to previous patch testing, but the degree of reactivity differed. Conclusions: When a high level of standardization of the patch test technique with the same test system was used, there were no differences in patch test reactions and sites of application on the upper back.

    • Paralysis of the orbicularis oculi muscle in patients with a facial nerve injury causes functional and cosmetic disabilities including inability to close the eyelid. This complication is often treated by implanting a gold weight in the upper eyelid, which, however, is not without side effects. Four patients are described who, after lid loading with a gold implant, acquired an inflammatory reaction due to contact allergy to the metal inserted. The allergy was demonstrated by patch testing with gold sodium thiosulfate. The implant was removed and the dermatitis resolved. Contact allergy to gold occurs frequently and may explain many cases of complications to lid loading with the metal.

    • A 5-year retrospective study of the frequency of sensitization to the 25 allergens of the European standard series (ESS) was conducted in 10 centres in 8 European countries. Included were the results of 26 210 patients. The range in sensitivities differed moderately between the centres. Combining results of different centres and drawing conclusions on incidences can be done only with great care. The information on the ranking of the allergens and their sensitization incidence in the clinics are useful for decisions on the future composition of the standard series. The ESS is still a valid screening tool, and no substances should be deleted.

    • For investigation of chemical exposure in allergic individuals, spot tests are invaluable. A reagent may react with a specific compound to give a specific typical colour, and thus indicate the presence of the specific substance. Spot tests can give both false-negative and false-positive reactions. To confirm the presence of the substance and quantify it, more sophisticated methods are required. On the basis of a positive cobalt spot test result, a woman was initially diagnosed with an occupational allergic contact dermatitis caused by cobalt.

  • A Retrospective Study of Aimed Patch Testing with Aqueous Nickel Sulfate Hexahydrate at 30% and 15% in Patients with Dermatitis

    • Background Sometimes, patients with a history of metal intolerance react negatively or doubtfully to nickel at patch testing. Objective The aim of the study was to investigate whether aqueous nickel sulfate hexahydrate at 15% and 30% traces more contact allergy than the nickel preparation in the baseline series at 5%. Methods In the period 1995-2015, more than 800 of 16,059 patients with a negative or doubtful patch test reaction to the petrolatum preparation of nickel at 5% on the day 3 reading were additionally tested with aqueous nickel at 15% and/or 30%. Conclusions In this retrospective study with aimed patch testing with aqueous nickel solutions, significantly more (P < 0.001) contact allergy was obtained as compared with the 5% preparation in petrolatum. The 30% solution was significantly better (P < 0.001) than the 15% solution. To demonstrate that patch testing with nickel at 15% or 30% is better than the present testing with 5% requires simultaneous testing with the 3 nickel preparations.
