  • Overweight and obese children´s ability to report energy intake using digital camera food records during a 2-year study

    • The objective was to evaluate overweight and obese children's ability to report reproducible and valid estimates of energy intake (EI) by using digital camera food records (FR) during a 2-year study, compared with objectively measured total energy expenditure (TEE). Seventy-three overweight/obese children, aged 8–12 years at inclusion, kept FR with the help of digital cameras for 16 days in total, on 7 occasions during a 2-year period. On the same days, their TEE was registered with SenseWear Armband (SWA). The children underestimated their EI by −2.8 (2.4) MJ/d on the first assessment occasion (95% CI: −3.3, −2.3). Reporting accuracy did not differ between the 7 assessment occasions (P=0.15). Variables negatively associated with reporting accuracy relative to TEE were increased age (95% CI: −0.07, −0.01) and BMI z-score (95% CI: −0.18, −0.06). Further, reporting accuracy relative to TEE was lower for girls than boys (95% CI: −0.14, −0.01) and on weekdays compared with weekend days (95% CI: −0.08, −0.001). In conclusion, overweight and obese children were able to report their EI using a digital camera FR with good reproducibility over a 2-year period, even though their EI was underestimated compared with objectively measured TEE.

    • BACKGROUND: Sensory hyperreactivity (SHR) has been suggested as one explanation for chemically induced airway symptoms; it can be diagnosed with a capsaicin inhalation test. Previous capsaicin inhalation studies of SHR have used a Pari Boy device. This model of inhalator device has become outdated, hence it is necessary to abandon it in favour of a new device. The aim of this study was to transfer the capsaicin inhalation test using the Pari Boy device to a corresponding model using the Maxin MA3 device. METHODS: Twenty-one patients with SHR and 44 healthy controls visited the clinic twice and underwent a randomised protocol. The participants were provoked with saline and capsaicin using one of two devices, Pari Boy and Maxin MA3. Eight patients also underwent two additional capsaicin provocations with Maxin MA3. A new series of capsaicin concentrations was chosen for Maxin MA3. The results from each device were analysed, the agreement between the two devices and the repeatability of the Maxin MA3 were evaluated. RESULTS: Among all participants, the mean number of coughs with the Pari Boy was 5.5 (95% CI: 2.7; 8.2) after inhalation of 0.4 micromol/L capsaicin and 20.0 (95% CI: 14.1; 25.9) after 2.0 micromol/L. With the Maxin MA3, the mean number of coughs was 3.6 (95% CI: 1.3; 4.0) after 0.06 micromol/L capsaicin, and 17.8 (95% CI: 12.0; 23.6) after 0.3 micromol/L. The Maxin MA3 showed good repeatability and the agreement between the devices with the capsaicin concentrations chosen for each device was considered to be good.

    • Introduction: A validated food frequency questionnaire (FFQ) adapted to preschool ages is needed to evaluate a new program for primary prevention of childhood obesity.  Methods: So far, in this ongoing validation study of 200 3-year olds and 200 5 year-olds (selected at random), food intake has been reported for 58 parent-offspring pairs. The parents (mostly mothers) reported their own and their child’s food intake in FFQs. In addition the child’s food intake was recorded prospectively in a food diary during two periods of four days duration each. Data on parental education, BMI, type of childcare, etc. were also collected.  Results: The Spearman correlations (with 95 % CI) between FFQ and food-diary in children was 0.39 (0.15; 0.63) for number of meals per week containing vegetables, 0.70 (0.53; 0.87) for number of fruits per week and 0.66 (0.48; 0.82) for number of soft drinks per month. The Spearman correlation between intake according to the FFQ among parents and their children was 0.41 (0.15; 0.66) for number of meals/week containing vegetables, 0.22 (-0.03; 0.48) for number of fruits per week and 0.44 (0.22; 0.66) for number of soft drinks per month. Conclusion: For the young children our preliminary results show associations of modest size between the two methods for meals with vegetables and stronger associations for fruits and soft drinks. The association between parents’ and children’s intake according to FFQ was weak for fruit, moderate for vegetables and fairly strong for soft drink. Results based on the full dataset will be presented.

    • To decrease BMI in overweight and obese children, improved dietary intake and increased physical activity are key elements. Our objective was to evaluate the impact of a 1-y food and physical activity intervention on energy and macronutrient intake in overweight and obese children. A randomized open trial was conducted with 92 overweight or obese 10.4 ± 1.08-y-old children. The intervention included 14 group sessions with different themes regarding food and physical activity. Dietary intake was assessed with diet history interviews covering 14 d at baseline and 4-d food records after 1 y and was evaluated according to national dietary recommendations. The control group participated in the same measurements as the intervention group but did not take part in group sessions. After 1 y, both groups had decreased their energy intake (EI) relative to total energy expenditure, but the effect was more pronounced for the intervention group than for the control group. At 1 y follow-up, a larger proportion of children in the intervention group compared with the control group met the recommended intake of refined sugar (P = 0.019). However, the groups did not differ in the proportion children who met the recommended intake of dietary fiber. Further, SFA intake relative to total EI did not differ between the groups at 1 y follow-up. In conclusion, despite a rather comprehensive intervention, only modest effects were achieved with respect to reduced EI and improved macronutrient intake.

    • Background Childhood overweight and obesity have increased during the past decades and there is a need for effective intervention programs both for treatment and prevention to interrupt the increased trend. Overweight and obesity result from a combination of genetic predisposition and lifestyle where an imbalance in energy intake (EI) and total energy expenditure (TEE) is the key factor. The objective of this thesis is to evaluate the impact of a 2-year lifestyle intervention on food habits, anthropometry and metabolic markers on children with overweight and obesity. Methods Overweight and obese children 8-12 years old were recruited to participate in a 2-year randomized controlled trial (RCT). One hundred and five children agreed to participate and were randomized into one intervention group and one control group. Both groups participated in the same measurements while the intervention group also participated in a lifestyle program aiming at improving food habits and increasing physical activity. The first year of the program consisted of 14 group sessions and the second year of the intervention was web-based. Food habits were assessed at baseline and at endpoint by a diet history interview (DHI) and by a 4-day food record at 1-year measurement. At baseline 22 randomly chosen children were included in a validation study to validate reported EI against TEE measured by doubly labeled water (DLW) method and SenseWear Armband Pro 2 and 3 (version 5.1) (SWA). Anthropometric and biochemical parameters were measured at baseline, 1-year and endpoint. Results The DHI underestimated EI by 14% when validated against measured TEE by DLW and SWA. At the 1-year measurement the intervention group had a lower intake of fat (g and E%), monounsaturated fat (MUFA) (g) and polyunsaturated fat (PUFA) (g and E%) compared to the control group. At endpoint the intervention group had a lower intake of fat (g), MUFA (g) and cholesterol compared to the control group. Children in the intervention group consumed less sugar sweetened beverages at endpoint and had increased their intake of keyhole labeled foods compared to the control group. During the first year the growing children in both groups remained stable with respect to BMI and had decreased their BMI zscore. Conclusion The 2-year lifestyle intervention resulted in some improvements regarding food habits, but overall the effects on anthropometrics and metabolic markers were limited. This strongly supports that efforts should primarily be aimed at primary prevention of childhood overweight and obesity.
