  • The Expiry of Humira® Market Exclusivity and the Entry of Adalimumab Biosimilars in Europe: An Overview of Pricing and National Policy Measures

    • Background: From October 2018, adalimumab biosimilars could enter the European market. However, in some countries, such as Netherlands, high discounts reported for the originator product may have influenced biosimilar entry.Objectives: The aim of this paper is to provide a European overview of (list) prices of originator adalimumab, before and after loss of exclusivity; to report changes in the reimbursement status of adalimumab products; and discuss relevant policy measures.Methods: Experts in European countries received a survey consisting of three parts: 1) general financing/co-payment of medicines, 2) reimbursement status and prices of originator adalimumab, and availability of biosimilars, and 3) policy measures related to the use of adalimumab.Results: In May 2019, adalimumab biosimilars were available in 24 of the 30 countries surveyed. Following introduction of adalimumab biosimilars, a number of countries have made changes in relation to the reimbursement status of adalimumab products. Originator adalimumab list prices varied between countries by a factor of 2.8 before and 4.1 after loss of exclusivity. Overall, list prices of originator adalimumab decreased after loss of exclusivity, although for 13 countries list prices were unchanged. When reported, discounts/rebates on originator adalimumab after loss of exclusivity ranged from 0% to approximately 26% (Romania), 60% (Poland), 80% (Denmark, Italy, Norway), and 80–90% (Netherlands), leading to actual prices per pen or syringe between €412 (Finland) and €50 – €99 (Netherlands). To leverage competition following entry of biosimilar adalimumab, only a few countries adopted measures specifically for adalimumab in addition to general policies regarding biosimilars. In some countries, a strategy was implemented even before loss of exclusivity (Denmark, Scotland), while others did not report specific measures.Conclusion: Even though originator adalimumab is the highest selling product in the world, few countries have implemented specific policies and practices for (biosimilar) adalimumab. Countries with biosimilars on the market seem to have competition lowering list or actual prices. Reported discounts varied widely between countries.

    • A new Certified Reference Material (CRM) for radionuclides in sediment (IAEA-385) is described and the results of the certification process are presented. Eleven radionuclides (K-40, Cs-137, Ra-226, Ra-228, Th-230, Th-232, U-234, U-238, Pu-238, Pu239+240 and Am-241) have been certified and information mass activities with 95% confidence intervals are given for seven other radionuclides (Sr-90, Pb-210(Po-210), U-235, Pu-239, Pu-240 and Pu-241). Results for less frequently reported radionuclides (Co-60, Tc-99, Cs-134, Eu-155, Ra-224 and Np-239) and information on some activity and mass ratios are also reported. The CRM can be used for quality assurance/quality control of the analysis of radionuclides in sediment samples, for the development and validation of analytical methods and for training purposes.   • Cognitive load in dialogue interpreting : Experience and directionality

    • This dissertation investigates the effect of experience and language direction on cognitive load in dialogue interpreting. The general objective of the study is to contribute to a better understanding of cognitive processes involved in dialogue interpreting. The present inquiry employs a multi- and mixed- method design and seeks to investigate disfluency measures as indicators of cognitive load in dialogue interpreting. Furthermore, the study aims to explore whether blink-based measures are sensitive to changes in cognitive load of dialogue interpreters. The present study is positioned within cognitive translation and interpreting studies (CTIS) and employs cognitive translatology as a framework, encompassing both cognitive and psycholinguistic approaches to translation and interpreting. Chen’s multidimensional theoretical construct of cognitive load in interpreting is explored in the study and remodeled to fit the context of dialogue interpreting and the assumptions of cognitive translatology. The data were collected from 17 dialogue interpreters during simulated interpreted encounters that recreated a situation commonly arising in a public service context in Sweden. The 10 inexperienced and 7 experienced interpreters all had Swedish as their working language, and the other working languages were French, Polish, and Spanish. Following the revised cognitive load model, the analyses of cognitive load focus on interpreter characteristics (interpreting experience) and on task and environmental characteristics (directionality). The results of analyses show that, in line with previous research, both interpreting experience and directionality modulate cognitive load of dialogue interpreters. Specifically, interpreting experience is demonstrated to attenuate cognitive load. In terms of directionality, interpreting into L2 is shown to be more cognitively demanding than interpreting into L1. Moreover, blink rate and blink rate variability (BRV) are explored as possible indicators of cognitive load. The analyses of blink measures suggest that no meaningful relationship can be found between blink measures and cognitive load.Finally, the complementary analyses of disfluency types in the utterances of the Polish interpreters (n=4) point to multifunctionality of disfluency in dialogue interpreting and to the multiple origins of cognitive load in interpreting dialogues. The analysis is performed from the perspective of the functional-cognitive view of disfluency proposed in the dissertation, whereby three disfluency context categories are identified and applied (cognitive-monitoring, cognitive-pragmatic, and cognitive-processing). Lexical access and rendition planning are identified as recurrent causes of cognitive load in dialogue interpreting. The study also makes theoretical and methodological contributions, primarily by revising the theoretical model of cognitive load in interpreting, which allows for operationalization of cognitive load with additional measures, in both experimental and naturalistic settings. Practical implications are a contribution to the understanding of the challenges interpreting into L2, and the impact of interpreters’ experience on interpreting. Overall, the study contributes to the emerging cognitive profile of dialogue interpreters.

    • We observe and explain theoretically a dramatic evolution of the Dzyaloshinskii-Moriya interaction (DMI) in the series of isostructural weak ferromagnets, MnCO3, FeBO3, CoCO3, and NiCO3. The sign of the interaction is encoded in the phase of the x-ray magnetic diffraction amplitude, observed through interference with resonant quadrupole scattering. We find very good quantitative agreement with first-principles electronic structure calculations, reproducing both sign and magnitude through the series, and propose a simplified "toy model" to explain the change in sign with 3d shell filling. The model gives insight into the evolution of the DMI in Mott and charge transfer insulators.

  • Exploring semi-structural differences in television news between the United States and Sweden

    • The purpose of this study is to explore differences in television news format and news framing between the US and Sweden. The study focuses on the news programs broadcast by the top three television networks in the US and the top two public service channels and the leading commercial television channel in Sweden: CBS Evening News, NBC Nightly News and ABC World News, and Rapport, Aktuellt and TV4 Nyheterna, respectively. Using the most different systems design, the content analysis reveals significant differences between the two countries across a number of important variables: amount of domestic vs foreign news coverage, level of journalistic visibility, length of politicians' sound bites, attribution of responsibility and thematic vs episodic framing of the news. The discussion addresses the possible causes for these crosscultural differences as well as the potential implications for news audiences in both countries.

  • Multiple pathways link urban green- and bluespace to mental health in young adults

    • Background: A growing body of scientific literature indicates that urban green- and bluespace support mental health; however, little research has attempted to address the complexities in likely interrelations among the pathways through which benefits plausibly are realized. Objectives: The present study examines how different plausible pathways between green/bluespace and mental health can work together. Both objective and perceived measures of green- and bluespace are used in these models. Methods: We sampled 720 students from the city of Plovdiv, Bulgaria. Residential greenspace was measured in terms of the Normalized Difference Vegetation Index (NDVI), tree cover density, percentage of green areas, and Euclidean distance to the nearest green space. Bluespace was measured in terms of its presence in the neighborhood and the Euclidean distance to the nearest bluespace. Mental health was measured with the 12-item form of the General Health Questionnaire (GHQ-12). The following mediators were considered: perceived neighborhood green/bluespace, restorative quality of the neighborhood, social cohesion, physical activity, noise and air pollution, and environmental annoyance. Structural equation modelling techniques were used to analyze the data. Results: Higher NDVI within a 300 m buffer around the residence was associated with better mental health through higher perceived greenspace; through higher perceived greenspace, leading to increased restorative quality, and subsequently to increased physical activity (i.e., serial mediation); through lower noise exposure, which in turn was associated with lower annoyance; and through higher perceived greenspace, which was associated with lower annoyance. Presence of bluespace within a 300 m buffer did not have a straightforward association with mental health owing to competitive indirect paths: one supporting mental health through higher perceived bluespace, restorative quality, and physical activity; and another engendering mental ill-health through higher noise exposure and annoyance. Conclusions: We found evidence that having more greenspace near the residence supported mental health through several indirect pathways with serial components. Conversely, bluespace was not clearly associated with mental health.

    • Background: Experiences afforded by natural settings promote health by helping people to build new adaptive capacities and to restore existing capacities. The aim of this study was to examine relations among restorative experience, mindfulness, rumination and psychological resilience in pathways linking residential greenspace to anxiety and depression symptoms.Methods: We sampled 529 university students residing in Plovdiv, Bulgaria. Residential greenspace was measured in terms of the Normalized Difference Vegetation Index (NDVI) and tree cover density for different buffer sizes. Symptoms of anxiety and depression were measured with the Generalized Anxiety Disorder scale (7-item) and Patient Health Questionnaire (9-item), respectively. The following mediators were assessed by self-report: perceived greenspace, restorative quality of the neighborhood, dispositional mindfulness, rumination, and psychological resilience. Structural equation modelling techniques were used to test the theoretically-indicated relations among the variables.Results: Across different buffer sizes, higher greenspace was consistently associated with reduced scores on the anxiety and depression scales. This effect was partially mediated via several pathways. Specifically, higher NDVI500-m was associated with higher perceived greenspace, and in turn, with higher restorative quality, and then with higher mindfulness, lower rumination, and greater resilience to stress, and consequently, with better mental health.Conclusions: Our findings affirm the potential of greenspace for building psychological resilience and promoting health by offsetting dysfunctional rumination and facilitating mindfulness as components of intertwined capacity-building and capacity-restoring pathways.
