    • Background: Limited therapeutic options exist for severe gastroparesis, where severe nausea and vomiting can lead to weight loss, dehydration and malnutrition due to inadequate caloric and fluid intake. TZP-101 (ulimorelin) is a ghrelin receptor agonist that accelerates gastric emptying and improves upper gastrointestinal symptoms in diabetic patients with gastroparesis. Aim To assess effects of TZP-101 in diabetic gastroparesis patients with severe nausea/vomiting and baseline severity scores of >= 3.5 (range: 0-5) on the Gastroparesis Cardinal Symptom Index (GCSI) Nausea/Vomiting subscale. Methods Patients were hospitalised and received four single daily 30-min infusions of one of six TZP-101 doses (range 20-600 mu g/kg) or placebo. Efficacy was assessed by symptom improvement. Results At baseline, 23 patients had a mean severity score for GCSI Nausea/Vomiting of 4.45 +/- 0.44. Statistically significant improvements over placebo occurred in the 80 mu g/kg group for end of treatment changes from baseline in GCSI Nausea/Vomiting subscale (reduction in score of -3.82 +/- 0.76, P = 0.011) and the GCSI Total score (-3.14 +/- 0.78, P = 0.016) and were maintained at the 30-day follow-up assessment (-2.02 +/- 1.63, P = 0.073 and -1.99 +/- 1.33, P = 0.032 respectively). The proportion of days with vomiting was reduced significantly (P = 0.05) in the 80 mu g/kg group (mean of 1.2 days of vomiting for four treatment days) compared with placebo (mean of 3.2 days of vomiting across 4 treatment days). Conclusions TZP-101 substantially reduced the frequency and severity of nausea and vomiting as well as overall gastroparesis symptoms. The results are consistent with gastrointestinal motility effects of TZP-101, supporting further investigation of TZP-101 in the management of severe gastroparesis.

    • We report the discovery of a ring-like cluster complex in the starburst galaxy NGC?2146. The Ruby Ring, so named due to its appearance, shows a clear ring-like distribution of star clusters around a central object. It is located in one of the tidal streams that surround the galaxy. NGC?2146 is part of the Snapshot Hubble U-band Cluster Survey (SHUCS). The WFC3/F336W data have added critical information to the available archival Hubble Space Telescope imaging set of NGC?2146, allowing us to determine ages, masses and extinctions of the clusters in the Ruby Ring. These properties have then been used to investigate the formation of this extraordinary system. We find evidence of a spatial and temporal correlation between the central cluster and the clusters in the ring. The latter are about 4?Myr younger than the central cluster, which has an age of 7?Myr. This result is supported by the Ha emission which is strongly coincident with the ring, and weaker at the position of the central cluster. From the derived total Ha luminosity of the system, we constrain the star formation rate density to be quite high (SSFR = 0.47?M??yr-1?kpc-2). The Ruby Ring is the product of an intense and localized burst of star formation, similar to the extended cluster complexes observed in M?51 and the Antennae, but more impressive because it is quite isolated. The central cluster contains only 5 per cent of the total stellar mass in the clusters that are determined within the complex. The ring-like morphology, the age spread and the mass ratio support a triggering formation scenario for this complex. We discuss the formation of the Ruby Ring in a collect and collapse framework. The predictions made by this model agree quite well with the estimated bubble radius and expansion velocity produced by the feedback from the central cluster, making the Ruby Ring an interesting case of triggered star formation.

    • A prototype optical data and Timing Trigger and Control transmission system based on LEDs and PIN-diodes has been constructed. The system would be suitable in terms of radiation hardness and radiation length for use in the ATLAS SemiConductor Tracker. Bit error rate measurements were performed for the data links and for the links distributing the Timing, Trigger and Control data from the counting room to the front-end modules. The effects of cross-talk between the emitters and receivers were investigated. The advantages of using Vertical Cavity Surface Emitting Lasers (VCSELs) instead of LEDs are discussed.

    • Pathogenic variants in the LRP2 gene, encoding the multiligand receptor megalin, cause a rare autosomal recessive syndrome: Donnai-Barrow/Facio-Oculo-Acoustico-Renal (DB/FOAR) syndrome. Because of the rarity of the syndrome, the long-term consequences of the tubulopathy on human renal health have been difficult to ascertain, and the human clinical condition has hitherto been characterized as a benign tubular condition with asymptomatic low-molecularweight proteinuria. We investigated renal function and morphology in a murine model of DB/FOAR syndrome and in patients with DB/FOAR. We analyzed glomerular filtration rate in mice by FETC-inulin clearance and clinically characterized six families, including nine patients with DB/FOAR and nine family members. Urine samples from patients were analyzed by Western blot analysis and biopsy materials were analyzed by histology. In the mouse model, we used histological methods to assess nephrogenesis and postnatal renal structure and contrast-enhanced magnetic resonance imaging to assess glomerular number. In megalin-deficient mice, we found a lower glomerular filtration rate and an increase in the abundance of injury markers, such as kidney injury molecule-1 and N-acetyl-11-n-glucosaminidase. Renal injury was validated in patients, who presented with increased urinary kidney injury molecule-1, classical markers of chronic kidney disease, and glomerular proteinuria early in life. Megalin-deficient mice had normal nephrogenesis, but they had 19% fewer nephrons in early adulthood and an increased fraction of nephrons with disconnected glomerulotubular junction. In conclusion, megalin dysfunction, as present in DB/FOAR syndrome, confers an increased risk of progression into chronic kidney disease.

    • We present an Embodied Conversational Agent (ECA) that incorporates a context-sensitive mechanism for handling user barge-in. The affective ECA engages the user in social conversation, and is fully implemented. We will use actual examples of system behaviour to illustrate. The ECA is designed to recognise and be empathetic to the emotional state of the user. It is able to detect, react quickly to, and then follow up with considered responses to different kinds of user interruptions. The design of the rules which enable the ECA to respond intelligently to different types of interruptions was informed by manually analysed real data from human-human dialogue. The rules represent recoveries from interruptions as two-part structures: an address followed by a resumption. The system is robust enough to manage long, multi-utterance turns by both user and system, which creates good opportunities for the user to interrupt while the ECA is speaking.

    • Increased hepatic de novo lipogenesis (DNL) is suggested to be an underlying cause in the development of nonalcoholic fatty liver disease and/or insulin resistance. It is suggested that omega-3 fatty acids (FA) lower hepatic DNL. We investigated the effects of omega-3 FA supplementation on hepatic DNL and FA oxidation using a combination of human in vivo and in vitro studies.Research design and methodsThirty-eight healthy men were randomized to take either an omega-3 supplement (4 g/day eicosapentaenoic acid (EPA)+docosahexaenoic acid (DHA) as ethyl esters) or placebo (4 g/day olive oil) and fasting measurements were made at baseline and 8 weeks. The metabolic effects of omega-3 FAs on intrahepatocellular triacylglycerol (IHTAG) content, hepatic DNL and FA oxidation were investigated using metabolic substrates labeled with stable-isotope tracers. In vitro studies, using a human liver cell-line was undertaken to gain insight into the intrahepatocellular effects of omega-3 FAs.ResultsFasting plasma TAG concentrations significantly decreased in the omega-3 group and remained unchanged in the placebo group. Eight weeks of omega-3 supplementation significantly decreased IHTAG, fasting and postprandial hepatic DNL while significantly increasing dietary FA oxidation and fasting and postprandial plasma glucose concentrations. In vitro studies supported the in vivo findings of omega-3 FAs (EPA+DHA) decreasing intracellular TAG through a shift in cellular metabolism away from FA esterification toward oxidation.ConclusionsOmega-3 supplementation had a potent effect on decreasing hepatic DNL and increasing FA oxidation and plasma glucose concentrations. Attenuation of hepatic DNL may be considered advantageous; however, consideration is required as to what the potential excess of nonlipid substrates (eg, glucose) will have on intrahepatic and extrahepatic metabolic pathways.Trial registration numberNCT01936779.

  • Formation of antihydrogen beams from positron-antiproton interactions

    • The formation of a beam of antihydrogen atoms when antiprotons pass through cold, dense positron plasmas is simulated for various plasma properties and antiproton injection energies. There are marked dependences of the fraction of injected antiprotons which are emitted as antihydrogen in a beam-like configuration upon the temperature of the positrons, and upon the antiproton kinetic energy. Yields as high as 13% are found at the lowest positron temperatures simulated here (5K) and at antiproton kinetic energies below about 0.1 eV. By 1 eV the best yields are as low as 10(-3), falling by about two orders of magnitude with an increase of the positron temperature to 50 K. Example distributions for the antihydrogen angular emission, binding energy and kinetic energy are presented and discussed. Comparison is made with experimental information, where possible.

Special attention is devoted to the role of metastable states, arising from the near conservation of the energy stored in the cyclotron motion of the positrons. We find that the decay of such states changes the density scaling of the formation rate.

    • The formation of antihydrogen atoms from antiprotons injected into a positron plasma is simulated, focussing on the fraction that fulfil the conditions necessary for confinement of anti-atoms in a magnetic minimum trap. Trapping fractions of around 10(-4) are found under conditions similar to those used in recent experiments, and in reasonable accord with their results. We have studied the behaviour of the trapped fraction at various positron plasma densities and temperatures and found that collisional effects play a beneficial role via a redistribution of the antihydrogen magnetic moment, allowing enhancements of the yield of low-field seeking states that are amenable to trapping.

  • The role of antihydrogen formation in the radial transport of antiprotons in positron plasmas

    • Simulations have been performed of the radial transport of antiprotons in positron plasmas under ambient conditions typical of those used in antihydrogen formation experiments. The parameter range explored includes several positron densities and temperatures, as well as two different magnetic fields (1 and 3 T). Computations were also performed in which the antihydrogen formation process was artificially suppressed in order to isolate its role from other collisional sources of transport. The results show that, at the lowest positron plasma temperatures, repeated cycles of antihydrogen formation and destruction are the dominant source of radial (cross magnetic field) transport, and that the phenomenon is an example of anomalous diffusion.
