    • Purpose: The number of persons who have successfully completed treatment for a cancer diagnosed during childhood and who have entered adulthood is increasing over time, and former patients will become aging citizens. Methods: Ten years ago, an expert panel met in Erice, Italy, to produce a set of principles concerning the cure and care of survivors of childhood and adolescent cancer. The result was the Erice Statement (Haupt et al. Eur J Cancer 43(12):1778–80, 2007) that was translated into nine languages. Ten years on, it was timely to review, and possibly revise, the Erice Statement in view of the changes in paediatric oncology and the number and results of international follow-up studies conducted during the intervening years. Results: The long-term goal of the cure and care of a child with cancer is that he/she becomes a resilient and autonomous adult with optimal health-related quality of life, accepted in society at the same level as his/her age peers. “Cure” refers to cure from the original cancer, regardless of any potential for, or presence of, remaining disabilities or side effects of treatment. The care of a child with cancer should include complete and honest information for parents and the child. Conclusions and implication for cancer survivors: Some members of the previous expert panel, as well as new invited experts, met again in Erice to review the Erice Statement, producing a revised version including update and integration of each of the ten points. In addition, a declaration has been prepared, by the Childhood Cancer International Survivors Network in Dublin on October 2016 (see Annex 1).

  • THE AFTERGLOWS OF SWIFT-ERA GAMMA-RAY BURSTS. I. COMPARING PRE-SWIFT AND SWIFT-ERA LONG/SOFT (TYPE II) GRB OPTICAL AFTERGLOWS

    • We have gathered optical photometry data from the literature on a large sample of Swift-era gamma-ray burst (GRB) afterglows including GRBs up to 2009 September, for a total of 76 GRBs, and present an additional three pre-Swift GRBs not included in an earlier sample. Furthermore, we publish 840 additional new photometry data points on a total of 42 GRB afterglows, including large data sets for GRBs 050319, 050408, 050802, 050820A, 050922C, 060418, 080413A, and 080810. We analyzed the light curves of all GRBs in the sample and derived spectral energy distributions for the sample with the best data quality, allowing us to estimate the host-galaxy extinction. We transformed the afterglow light curves into an extinction-corrected z = 1 system and compared their luminosities with a sample of pre-Swift afterglows. The results of a former study, which showed that GRB afterglows clustered and exhibited a bimodal distribution in luminosity space, are weakened by the larger sample. We found that the luminosity distribution of the two afterglow samples (Swift-era and pre-Swift) is very similar, and that a subsample for which we were not able to estimate the extinction, which is fainter than the main sample, can be explained by assuming a moderate amount of line-of-sight host extinction. We derived bolometric isotropic energies for all GRBs in our sample, and found only a tentative correlation between the prompt energy release and the optical afterglow luminosity at 1 day after the GRB in the z = 1 system. A comparative study of the optical luminosities of GRB afterglows with echelle spectra (which show a high number of foreground absorbing systems) and those without, reveals no indication that the former are statistically significantly more luminous. Furthermore, we propose the existence of an upper ceiling on afterglow luminosities and study the luminosity distribution at early times, which was not accessible before the advent of the Swift satellite. Most GRBs feature afterglows that are dominated by the forward shock from early times on. Finally, we present the first indications of a class of long GRBs, which form a bridge between the typical high-luminosity, high-redshift events and nearby low-luminosity events (which are also associated with spectroscopic supernovae) in terms of energetics and observed redshift distribution, indicating a continuous distribution overall.

  • Constraints on the Hubble constant from supernova Refsdal's reappearance

    • The gravitationally lensed supernova Refsdal appeared in multiple images produced through gravitational lensing by a massive foreground galaxy cluster. After the supernova appeared in 2014, lens models of the galaxy cluster predicted that an additional image of the supernova would appear in 2015, which was subsequently observed. We use the time delays between the images to perform a blinded measurement of the expansion rate of the Universe, quantified by the Hubble constant (H0). Using eight cluster lens models, we infer kilometers per second per megaparsec. Using the two models most consistent with the observations, we find kilometers per second per megaparsec. The observations are best reproduced by models that assign dark-matter halos to individual galaxies and the overall cluster.

  • It Takes Two Planets in Resonance to Tango around K2-146

    • K2-146 is a cool, 0.358M dwarf that was found to host a mini-Neptune with a 2.67 day period. The planet exhibited strong transit timing variations (TTVs) of greater than 30 minutes, indicative of the presence of an additional object in the system. Here we report the discovery of the previously undetected outer planet in the system, K2-146 c, using additional photometric data. K2-146 c was found to have a grazing transit geometry and a 3.97 day period. The outer planet was only significantly detected in the latter K2 campaigns presumably because of precession of its orbital plane. The TTVs of K2-146 b and c were measured using observations spanning a baseline of almost 1200 days. We found strong anti -correlation in the TTVs, suggesting the two planets are gravitationally interacting. Our TTV and transit model analyses revealed that K2-146 b has a radius of 2.25 0.10 Re and a mass of 5.6 0.7 Me, whereas K2-146 c has a radius of 2.591 Re and a mass of 7.1 0.9 Me. The inner and outer planets likely have moderate eccentricities of e = 0.14 0.07 and 0.16 0.07, respectively. Long-term numerical integrations of the two -planet orbital solution show that it can be dynamically stable for at least 2 Myr. We show that the resonance angles of the planet pair are librating, which may be an indication that K2-146 b and c are in a 3:2 mean motion resonance. The orbital architecture of the system points to a possible convergent migration origin.

    • Weather routing represents one increasingly recognized method for reducing fuel consumption, hereby lowering emissions and costs. This paper provides a short outline of the general concept of weather routing of ships, while focus is on the implementation of the DIRECT (DIviding RECTangles) algorithm in the construction of a novel weather routing tool made in cooperation with Maersk Maritime Technology. The developed routing tool is demonstrated on several conceptual cases to determine capabilities of route profiles avoiding obstacles such as land, avoiding adverse storms by varying voyage speed and utilizing weather to schedule a route with minimized fuel consumption. Strengths and weaknesses of the utilization of the DIRECT algorithm are discussed.

Strengths and weaknesses of the utilization of the DIRECT algorithm are discussed.

    • Background and Objectives. From 1994 to 1997 we conducted a population-based, prospective study on intensive therapy in newly diagnosed symptomatic myeloma patients younger than 60 years, comparing their survival to that of a conventionally treated historic population. Long-term results are presented, including the impact of the degree of response on survival and relapse pattern after transplantation. Design and Methods. The prospective population was formed of 397 patients and the historic population of 313 patients. Both populations were calculated to comprise more than 75% of the expected number of new cases. Results. After a median follow-up of 7 years survival was longer in the prospective population than in the historic one (median 60 versus 39 months; p=0.0002). When comparing only patients eligible for intensive therapy the median survival was 63 versus 44 months (p < 0.0001). Attaining a complete response was associated with prolonged event-free survival but not overall survival. The pattern of relapse after transplantation was heterogeneous but could be divided into four major groups; insidious, classical, plasmacytoma form and transformed disease. The median survival after relapse was 29 months. The relapse pattern and time to relapse predicted outcome. Patients relapsing with an insidious or classical form of disease with skeletal events only, or after a long lasting first response were likely to respond well to conventional salvage therapy. In contrast, relapse with multiple symptoms, transformed disease or a short duration of first response implied bad prognosis. Interpretation and conclusions. The relapse pattern after autologous transplantation is heterogeneous and response to salvage therapy is variable. The degree of response and event-free survival after transplantation are not reliable surrogate markers for survival.
