    • To compare the prevalence of cardiovascular disease (CVD) and major CVD risk factors among rheumatoid arthritis (RA) patients enrolled in a large US and multinational registry. We compared CVD and CVD risk factor prevalence from 11 countries enrolled in the CORRONA US and CORRONA International registries; patients from the 10 ex-US participating countries were grouped by region (Eastern Europe, Latin America, and India). Unadjusted summary data were presented for demographics and disease characteristics; comparisons for prevalence of CVD risk factors and CVD were age/gender standardized to the age/gender distribution of the US enrolled patients. Overall, 25,987 patients were included in this analysis. Compared to patients from the ex-US regions, US participants had longer disease duration and lower disease activity, yet were more likely to receive a biologic agent. Additionally, CORRONA US participants had the highest body mass index (BMI). Enrolled patients in India had the lowest BMI, were more rarely smokers, and had a low prevalence of hyperlipidemia, hypertension, and prior CVD compared to the US and other ex-US regions. Participants from Eastern Europe had a higher prevalence of hypertension and hyperlipidemia and highest prevalence of all manifestations of CVD. Differences in the prevalence of both CVD and major CVD risk factors were observed across the four regions investigated. Observed differences may be influenced by variations in both non-modifiable/modifiable characteristics of patient populations, and may contribute to heterogeneity on the observed safety of investigational and approved therapies in studies involving RA patients from different origins.

  • Investigating the Validity Evidence of the Swedish TriPM in High Security Prisoners Using the PCL-R and NEO-FFI

    • The triarchic model of psychopathy was developed to bridge opposing descriptions of psychopathy by separating the core construct in three domains; boldness, meanness, and disinhibition. The Triarchic Psychopathy Measure (TriPM) operationalizes the model through a 58-item self-report questionnaire. The current study examined the psychometric properties of the Swedish translation by investigating intercorrelations and associations to expert-rated psychopathy, general personality and psychopathy-related traits in male high-security prisoners (n = 191). Psychopathy rated with TriPM and the Psychopathy Checklist Revised (PCL-R) demonstrated expected convergence, as did empathy and impulsivity measures. The Disinhibition and Meanness scales were highly correlated, indicating that the scales might not be adequately differentiated. Nevertheless, the divergent association patterns to other important variables, particularly neuroticism and empathy, also points to meaningful differences. However, the lack of association between Disinhibition and Boldness may put into question if these domains are related at all, especially since there was a lack of similarity in the association patterns with other clinical variables. The influence of antisocial behavior in the TriPM operationalization might amplify the similarities of the Meanness and Disinhibition scales, while diluting the associations between Meanness and Boldness. In conclusion, the Swedish TriPM is effective in measuring the domains of triarchic model in forensic settings, even though a revision of the scales might improve the psychometric properties of the instrument.

    • Wide-field optical surveys have begun to uncover large samples of fast (t(rise) less than or similar to 5 d), luminous (M-peak < 18), blue transients. While commonly attributed to the breakout of a supernova shock into a dense wind, the great distances to the transients of this class found so far have hampered detailed investigation of their properties. We present photometry and spectroscopy from a comprehensive worldwide campaign to observe AT 2018cow (ATLAS 18qqn), the first fast-luminous optical transient to be found in real time at low redshift. Our first spectra (<2 days after discovery) are entirely featureless. A very broad absorption feature suggestive of near-relativistic velocities develops between 3 and 8 days, then disappears. Broad emission features of H and He develop after >10 days. The spectrum remains extremely hot throughout its evolution, and the photospheric radius contracts with time (receding below R < 10 (14) cm after 1 month). This behaviour does not match that of any known supernova, although a relativistic jet within a fallback supernova could explain some of the observed features. Alternatively, the transient could originate from the disruption of a star by an intermediate-mass black hole, although this would require long-lasting emission of highly super-Eddington thermal radiation. In either case, AT 2018cow suggests that the population of fast luminous transients represents a new class of astrophysical event. Intensive follow-up of this event in its late phases, and of any future events found at comparable distance, will be essential to better constrain their origins.

    • We conducted a two-stage genome-wide association study of renal cell carcinoma (RCC) in 3,772 affected individuals (cases) and 8,505 controls of European background from 11 studies and followed up 6 SNPs in 3 replication studies of 2,198 cases and 4,918 controls. Two loci on the regions of 2p21 and 11q13.3 were associated with RCC susceptibility below genome-wide significance. Two correlated variants (r² = 0.99 in controls), rs11894252 (P = 1.8 × 10⁻⁸) and rs7579899 (P = 2.3 × 10⁻⁹), map to EPAS1 on 2p21, which encodes hypoxia-inducible-factor-2 alpha, a transcription factor previously implicated in RCC. The second locus, rs7105934, at 11q13.3, contains no characterized genes (P = 7.8 × 10⁻¹⁴). In addition, we observed a promising association on 12q24.31 for rs4765623, which maps to SCARB1, the scavenger receptor class B, member 1 gene (P = 2.6 × 10⁻⁸). Our study reports previously unidentified genomic regions associated with RCC risk that may lead to new etiological insights.

    • We report the novel use of organically modified silica (ORMOSIL) nanoparticles to encapsulate hydrophobic quantum dots (QDs), thus making them hydrophilic and yet maintaining stable optical properties. QDs are resistant to photobleaching, and the cationic charge on the surfaces of the ORMOSIL nanoparticles can facilitate them being uptaken at the cell substrate regions. An objective-lens-based total internal reflection fluorescence (TIRF) microscope is used to observe the staining and dynamics of HeLa cancer cells, which have been targeted by the synthesized QD-doped ORMOSIL nanoparticles. Since the same microscope objective is used for launching the incident light beam and the collection lens, the observation and manipulation of live samples under a TIRF microscope is therefore much easier than that in the case of using the conventional prism-based TIRF setup. The present nanoparticle-assisted approach has made TIRF imaging a much more powerful tool for real-time monitoring of intracellular biological metabolic activities.

  • Effect of short-term growth hormone and testosterone administration on body composition and glucose homeostasis in men receiving chronic glucocorticoid therapy.

    • OBJECTIVE: Long-term pharmacological glucocorticoid (GC) therapy leads to skeletal muscle atrophy and weakness. The objective of this study was to investigate whether short-term treatment with growth hormone (GH) and testosterone (T) can increase lean mass without major impairment of glucose homeostasis in patients on GC therapy. DESIGN, MATERIALS AND METHODS: This was a prospective, open-label, randomized, crossover study. Twelve men (age 74±6 years) on chronic GC treatment participated. The effects of two weeks treatment with GH, testosterone and the combination of both, on lean body mass (LBM), appendicular skeletal muscle mass (ASMM), extracellular water (ECW), body cell mass (BCM) and plasma glucose concentrations were investigated. RESULTS: LBM increased significantly after GH (Δ1.7±1.4 kg; P=0.007) and GH+T (Δ2.4±1.1 kg; P=0.003), but not T alone. ASMM increased after all three treatment periods; by 1.0±0.8 kg after GH (P=0.005), 1.7±0.4 kg after GH+T (P=0.002) and 0.8±1.0 kg after T (P=0.018). The increase in ASMM was larger with combined treatment than either GH or T alone (P<0.05). ECW increased significantly after GH+T by 1.5±2.6 L (P=0.038) but not after GH or T alone. BCM increased slightly after single and combined treatments but the changes were not significant. Fasting glucose increased significantly after GH (Δ0.4±0.4 mmol/L, P=0.006) while both fasting (Δ0.2±0.3 mmol/L, P=0.045) and post glucose-load (Δ1.8±2.3 mmol/L, P=0.023) plasma glucose concentrations increased after GH+T. CONCLUSIONS: GH and T induce favourable and additive body compositional changes in men on chronic, low-dose GC treatment. In the doses used, combination therapy increases fasting and postprandial glucose concentration.

  • Heat-related cardiorespiratory mortality : effect modification by air pollution across 482 cities from 24 countries

    • Background: Evidence on the potential interactive effects of heat and ambient air pollution on cause-specific mortality is inconclusive and limited to selected locations. Objectives: We investigated the effects of heat on cardiovascular and respiratory mortality and its modification by air pollution during summer months (six consecutive hottest months) in 482 locations across 24 countries.Methods: Location-specific daily death counts and exposure data (e.g., particulate matter with diameters ≤ 2.5 µm [PM2.5]) were obtained from 2000 to 2018. We used location-specific confounder-adjusted Quasi-Poisson regression with a tensor product between air temperature and the air pollutant. We extracted heat effects at low, medium, and high levels of pollutants, defined as the 5th, 50th, and 95th percentile of the location-specific pollutant concentrations. Country-specific and overall estimates were derived using a random-effects multilevel meta-analytical model.Results: Heat was associated with increased cardiorespiratory mortality. Moreover, the heat effects were modified by elevated levels of all air pollutants in most locations, with stronger effects for respiratory than cardiovascular mortality. For example, the percent increase in respiratory mortality per increase in the 2-day average summer temperature from the 75th to the 99th percentile was 7.7% (95% Confidence Interval [CI] 7.6–7.7), 11.3% (95%CI 11.2–11.3), and 14.3% (95% CI 14.1–14.5) at low, medium, and high levels of PM2.5, respectively. Similarly, cardiovascular mortality increased by 1.6 (95%CI 1.5–1.6), 5.1 (95%CI 5.1–5.2), and 8.7 (95%CI 8.7–8.8) at low, medium, and high levels of O3, respectively.Discussion: We observed considerable modification of the heat effects on cardiovascular and respiratory mortality by elevated levels of air pollutants. Therefore, mitigation measures following the new WHO Air Quality Guidelines are crucial to enhance better health and promote sustainable development.
