  • Preterm birth and risk of IgE sensitization up to early adulthood : a population-based birth cohort study

    • Background: Immunoglobulin E (IgE) sensitization is associated with asthma and allergic diseases. Gestational age influences early immune system development, thereby potentially affecting the process of tolerance induction to allergens. We aimed to study IgE sensitization to common allergens by gestational age from childhood up to early adulthood.  Method: Population-based birth cohort, data from the Swedish BAMSE study was used. Allergen-specific IgE to a mix of common food (fx5) and inhalant (Phadiatop) allergens were analysed at 4, 8, 16 and 24 years. Sensitization was defined as allergen-specific IgE ≥0.35 kU A /l to fx5 and/or Phadiatop at each time-point. Using logistic regression and generalized estimated equations, adjusted odds ratios (aORs) for sensitization in relation to gestational age were calculated. Replication was sought within the Swedish twin study STOPPA.Results: In BAMSE, 3523 participants were screened for IgE anti-bodies during follow-up; of these, 197 (5.6%) were born preterm (<37 gestational weeks), and 330 (9.4%) post-term (≥42 weeks). Preterm birth was inversely associated with sensitization to common food and/or inhalant allergens up to early adulthood; overall aOR = 0.71 (95% CI = 0.51- 0.98). An inverse association was also observed between preterm birth and sensitization to food allergens specifically; overall aOR = 0.59 (95% CI = 0.38- 0.95). No relation was found between post-term birth and IgE sensitization at any time-point. Replication analyses in STOPPA ( N  = 675) showed similar risk estimates for sensitization to food and/or inhalant allergens (aOR = 0.76; 95% CI: 0.45- 1.26), which resulted in a combined meta-analysis aOR = 0.72 (95% CI: 0.55- 0.95).  Conclusion: Our study suggests an inverse association between preterm birth and later IgE sensitization.

    • Integrating stakeholder knowledge, views and needs in marine or maritime spatial planning (MSP) processes is important from a governance and social sustainability perspective both for MSP practitioners and for the evolving field of MSP research. Transboundary MSP appears particularly challenging for participation, which is why it is important to identify opportunities and address obstacles for stakeholder integration in this specific context. This article examines how stakeholder integration is currently practiced in the Baltic Sea Region (BSR), an enclosed sea where policy coherence and addressing conflicting interests across borders are especially relevant. It synthesises a range of challenges and enablers for stakeholder participation and mobilisation that have emerged from two transboundary MSP research and development projects, BaltSpace and Baltic SCOPE. The article finds that with the exception of statutory authorities, stakeholder engagement in the BSR is mostly limited to self-motivated stakeholders and consultation rather than more inclusive forms of participation. This can reduce the quality and legitimacy of MSP processes and risks to concentrate power in the hands of a small group of actors. For transboundary stakeholder integration to become more interactive and effective, five types of challenges need attention, regarding a) timing, b) governance systems, c) capacity and processes, d) stakeholder characteristics and e) knowledge and language. These obstacles can be addressed by (1) a dedicated research and development agenda that critically reflects on integrative tools and processes, and (2) by encouraging transnational institutions in the BSR to devote more resources to transboundary stakeholder integration and adopt flexible and adaptive strategies and tools that can facilitate stakeholder involvement throughout the MSP policy cycle.

  • Replication of genomewide associations with allergic sensitization And allergic rhinitis

    • Background: Three genomewide metastudies have recently reported associations with self-reported allergic rhinitis and allergic sensitization. The three studies together identified a set of 37 loci but showed low concordance. This study investigates the reproducibility of the detected single nucleotide polymorphism (SNP) associations in an extensively characterized longitudinal cohort, BAMSE. Methods: Phenotypic evaluation of allergic rhinitis (AR) and allergic sensitization was performed on 2153 children from BAMSE at 8 and 16 years of age. Allele frequencies of 39 SNPs were investigated for association with the exact allergic phenotypes of the metastudies. Odds ratios and false discovery rates were calculated, and the impact of asthma was evaluated. The cases were also evaluated for age at onset effects ( <= or >8 years of age). Results: Association tests of the 39 SNPs identified 12 SNPs with P-values <0.05 and Q-values <0.10. Two of the four loci (TLR6-TLR1 and HLA-DQA1-HLA-DQB1) identified in all three original studies were also identified in this study. Three SNPs located in the TLR6-TLR1 locus had the lowest P-values and Q-values <0.1 when using a well-defined AR phenotype. Two loci showed significant age at onset effects, but the effect of asthma on the associations was very limited. Conclusion: The TLR6-TLR1 locus is likely to have a central role in the development of allergic disease. The association between genetic variation in the SSTR1-MIPOL1 and TSLP-SLC25A46 loci and age at onset is the first report of age at onset effects in allergic rhinitis.
