  • Maktspel och arbetsmoral bland sjukskrivna och andra aktörer - Sjukförsäkringssystemet och moral hazard

    • Bakgrund: Aktiveringspolicy och teorier om moral hazard har influerat utformningen av sjukförsäkringssystem i flera delar av västvärlden. Detta har lett till att sjukpenning blivit allt svårare att erhålla, mer kopplat till arbete och aktivitet, samt insatser för att genomskåda fusk bland sjukskrivna.Syftet med studien var att utforska hur makt och arbetsmoral uttrycks av sjukskrivna och andra aktörer i sjukförsäkringssystemet.Metod: Detta var en kvalitativ longitudinell studie baserad på intervjuer och akter från 31 sjukskrivna som varit på en försäkringsmedicinsk utredning. Data analyserades genom tematisk analys.Resultat: Resultatet visar hur olika aktörer misstänkliggör varandras agendor och kompetens, samt hur olika maktmedel används för att rättfärdiga deras slutsatser. Det framkom även vissa allianser mellan aktörer där dessa präglas av samsyn och samarbete mot gemensamma mål i sjukskrivningsprocessen. Primärt ses dessa allianser mellan sjukskriven och sjukskrivande läkare, men även i enstaka fall mellan handläggare och sjukskrivande läkare i de fall läkaren förespråkar aktivering. De sjukskrivna personerna uttryckte en stark vilja och försök mot arbete eller aktivitet, och deras arbetsmoral verifierades ofta av andra aktörer.  Slutsats: Maktspel förekommer på olika sätt inom sjukförsäkringssystemet och de medel som användes i den här studien gavs olika betydelse beroende på vilken aktör som använde dem (exempelvis tolkningen av en försäkringsmedicinsk utredning). Den här studien kan ifrågasätta vissa antaganden om moral hazard och simulering bland sjukskrivna då personerna ofta presterade över sin förmåga under en försäkringsmedicinsk utredning i förhoppningen om att i någon mån visa sig förmögen att bidra till samhället, vilket även verifierades av andra aktörer.

  • Social Insurance Literacy Among the Sick-listed-A Study of Clients' Comprehension and Self-Rated System Comprehensibility of the Sickness Insurance System

    • IntroductionSickness insurance systems and their processes have been studied in terms of transparency, comprehensibility and fairness, highlighting the importance of just procedures that make sense to clients. Related research demonstrates differences between groups of clients, pointing towards a social gradient. The concept of social insurance literacy and the Social Insurance Literacy Questionnaire (SILQ) was recently developed and serves as a measure for client's ability to obtain, understand and act on information in a sickness insurance system, relating to the comprehensibility of the information that the system provides.ObjectiveThe purpose of this study was to investigate social insurance literacy among clients on sick leave and its associations with perceived justice, being granted sickness benefits and background factors.MethodsThis was a questionnaire study with clients on sick leave in Sweden. In the selection process 3993 clients were invited, of which 1173 recently had their sickness benefits withdrawn. Those who answered the SILQ (n = 1152) also answered a perceived justice measure and accepted sharing register data from the Swedish Social Insurance Agency. Data were analyzed through regression analysis.ResultsThe findings demonstrate that clients' perceptions of system comprehensibility and the status of their sick leave case was significantly associated with perceived justice, and being granted sickness benefits, while their individual abilities to obtain, understand, and act on information had lesser influence.ConclusionsThe system's ability to provide understandable information seems more important than clients' abilities to comprehend it. From a client perspective, a just system seems to be related to their experiences of the sick leave process (i,e., whether they had an ongoing or closed case) rather than their skills to obtain the correct information.

  • Social validitet för försäkringsmedicinska utredningar och påföljande myndighetsbeslut inom sjukförsäkringen

    • Bakgrund: I det svenska sjukförsäkringssystemet utförs försäkringsmedicinska utredningar av särskilda enheter inom hälso- och sjukvården på uppdrag av Försäkringskassan. Dessa utredningar ligger sedan till grund för handläggares beslut om sjukskrivnas rätt till sjukpenning. Studier av social validitet är ännu ovanliga inom försäkringsmedicin och folkhälsa men kan bidra med kunskap om vad som främjar eller hindrar att metoder uppfattas som godtagbara, begripliga och betydelsefulla.Syfte: Att utforska sjukskrivna personers upplevelse av social validitet för försäkringsmedicinska utredningar och påföljande myndighetsbeslut avseende sjukpenning inom sjukförsäkringen.Metod: Detta var en kvalitativ longitudinell studie baserad på telefonintervjuer med 30 sjukskrivna som varit på en försäkringsmedicinsk utredning, samt delvis deras akter från Försäkringskassan. En deduktiv innehållsanalys användes.Resultat: Sjukskrivnas förståelse för utredningen var beroende av huruvida de specifika testerna var tydligt relaterade till personernas svårigheter eller ej och vilken information de hade fått. De anser att den standardiserade strukturen inte är relevant för alla och att bristen på individanpassning förvårar möjligheten att få en rättvis bild av deras arbetsförmåga. Diskussion: Social validitet utforskades för försäkringsmedicinska utredningar och myndighetsbeslut i termer av godtagbarhet, begriplighet och betydelse. Utredningarna indikerar en låg social validitet i termer av godtagbarhet på grund av bristen på individanpassning, även om vissa delar är mer socialt valida än andra. Vidare varierade utredningarnas begriplighet beroende på deras applicerbarhet och den erhållna informationen, medan dimensionen betydelse indikerar en högre grad av social validitet. Handläggarens påföljande myndighetsbeslut ansågs däremot orelaterat till utredningens resultat, sakna konkreta argument och ibland motsägande andra aktörers rekommendationer, vilket indikerar en låg social validitet i termer av både godtagbarhet, begriplighet och betydelse. Slutsats: Social validitet finns för delar av utredningarna men är låg för det påföljande myndighetsbeslutet.

  • Social validity of work ability evaluations and official decisions within the sickness insurance system : a client perspective

    • BACKGROUND: Studies of the social validity of work ability evaluations are rare, although the concept can provide valuable information about the acceptability, comprehensibility and importance of procedures. OBJECTIVE: The aim of this study was to explore clients’ perceptions of social validity of work ability evaluations and the following official decisions concerning sickness benefits within the Swedish sickness insurance system.MATERIALS AND METHODS: This was a longitudinal qualitative study based on interviews with 30 clients on sick leave, analyzed through deductive content analysis. RESULTS: Clients’ understanding of the evaluation was dependent on whether the specific tests were perceived as clearly related to the clients’ situation and what information they received. For a fair description of their work ability, clients state that the strict structure in the evaluation is not relevant to everyone. CONCLUSION: The work ability evaluations indicate low acceptability due to lack of individual adaptation, the comprehensibility varied depending on the applicability of the evaluation and information provided, while the dimension ‘importance’ indicated as higher degree of social validity. The official decision about sickness benefits however was considered unrelated to the evaluation results, lacking solid arguments and sometimes contradictory to other stakeholders’ recommendations indicating poor social validity.

  • Achieving net-zero carbon emissions in construction supply chains - A multidimensional analysis of residential building systems

    • The construction sector accounts for approximately 25% of global CO2 emissions. In this paper, we provide a multidimensional assessment of the potential for greenhouse gas emissions abatement in relation to the construction of multi-family residential buildings. Different building designs are compared, whereby the study analyzes the potential reductions in greenhouse gas emissions when combining abatement measures with a perspective of the technologies and practices available now, and those that are likely to become available on a timescale up to Year 2045. Further, the assessment analyzes the potential for emissions reductions when applying abatement measures at different points in the supply chain, from primary material production via material composition to the final building structure. The results indicate that the greenhouse gas emissions can be reduced by up to 40% with currently available technologies and practices, with even greater potential reductions of 80% to Year 2030 and 93% to Year 2045.

  • Agricultural factors affecting Fusarium communities in wheat kernels

    • Fusarium head blight (FHB) is a devastating disease of cereals caused by Fusarium fungi. The disease is of great economic importance especially owing to reduced grain quality due to contamination by a range of mycotoxins produced by Fusarium. Disease control and prediction is difficult because of the many Fusarium species associated with FHB. Different species may respond differently to control methods and can have both competitive and synergistic interactions. Therefore, it is important to understand how agricultural practices affect Fusarium at the community level.Lower levels of Fusariwn mycotoxin contamination of organically produced cereals compared with conventionally produced have been reported, but the causes of these differences are not well understood. The aim of our study was to investigate the effect of agricultural factors on Fusarium abundance and community composition in different cropping systems. Winter wheat kernels were collected from 18 organically and conventionally cultivated fields in Sweden, paired based on their geographical distance and the wheat cultivar grown. We characterised the Fusarium community in harvested wheat kernels using 454 sequencing of translation elongation factor 1-alpha amplicons. In addition, we quantified Fusariwn spp. using real-time PCR to reveal differences in biomass between fields.We identified 12 Fusariwn operational taxonomic units (OTUs) with a median of 4.5 OTUs per field. Fusarium graminearum was the most abundant species, while F. avenaceum had the highest occurrence. The abundance of Fusariwn spp. ranged two orders of magnitude between fields. Two pairs of Fusariurt species co-occurred between fields: F. poae with F. tricinctwn and F. culmorwn with F. sporofrichoides. We could not detect any difference in Fusariwn communities between the organic and conventional systems. However, agricultural intensity, measured as the number of pesticide applications and the amount of nitrogen fertiliser applied, had an impact on Fusariwn communities, specifically increasing the abundance of F. tricinctwn. There were geographical differences in the Fusarium community composition where F. graminearwn was more abundant in the western part of Sweden. The application of amplicon sequencing provided a comprehensive view of the Fusarium community in cereals. This gives us better opportunities to understand the ecology of Fusarium spp., which is important in order to limit FHB and mycotoxin contamination in cereals.

  • Decarbonization of construction supply chains - Achieving net-zero carbon emissions in the supply chains linked to the construction of buildings and transport infrastructure

    • Sweden has committed to reducing greenhouse gas (GHG) emissions to a net-zero level by Year 2045. In Sweden, about 20% of its annual CO2 emissions are from the manufacture, transport and processing of materials for both the construction and refurbishment of buildings and transport infrastructure. Cement and steel, together with diesel use in construction processes and material transport account for the majority of the CO2 emissions associated with building and infrastructure construction. This thesis assesses the challenges associated with reducing CO2 emissions from the supply chains for buildings and transport infrastructure construction. The main aim is to determine the extent to which abatement technologies across the supply chain can reduce the GHG emissions associated with construction if combined to exploit their full potential, while identifying key barriers towards their implementation. The work takes its starting point from material, energy and emissions flow analyses conducted across the construction supply chain, followed by the development of stylized models, which are subsequently used for scenario analysis. This quantitative analysis work is integrated with a participatory process that involves relevant stakeholders in the assessment process. The participatory process serves to identify the main abatement options, as well as to adjust decisions and assumptions regarding abatement portfolios and timelines, so as to make these as realistic and feasible as possible. Supported by a comprehensive literature review, a detailed inventory of abatement options in the supply chain of building and transport infrastructure construction is developed. This includes technologies and practices that are currently available and that are deemed available on a timescale up to Year 2045. The results show that on a national level, it is possible to reduce GHG emissions associated with the construction of buildings and transport infrastructure by 50% up to Year 2030, through applying already available measures. Moreover, it will be feasible to reach close-to-zero emissions by Year 2045, with this requiring comprehensive measures across-the-board, including breakthrough technologies for heavy vehicles, cement and steel production. Attaining the full abatement potential of measures that are already available would rely on sufficient availability of sustainably produced second-generation biofuels, requiring accelerated implementation of alternative abatement measures, involving optimization of material use, mass handling and transport systems, as well as the use of alternative materials and designs, with focus on circularity and material efficiency measures. To realize the potential linked to applying measures across the supply chain, there is a need for extensive collaboration along the whole value chain. Policy measures and procurement strategies should be aligned to support these measures with a clear supply chain focus, so as to enable balanced risk sharing and the involvement of contractors early in the planning and design process. The results also illustrate the importance of intensifying efforts to identify and manage both soft and hard barriers to implementation and the importance of acting promptly to implement available measures (e.g., material efficiency, recycling and material/fuel substitution measures) while actively planning for long-term measures (electrification of heavy vehicles and low-CO2 steel or cement). There are immediate and clear needs to prepare for deeper abatement and associated transformative shifts and to consider carefully the pathway towards these goals while avoiding pitfalls along the way, such as an over-reliance on biofuels or cost optimizations that cannot be scaled up to the levels required to reach deep emissions reductions. Therefore, strategic planning must be initiated as early as possible, as lead times related to planning, securing permits and construction of the support infrastructure (renewable electricity supply, electricity grid expansion, hydrogen storage, CCS infrastructure) and piloting and upscaling to commercial scale of the actual production units will all influence the speed of change.

  • Diversity of wheat phyllosphere fungi in different agricultural production systems : with special reference to Fusarium

    • Fusarium head blight (FHB) is a disease affecting cereals world-wide caused by Fusarium fungi. The disease is of great economic importance especially since it is associated with contamination by harmful mycotoxins produced by Fusarium spp. In recent years, several studies have reported lower mycotoxin contamination of organically produced cereals compared with those produced conventionally. An unexplored factor is differences in the microbial communities on cereal crops between the organic and conventional systems, which may exert a biocontrol effect on FHB. The aim of this thesis was to investigate the effect of different environmental and agricultural factors on Fusarium communities and fungal phyllosphere communities in wheat in different agricultural production systems. New primers for amplifying Fusarium communities, in conjunction with 454 se-quencing, were evaluated, and Fusarium communities associated with soil and wheat kernels were successfully characterised. Comparison of fungicide-treated and untreated wheat leaves from 18 fields in Sweden in 2011 revealed that fungicide use was associ-ated with decreased evenness of fungal communities. Furthermore, organic farming was associated with increased fungal species richness on wheat leaves when pairs of 22 organically and conventionally managed fields in Sweden were compared in 2012. There was no difference in the abundance of leaf pathogens between cropping systems. Several fungal species were consistently found regardless of geographical location, year and cropping system, while fungal abundance was highly variable between fields. Both Fusarium and fungal communities were affected by agricultural intensity. How-ever, there was no difference in Fusarium community composition in wheat kernels from organically and conventionally managed fields. The method evaluated in this thesis can be used to monitor Fusarium communities in the field, which is important in order to develop strategies for limiting FHB and mycotoxin contamination. The present thesis demonstrates that fungicide use and organic farming are associated with differen-tial fungal communities in the wheat phyllosphere. This indicates that there is an oppor-tunity for the farmer to manage these communities with the aim of enhancing their biocontrol potential against plant pathogens such as Fusarium.

    • Purpose This study aims to explore ethical challenges potentially arising from a problem-solving intervention with workplace involvement (PSI-WPI) in primary health care (with first-line manager involvement) for employees on sickness absence due to common mental disorders. Methods A qualitative design guided by the theoretical framework for systematic identification of ethical aspects of healthcare technologies. Semi-structured interviews were performed with coordinators (n = 6), employees (n = 13), and first-line managers (n = 8). Reflexive thematic analysis was used to analyse and interpret themes. Results A main theme was identified “the workplace and healthcare hold different organizational value logics” and four sub-themes: “the PSI-WPI challenged the organizational goals and values of the workplace and healthcare”, “the PSI-WPI challenged organizational values on fairness”, “the PSI-WPI challenged the professional roles of first-line managers and rehabilitation coordinators” and “the PSI-WPI introduced a need for the employee to juggle the employee and patient roles”. Conclusion Different organizational value logics, values, and goals can introduce ethical challenges. We advise clarifying stakeholders’ roles and preparing employees and managers for the return to work process by providing sufficient information. The ethical challenges and suggested measures to minimize them, should be considered when planning return to work interventions that involve several stakeholders.

    • BackgroundWork-directed interventions that include problem-solving can reduce the number of sickness absence days. The effect of combining a problem-solving intervention with involvement of the employer is currently being tested in primary care in Sweden for employees on sickness absence due to common mental disorders (PROSA trial). The current study is part of the PROSA trial and has a two-fold aim: 1) to explore the experiences of participating in a problem-solving intervention with workplace involvement aimed at reducing sickness absence in employees with common mental disorders, delivered in Swedish primary health care, and 2) to identify facilitators of and barriers to participate in the intervention. Both aims targeted rehabilitation coordinators, employees on sickness absence, and first-line managers.MethodsData were collected from semi-structured interviews with participants from the PROSA intervention group; rehabilitation coordinators (n = 8), employees (n = 13), and first-line managers (n = 8). Content analysis was used to analyse the data and the Consolidated Framework for Implementation Research was used to group the data according to four contextual domains. One theme describing the participation experiences was established for each domain. Facilitators and barriers for each domain and stakeholder group were identified.ResultsThe stakeholders experienced the intervention as supportive in identifying problems and solutions and enabling a dialogue between them. However, the intervention was considered demanding and good relationships between the stakeholders were needed. Facilitating factors were the manual and work sheets which the coordinators were provided with, and the manager being involved early in the return-to-work process. Barriers were the number of on-site meetings, disagreements and conflicts between employees and first-line managers, and symptom severity.ConclusionsSeeing the workplace as an integral part of the intervention by always conducting a three-part meeting enabled a dialogue that can be used to identify and address disagreements, to explain CMD symptoms, and how these can be handled at the workplace. We suggest allocating time towards developing good relationships, provide RCs with training in handling disagreements, and additional knowledge about factors in the employee's psychosocial work environment that can impair or promote health to increase the RCs ability to support the employee and manager.
