  • Physical Activity Intensity Distribution during Physical Education among Swedish Upper Secondary School Students

    • Background: Physical education (PE) has been identified as a potentially powerful domain to promote physical activity (PA) among school-aged youth. In Sweden, there is a lack of studies investigating PA intensity distribution during PE among upper secondary school student. Aim: The aim of this study was to investigate PA intensity distribution during PE among Swedish upper secondary school students. Methods: PA was measured during 41 PE lessons among 121 (n = 75 boys) Swedish upper secondary school students (ages 16-18 years) using hip-mounted objective activity monitors from ActiGraph™ (GT3X+ accelerometers). Results: The participants spent a median of 23% of the PE lessons time being sedentary, 17% in light PA (LPA), and 59% in moderate-to-vigorous PA (MVPA). There were no differences between boys and girls for neither sedentary (p = .185), LPA (p = .377), or MVPA (p = .839). Sedentary (p = .001) and LPA (p < .0001) were lower and MVPA (p < .0001) higher during fitness-related activities when compared to invasion games. Conclusion: A median of 59% of the PEL time was spent in MVPA which is relatively high in an international comparison. No differences were observed between boys and girls. Fitness-related activities contributed to higher MVPA when compared to invasion games.

  • COVID-19 induced changes in physical activity patterns, screen time and sleep among Swedish adolescents - a cohort study.

    • BACKGROUND: The coronavirus disease 2019 (COVID-19) pandemic had a huge impact on daily life, even in countries such as Sweden where the restrictions were relatively mild. This paper assesses the effects of the COVID-19 pandemic restrictions on physical activity (PA) patterns, screen time, and sleep among Swedish adolescents. The exposures explored include gender, parental education, anthropometrics, and cardiovascular fitness (CVF).METHODS: Cohort data were collected from September 26th to December 6th, 2019, and from April 12th to June 9th, 2021. Participants were 13-14 years-old (7th graders) at baseline with 585 participating at both baseline and follow-up. At both baseline and follow-up PA and sedentary time were measured with accelerometers, and sleep and screen time with questionnaires. The exposure variables (gender, parental education, anthropometrics and CVF) were collected at baseline. Multilevel linear regression analyses were performed.RESULTS: Moderate-to-vigorous-physical activity (MVPA) remained unchanged while light physical activity (LiPA) decreased and sedentary time increased. Sleep duration decreased and screen time increased. Girls, adolescents with overweight/obesity (BMI and percent body fat), and those with lower CVF at baseline had less favourable changes in PA patterns, sleep and screen time.CONCLUSIONS: Although no significant (α = 0.05) changes were seen in MVPA, both LiPA and sedentary time as well as sleep and screen time changed in unfavourable ways. More intense activities are often organised and seem to have withstood the pandemic, while less intense activities decreased. Some groups were more vulnerable and will need directed intervention in the post-pandemic period as well as when future pandemics hit.

  • Has COVID-19 led to changes in physical activity patterns, screen time and sleep among Swedish adolescents? : A cohort study

    • Purpose: The COVID-19 pandemic has had an impact on daily life around the world though in Sweden the restrictions have been rather mild. The aim is to explore whether the pandemic has led to changes in physical activity (PA) patterns, including sedentary time, Light Physical Activity (LiPA) and Moderate-to-Vigorous-Physical Activity (MVPA) during weekdays and weekends, as well as screen time and sleep. The potential predictors explored include gender, parental education, anthropometrics, and cardiovascular fitness (CVF).Methods: Data were collected in the fall of 2019 and the spring of 2021. The participants were 13-14 years-old at baseline and lived in the Stockholm area. In total 585 participated at both baseline and follow-up. PA and sedentary time were measured with accelerometers and sleep and screen time with questionnaires at baseline and follow-up. The exposure variables were collected at baseline: gender and parental education via questionnaire, anthropometrics (BMI and body fat percentage measured with standard methods by researchers) and CVF with a submaximal test. Multilevel linear regression analyses were performed.Results: MVPA remained unchanged while LiPA decreased by 25.5 minutes on weekdays and 10.3 minutes on weekends (both p<0.001) and sedentary time increased by 9.4 minutes on weekdays (p=0.023). Sleep duration decreased by 27.4 minutes on weekdays and 19.1 minutes on weekends (both p<0.001) and screentime increased by around 45 minutes both on weekdays and weekends (p<0.001). Girls, adolescents with overweight/obesity (BMI and percent body fat), and those with lower CVF at baseline had less favourable changes in PA patterns, sleep and screen time.Conclusions: Previous self-reported data seems to suggest a decrease in physical activity due to the pandemic; this study only found such changes to be present in the lower intensity levels of physical activity but not in the MVPA. It is possible that more strenuous physical activity is more often part of organized sport which seems to have prevailed in Sweden despite the pandemic while habitual less intense activity decreased. Some groups were found to be more vulnerable and might need more support to maintain their physical activity levels, both now in the post-pandemic periods and during future pandemics.

    • Purpose:This study aimed to describe adolescents’ experiences of participating in a health-promoting school-based intervention regarding food and physical activity, with a focus on empowering aspects. Method:The school was located in a urban disadvantaged community in Sweden, characterized by poorer self-reported health and lower life expectancy than the municipality average. Focus group interviews with adolescents (29 girls, 20 boys, 14–15 years) and their teachers (n = 4) were conducted two years after intervention. Data were categorized using qualitative content analysis. Results: A theme was generated, intersecting with all the categories: Gaining control over one’s health: deciding, trying, and practicing together, in new ways, using reflective tools. The adolescents appreciated influencing the components of the intervention and collaborating with peers in active learning activities such as practicing sports and preparing meals. They also reported acquiring new health information, that trying new activities was inspiring, and the use of pedometers and photo-food diaries helped them reflect on their health behaviours. The adolescents’ experiences were also echoed by their teachers. Conclusions: To facilitate empowerment and stimulate learning, health-promotion interventions targeting adolescents could enable active learning activities in groups, by using visualizing tools to facilitate self-reflection, and allowing adolescents to influence intervention activities.

    • In this paper, we describe and critically reflect on the possibilities and challenges of developing and implementing an empowerment-based school intervention regarding healthy food and physical activity (PA), involving participants from a Swedish multicultural area characterized by low socioeconomic status. The 2-year intervention was continually developed and implemented, as a result of cooperation and shared decision making among researchers and the participants. All 54 participants were seventh graders, and the intervention comprised health coaching, health promotion sessions and a Facebook group. We experienced that participants valued collaborating with peers, and that they took responsibility in codeveloping and implementing the intervention. Participants expressed feeling listened to, being treated with respect and taken seriously. However, we also experienced a number of barriers that challenged our initial intentions of aiding participation and ambition to support empowerment. Moreover, it was challenging to use structured group health coaching and to work with goal-setting in groups of participants with shared, and sometimes competing, goals, wishes and needs related to food and PA. Successful experiences from this intervention was the importance of acquiring a broad and deep understanding of the context and participants, being open to negotiating, as well as adjusting the intervention. © The Author(s) 2019.
