    • Information from elemental and mass composition of size-fractionated airborne particle matter (PM) provides insightfulknowledge about their impact on human health, meteorology and climate. To attain insight into the nature of sizefractionatedPM from a typical African city, samples were collected from an urban background site in Nairobi, Kenya,during the months of August and September in 2007. PM samples ranging in size from 0.06 to 16 µm aerodynamicdiameter were collected on pre-weighed polycarbonate filters with 0.4 µm pore size using a nine-stage cascade impactor.Particles less than 0.06 µm were collected on a backup filter. A total of 170 samples were collected and analysed for traceelements using the Proton Induced X-Ray Emission (PIXE) technique. The analysis showed that Si, Fe and S dominated inall size ranges and displayed unimodal mass-size distribution whereas K, Cu, Zn and Pb, depicted bimodal mass-sizedistribution highlighting the multiplicity of their sources. To estimate human exposure to PM, deposition fractions of boththe coarse and fine PM in the human respiratory system were calculated. The deposited concentration was found to behighest in the head airways region compared to the tracheobronchial and pulmonary regions.

  • Ten years of marine evolutionary biology - challenges and achievements of a multidisciplinary research initiative

    • The Centre for Marine Evolutionary Biology (CeMEB) at the University of Gothenburg, Sweden, was established in 2008 through a 10-year research grant of 8.7 m€ to a team of senior researchers. Today, CeMEB members have contributed >500 scientific publications, 30 PhD theses and have organised 75 meetings and courses, including 18 three-day meetings and four conferences. What are the footprints of CeMEB, and how will the centre continue to play a national and international role as an important node of marine evolutionary research? In this perspective arcticle we first look back over the 10 years of CeMEB activities and briefly survey some of the many achievements of CeMEB. We furthermore compare the initial goals, as formulated in the grant application, with what has been achieved, and discuss challenges and milestones along the way. Finally, we bring forward some general lessons that can be learnt from a research funding of this type, and we take also look ahead, discussing how CeMEB’s achievements and lessons can be used as a springboard to the future of marine evolutionary biology.

    • OBJECTIVE: Cerebral white-matter changes (WMCs) are frequently found in dementia and have been proposed to be related to vascular factors and a certain symptomatological profile. However, few studies have included both vascular factors and a broad spectrum of cognitive, neurological and psychiatric symptoms, easily detectable by the physician in the everyday clinical work. The objective was to study the relationships between WMCs on MRI/CT and neuropsychiatric symptoms and vascular factors in patients with cognitive impairment. METHODS: One hundred and seventy-six patients with Alzheimer's disease, vascular dementia, mixed dementia, and mild cognitive impairment were included. All patients underwent a standardized examination including medical history, clinical examinations, laboratory tests and brain imaging (CT or MRI). The identification and severity degree of WMCs was assessed blindly to clinical findings, using a semi-quantitative scale. For statistical analyses, patients were grouped based on absence or presence of WMCs. Significant variables in bivariate analyses were included as predictors in stepwise multiple logistic regression analyses. RESULTS: Bivariate analyses showed significant associations between WMCs and age, gender, blood pressure, hypertension, ischaemic heart disease and TIA/RIND. Furthermore, there were significant associations between WMCs and apathy, mental slowness, disinhibition, gait disturbance and focal neurologic symptoms. The multivariate logistic model revealed apathy, mental slowness and age as the most consistent predicting factors for WMCs, together with MRI as a radiological method for the detection of WMCs. CONCLUSIONS: The findings indicate that WMCs in patients with dementia are associated with a dysexecutive-related behavioural symptom profile, vascular factors related to small and large vessel diseases and age.   • Cerebral white matter changes in patients with cognitive impairment - clinical and pathophysiological aspects

    • Cerebral white matter changes (WMC), detected with computed tomography (CT) or magnetic resonance imaging (MRI), represent a common condition in elderly people. However, the prognostic, symptomatological and biochemical constituents of WMC are only partially known. The aim of the present study was to evaluate WMC in relation to clinical manifestations in patients with mild cognitive impairment (MCI) and dementia and, by means of cerebrospinal fluid (CSF) analyses, to study different structural biomarkers possibly reflecting the pathophysiological process of WMC in non-disabled patients. In study I, significant associations were found between WMC and age, sex, hypertension, ischemic heart disease and TIA/minor stroke. Furthermore there were significant associations between WMC and apathy, mental slowness, disinhibition, gait disturbance and focal neurological symptoms, but not with depressed mood. In study II, CSF was analyzed for biomarkers known to be related to Alzheimer’s disease [AD; the 1-40 and 1-42 fragments of amyloid-β, α- and β-cleaved soluble amyloid precursor proteins (sAPPα, sAPPβ), total tau (T-tau), hyperphosphorylated tau (P-tau)] and vascular dementia [VaD; neurofilament protein light subunit (NFL), sulfatide, and CSF/S-albumin ratio]. NFL and sulfatide but not the AD biomarkers were related to WMC. In study III, low CSF levels of the myelin lipid sulfatide but not biomarker deviations associated with axonal degeneration (NFL), or AD were found to be related to progressive WMC. In study IV it was found that WMC were associated with ventricular atrophy which in turn was associated with neuropsychological dysfunction. Furthermore, tissue inhibitor of metalloproteinase 1 (TIMP-1), NFL and sAPPβ were related to both ventricular atrophy as well as WMC. Matrix metalloproteinase 9 (MMP-9) was the only marker representing WMC progression. The results indicate that WMC in patients with cognitive impairment are independently related to a dysexecutive-related behavioural symptom profile, vascular disorders and a non-AD biochemical profile associated with vessel-wall pathology and demyelination. The findings have may have implications for definition and nosological knowledge of AD and vascular cognitive disorder.

  • Cerebrospinal fluid biomarkers of white matter lesions - cross-sectional results from the LADIS study.

    • Background and purpose: White matter lesions (WMLs) caused by small vessel disease are common in elderly people and contribute to cognitive impairment. There are no established biochemical markers for WMLs. We aimed to study the relation between degree of WMLs rated on magnetic resonance imaging of the brain and cerebrospinal fluid (CSF) levels of structural biomarkers associated with Alzheimer's disease (AD) and subcortical vascular dementia. Methods: Fifty-three non-demented elderly individuals with WMLs were subjected to lumbar puncture. Degree of WMLs was rated using the Fazekas scale. Volumetric assessment of WMLs was performed. CSF samples were analyzed for the 40 and 42 amino acid fragments of amyloid beta, alpha- and beta-cleaved soluble amyloid precursor protein, total tau (T-tau), hyperphosphorylated tau (P-tau(181)), neurofilament light protein (NFL), sulfatide and CSF/Serum-albumin ratio. Results: Fifteen subjects had mild, 23 had moderate and 15 had severe degree of WMLs. CSF-NFL levels differed between the groups (P < 0.001) and correlated with the volume of WMLs (r = 0.477, P < 0.001). CSF sulfatide concentration displayed similar changes but less strongly. T-tau, P-tau(181) and the different amyloid markers as well as CSF/S-albumin ratio did not differ significantly between the groups. Conclusions: The association of increased CSF-NFL levels with increasing severity of WMLs in non-demented subjects suggests that NFL is a marker for axonal damage in response to small vessel disease in the brain. This manifestation may be distinct from or earlier than the neurodegenerative process seen in AD, as reflected by the lack of association between WMLs and AD biomarkers.

  • Low Cerebrospinal Fluid Sulfatide Predicts Progression of White Matter Lesions - The LADIS Study

    • Background/Aims: Demyelination and axonal degeneration are the hallmarks of established white matter lesions (WML). The neurochemistry of ongoing WML is only partially known. We explored cerebrospinal fluid (CSF) substances as markers of brain tissue damage in relation to progression of WML rated on magnetic resonance imaging. Methods: CSF from elderly individuals with WML was analyzed for amyloid markers, total tau, hyperphosphorylated t, neurofilament protein light subunit, sulfatide and CSF/serum-albumin ratio. After 3 years, a follow-up magnetic resonance imaging was performed. Progression of WML was rated using the Rotterdam Progression Scale (RPS). Results: 37 subjects (age 73.6 +/- 4.6 years) were included. Subjects with more pronounced progression (RPS > 2; n = 15) had lower mean sulfatide concentration at baseline as compared to subjects with no or minimal progression (RPS 0-2; n = 22) according to univariate analyses (p = 0.009). Sulfatide was the only biomarker that predicted the RPS score according to regression analysis, explaining 18.9% of the total variance (r = 0.38, p = 0.015). Conclusion: The correlation of CSF sulfatide levels and RPS scores may reflect a remyelination response to the demyelination process associated with WML. Furthermore, the results strengthen the notion that WML pathology is different from that of Alzheimer's disease.

    • Subcortical small-vessel disease (SSVD) is the most common vascular cognitive disorder. However, because no disease-specific cerebrospinal fluid (CSF) biomarkers are available for SSVD, our aim was to identify such markers.We included 170 healthy controls and patients from the Gothenburg Mild Cognitive Impairment (MCI) study clinically diagnosed with SSVD dementia, Alzheimer's disease (AD), or mixed AD/SSVD. We quantified CSF levels of amyloid-β (Aβ)x-38, Aβx-40, Aβx-42, as well as soluble amyloid precursor protein (sAPP)-α and sAPP-β.sAPP-β was lower in SSVD patients than in AD patients and controls. Receiver-operating characteristic (ROC) analyses showed that sAPP-β moderately separated SSVD from AD and controls. Moreover, the CSF/serum albumin ratio was elevated exclusively in SSVD and could moderately separate SSVD from the other groups in ROC analyses.SSVD has a biomarker profile that differs from that of AD and controls, and to some extent also from mixed AD/SSVD, suggesting that signs of blood-brain barrier (BBB) dysfunction and sAPP-β could be additional tools to diagnose SSVD.Patients with subcortical small-vessel disease (SSVD) exhibited reduced levels of sAPP-β and disturbances of the blood-brain barrier (BBB).This biochemical pattern is different from that of Alzheimer's disease (AD) and to some degree from that of mixed AD/SSVD.Our findings are speaking in favor of the concept that SSVD is a distinct vascular cognitive disorder (VCD) form.

    • The prevalence of exfoliation syndrome was studied in 39 patients suffering from dementia and cognitive impairment; a positive finding of exfoliation was detected in 11/39 of these patients. A comparison with an age-matched population survey showed that the prevalence of ocular exfoliation and the relative risk were significantly elevated. These results suggested that lesions related to the exfoliative process might be located also in the brain of patients suffering from dementia and cognitive impairment.
