  • Eosinophils from hematopoietic stem cell recipients suppress allogeneic T cell proliferation.

    • Eosinophilia has been associated with less severe graft-versus-host disease (GVHD), but the underlying mechanism is unknown. We hypothesized that eosinophils diminish allogeneic T cell activation in patients with chronic GVHD. The capacity of eosinophils derived from healthy subjects and hematopoietic stem cell (HSC) transplant recipients, with or without chronic GVHD, to reduce allogeneic T cell proliferation was evaluated using a mixed leukocyte reaction. Eosinophil-mediated inhibition of proliferation was observed for the eosinophils of both healthy subjects and patients who underwent HSC transplantation. Eosinophils from patients with and without chronic GVHD were equally suppressive. Healthy eosinophils required cell-to-cell contact for their suppressive capacity, which was directed against CD4(+) T cells and CD8(+) T cells. Neither eosinophilic cationic protein, eosinophil-derived neurotoxin, indoleamine 2,3-dioxygenase, or increased numbers of regulatory T cells could account for the suppressive effect of healthy eosinophils. Real-time quantitative PCR analysis revealed significantly increased mRNA levels of the immunoregulatory protein galectin-10 in the eosinophils of both chronic GVHD patients and patients without GVHD, as compared with those from healthy subjects. The upregulation of galectin-10 expression in eosinophils from patients suggests a stimulatory effect of HSC transplantation in itself on eosinophilic galectin-10 expression, regardless of chronic GVHD status. To conclude, eosinophils from HSC transplant recipients and healthy subjects have a T cell suppressive capacity.

    • Background The aim of this study was to investigate relations between psychosocial factors, signs and symptoms of orofacial pain and jaw dysfunction in patients with juvenile idiopathic arthritis (JIA). Methods Forty-five patients with JIA (median age 12 years) and 16 healthy matched controls (median age 13 years) were examined according to the diagnostic criteria for temporomandibular disorders (DC/TMD). The subjects answered the DC/TMD questionnaires regarding psychosocial factors (pain intensity, pain-related disability, depression, stress, catastrophizing, pain locations and jaw function). Results JIA patients with orofacial pain had higher degree of stress, depression, catastrophizing and jaw dysfunction compared to subjects without. In turn, these factors were associated with orofacial pain intensity. Also, patients with orofacial pain had higher systemic inflammatory activity. Conclusions Orofacial pain in patients with JIA is associated with stress, psychological distress, jaw dysfunction and loss of daily living activities. Pain intensity seems to be the major pain aspect related to these factors. In addition, systemic inflammatory activity appears to be an important factor contributing to orofacial pain in JIA.

    • Departing from the theory of street-level bureaucracy, this article usesa qualitative approach to examine employment officers’ perceptions of workingin the Swedish introduction programme for newly arrived migrants. Theprogramme is managed by the Swedish Public Employment Service, and theaim of the study is to illuminate how street-level bureaucrats perceive theprerequisites of implementing integration policy within the introductionprogramme, and how they respond to these prerequisites. The study showsthat the interviewees perceive the working conditions as difficult, characterizedby a pronounced tension between organizational demands andmigrants’ needs. To manage this dilemma, the street-level bureaucratsapply several coping strategies, and we highlight two broad patterns ofpractice. Within the client-centred pattern, attempts were made to use discretionto assist the participants in accordance with their needs. Within theauthority-centred pattern, the street-level bureaucrats applied a formal andrule-oriented understanding of their assignment, concentrating their effortson maintaining the functionality of the introduction programme. The mostimportant implication of the study is that it reveals a mismatch between thepolitically formulated integration policy and the actual needs of the migrants,as perceived by the interviewees. The current integration policy is heavilyinfluenced by a workfare logic, causing the introduction programme to befocussed on providing support connected to labour-market matching.However the programme lacks adequate structures to support its participantsto handle e.g. practical, health-related and psychosocial issues that wouldindirectly facilitate labour-market participation. Thus, this study encouragespoliticians and policymakers to formulate a more holistic integration policy.

    • Recent years Lean Production (”Lean”) has become a prevalent rationalization strategy in healthcare to create more efficient value streams. Research shows that this often results in impaired Ergonomics, i.e. impaired work content and physical work load (“physical ergonomics”). A common Lean tool used to rationalize value streams in healthcare is Value Stream Mapping (VSM). An ergonomic “add-in module” Ergonomic Value Stream Mapping (ErgoVSM) – has therefore been developed to be used as an integrated part of VSM in the analysis of value streams in healthcare. ErgoVSM as well as VSM are participative tools. All occupational groups performing tasks directly related to the investigated value stream should be represented when using the tool. This Guide includes description of methods and assessment templates for physical ergonomics and work content. An initial version of ErgoVSM for manufacturing industry was developed in Sweden. Later, this was further developed for the health-care sector. This tool was then evaluated in a Nordic Multicenter Study by national projects in Sweden, Denmark and Iceland. The Nordic evaluation showed that using the ErgoVSM tool may result in more focus on ergonomics in the Action Plans compared to just using VSM. This is obtained without jeopardizing performance aspects. Based on the field experiences from the Nordic Multicentre Study I, the ErgoVSM tool was further developed, resulting in this present English version.

    • En väl etablerad metod inom Lean Production är värdeflödesanalysen (VFA) som är ett proaktivt processverktyg baserat på delaktighet. Verktyget visualiserar flöden och slöserier. Nuläget visualiseras, problem identifieras och genom förslag till förbättringar skisseras ett framtida bättre och mer effektivt läge, där tid i icke-värdeskapande arbete minimeras i omfattning. Våra tidigare studier tyder på att värdeskapande arbete innebär mer riskfyllda exponeringar än ickevärdeskapande arbete. Mot denna bakgrund har vi påbörjat utveckling av en evidensbaserad belastningsergonomisk tilläggsmodul ERGONOVA (Ergonomisk Värdeflödesanalys). Nästa steg är att även utveckla denna tilläggsmodul för användning inom vård- och omsorgssektorn där lean-principerna håller på att introduceras. Verktyget ska utvecklas till ett fungerande förändringsverktyg innehållande såväl belastningsergonomiska som psykosociala dimensioner i syfte att underlätta en balansering mellan värdeflödesbaserade effektiviseringar och arbetsmiljöinsatser.

    • Over the last two decades, a movement for Evidence-Based Management (EBM) has surfaced across the Atlantic world with pretensions of being a successor of New Public Management (NPM). In this paper, we focus on Swedish social welfare as an arena where persistent government attempts have been made to implement locally new evidence-based ideas, specifically evidence-based practice (EBP). In Swedish discourse, the meaning of “evidence-based” is contested. One interpretation maintains that best (and only acceptable) evidence comes from the use of randomized controlled trials (RCTs). Another interpretation maintains that evidence from research constitutes only one leg of a multi-factorial definition; that is, this view contends that RCT evidence should be considered along with experience of practitioners and clients (users). Although client participation was an important tenet in the incipient attempts to implement EBP, by using translation theory this article will show that later attempts have tended to ignore the client’s perspective. From this foundation, we address why client views and outlooks have been ignored in EBP implementation.

    • On the sixth semester at the program for Social Work at Växjö University, the students choose one of two courses in which one seeks to deepen knowledge in social work with children, youth and family. It is our experience from this course that are in the focus of this report.The purpose of this report is to describe and reflect on how, through an educational model applied in professional education, you can combine research, theory, training and personal development. The reflection mirrors both teachers and students perspective in relation to the development of knowledge and professional social work and the report should be seen as a form of self-evaluation on the objectives of the course.Work on the report have been implemented by the course responsible teachers and the students who took the course during the spring semester 2006th The idea to write a joint report arose from discussions held at one of the seminars on the course. Format of the report is based on discussions gradually developed during the work and is based on a description of the course three sections of which are described in the text. These should be considered as a whole to the parts that include training in methods and calls that are in focus. All students from the course contributed with their papers, but it is the main author (Kerstin Johansson and Eva Johnson) who are responsible for the final product. Former students Margita Björkenklint, Sofia Eneroth, Carolina Karlsson and Tina Svanbring have all been a great help.Our aim has been to ultimately contribute to the discussion on the development of social work and the importance of skills development and the link between science and practice. Work on these issues must take place at several levels and both within academia and in practice. An important part of the work is the common, between scientists and the so-called practitioner or business representatives responsible to link science and practical social work for the benefit of those who are users of Social Work.
