  • The mental and physical health of older offenders : A systematic review and meta-analysis

    • A systematic review with meta-analysis was performed to: 1) estimate the prevalence of both mental and physical health problems in older offenders; 2) calculate relative risks for the health conditions in relation to non-offender older adults and; 3) explore the potential confounding role of several variables. We searched five databases up to August 2019. Studies involving offenders older than 50 years old were included. Fifty-five publications met criteria. The pooled prevalence for 18 mental and 28 physical health problems was calculated. In comparison with non-offender older adults, older offenders showed significantly higher risk for Hypertension (RR = 1.16, CI = 1.1, 1.2), Cardiovascular Diseases (RR = 1.24, CI = 1.09, 1.41), Respiratory diseases (RR = 1.75, CI = 1.29, 2.35), and Arthritis (RR = 1.19, CI = 1.12, 1.25). Heterogeneity was significant for all meta-analyses and partially explained by the confounding effect of country, the diagnosis assessment method, and the sample characteristics. Future research should include comparison groups of non-offender older adults and use longitudinal study designs to identify risk factors that can be targeted in preventive programmes.

  • The impact of criminal and externalizing behaviors on aging : Long-term associations with health and dementia

    • Previous studies have shown that criminal and other externalizing behaviors are associated with several adverse outcomes, but very little is known about the impact of these behaviors beyond middle adulthood. Few studies have explored how a life-course background of criminal and externalizing behaviors influence aging and more specifically, whether it is associated with the onset and development of different neurodegenerative, mental, and physical health disorders when aging. The overarching aim of this dissertation is to advance the knowledge about the long-term influence that criminal and other externalizing behaviors along the lifespan may have on health and neurodegeneration while individuals age. This aim was explored throughout three studies: Study I, a systematic review and meta-analysis performed to investigate the prevalence of several mental and physical health problems of older offenders; Study II, a Swedish population-based register study which examined how the severity of the criminal background associated with dementia and mild cognitive impairment (MCI), and how several life-course factors influenced these associations and; Study III, a multi-generation cohort study investigating whether externalizing behaviors and dementia co-aggregate in families. The main findings suggest that older adults with criminal and externalizing behavioral backgrounds, and overall, those with a severe criminal history, exhibit an increased liability to develop physical and mental health problems as well as MCI and dementia when aging. This increased risk is influenced by life-course health and psychosocial problems as well as genetic and familial environmental factors. In general, findings from this thesis point towards a better understanding of the aging process of individuals with this background, and to further the scientific knowledge about the influence of life-course adverse behaviors on aging. This knowledge may promote the development of preventive and interventive strategies for individuals with a criminal and externalizing behavioral background.

    • This paper presents the main experiences gained and conclusions drawn from the demonstration of a first-of-its-kind wood-based biomethane production plant (20-MW capacity, 150 dry tonnes of biomass/day) and 10 years of operation of the 2–4-MW (10–20 dry tonnes of biomass/day) research gasifier at Chalmers University of Technology in Sweden. Based on the experience gained, an elaborated outline for commercialization of the technology for a wide spectrum of applications and end products is defined. The main findings are related to the use of biomass ash constituents as a catalyst for the process and the application of coated heat exchangers, such that regular fluidized bed boilers can be retrofitted to become biomass gasifiers. Among the recirculation of the ash streams within the process, presence of the alkali salt in the system is identified as highly important for control of the tar species. Combined with new insights on fuel feeding and reactor design, these two major findings form the basis for a comprehensive process layout that can support a gradual transformation of existing boilers in district heating networks and in pulp, paper and saw mills, and it facilitates the exploitation of existing oil refineries and petrochemical plants for large-scale production of renewable fuels, chemicals, and materials from biomass and wastes. The potential for electrification of those process layouts are also discussed. The commercialization route represents an example of how biomass conversion develops and integrates with existing industrial and energy infrastructures to form highly effective systems that deliver a wide range of end products. Illustrating the potential, the existing fluidized bed boilers in Sweden alone represent a jet fuel production capacity that corresponds to 10% of current global consumption.

  • Do psychopathic personality traits in childhood predict subsequent criminality and psychiatric outcomes over and above childhood behavioral problems?

    • Purpose: We investigated whether childhood psychopathic personality traits predicted subsequent self-reported and register-based criminality and psychiatric outcomes when childhood behavioral problems (conduct disorder [CD] and ADHD symptoms) were accounted for.Methods: In the Child and Adolescent Twin Study in Sweden, parents rated their children's (n = 12,394) psychopathic personality traits, CD, and ADHD symptoms at age 9/12. We studied the risk for self-reported delinquency, problematic substance use, and anxiety/depression at age 18. The sample was also linked to nationwide registers where we studied suspicion of crimes, and diagnoses of substance use disorders and anxiety/depression up to age 21.Results: Childhood psychopathic personality traits were associated with self-reported delinquency (β = 0.65, 95% CI: 0.41–0.90) and suspicions of violent (hazard ratio [HR] = 1.33, [1.23–1.45]) and non-violent (HR = 1.28, [1.22–1.36]) crimes. The estimates were attenuated, but remained elevated for delinquency and violent crimes after accounting for childhood behavioral problems. Psychopathic personality traits were associated with substance use problems and anxiety/depression, but these associations were mainly explained by childhood behavioral problems.Conclusions: Psychopathic personality traits were a risk marker for criminality and psychiatric outcomes, particularly in children with co-occurring behavioral problems. However, the independent contribution of psychopathic personality traits was modest at best, when behavioral problems were accounted for.

  • Relative Immaturity in Childhood and Attention-Deficit/Hyperactivity Disorder Symptoms From Childhood to Early Adulthood : Exploring Genetic and Environmental Overlap Across Development

    • Objective: Attention-deficit/hyperactivity disorder (ADHD) has been linked to immaturity relative to peers in childhood, yet it is unclear how such immaturity is associated with ADHD across development. This longitudinal twin study examined the genetic and environmental contributions to the association between parents' perception of their child's immaturity relative to peers (RI) in childhood and ADHD symptoms across development.Method: 1,302 twin pairs from the Swedish Twin Study of Child and Adolescent Development were followed prospectively from childhood to early adulthood. Parent ratings of RI were collected at 8 to 9 years and parent and self-ratings of ADHD symptoms were collected at 8 to 9, 13 to 14, 16 to 17, and 19 to 20 years using the Child Behavior Checklist Attention Problems scale. In addition, ADHD symptoms corresponding to DSM criteria were used for sensitivity analysis. Analyses were conducted using longitudinal structural equation modeling with multiple raters.Results: RI-related etiologic factors, predominantly influenced by genes, explained 10-14% of the variance in ADHD symptoms from 8 to 9 up to 16 to 17 years. The influence of these RI-related factors on ADHD symptoms attenuated to 4% by 19 to 20 years of age. The remaining variance in ADHD symptoms was primarily explained by genetic factors independent of RI, which remained relatively stable across development, explaining 19% to 30% of the variance in ADHD symptoms from 13 to 14 up to 19 to 20 years.Conclusion: The results show that RI is significantly associated with ADHD symptoms, particularly during childhood and adolescence, and that the association is primarily explained by a shared genetic liability. Nevertheless, the magnitude of associations across development was modest, highlighting that RI is merely one aspect contributing to the complex etiology of ADHD symptoms.

  • How interactions between ADHD and schools affect educational achievement : a family-based genetically sensitive study.

    • BACKGROUND: Children with ADHD tend to achieve less than their peers in school. It is unknown whether schools moderate this association. Nonrandom selection of children into schools related to variations in their ADHD risk poses a methodological problem.METHODS: We linked data on ADHD symptoms of inattention and hyperactivity and parent-child ADHD polygenic scores (PGS) from the Norwegian Mother, Father, and Child Cohort Study (MoBa) to achievement in standardised tests and school identifiers. We estimated interactions of schools with individual differences between students in inattention, hyperactivity, and ADHD-PGS using multilevel models with random slopes for ADHD effects on achievement over schools. In our PGS analyses, we adjust for parental selection of schools by adjusting for parental ADHD-PGS (a within-family PGS design). We then tested whether five school sociodemographic measures explained any interactions.RESULTS: Analysis of up to 23,598 students attending 2,579 schools revealed interactions between school and ADHD effects on achievement. The variability between schools in the effects of inattention, hyperactivity and within-family ADHD-PGS on achievement was 0.08, 0.07 and 0.05 SDs, respectively. For example, the average effect of inattention on achievement was β = -0.23 (SE = 0.009), but in 2.5% of schools with the weakest effects, the value was -0.07 or less. ADHD has a weaker effect on achievement in higher-performing schools. Schools make more of a difference to the achievements of students with higher levels of ADHD, explaining over four times as much variance in achievement for those with high versus average inattention symptoms. School sociodemographic measures could not explain the ADHD-by-school interactions.CONCLUSIONS: Although ADHD symptoms and genetic risk tend to hinder achievement, schools where their effects are weaker do exist. Differences between schools in support for children with ADHD should be evened out.

  • Clinical Characteristics and Pharmacological Treatment of Individuals With and Without Intellectual Disability in Pre-trial Assessment : A Population-Based Study

    • Background: The current lack of knowledge about intellectual disability (ID) in forensic psychiatric contexts can compromise the legal certainty of these individuals during the medico-legal process. To address ambiguous results in previous literature, the aim of the current study was to estimate the prevalence of ID in a pre-trial forensic psychiatric settings. Moreover, as little is known about the characteristics of offenders with ID, we conducted a clinical characterization of individuals with and without ID being subject to forensic psychiatric assessment.Methods: Using data from several Swedish national registers, we conducted a population-based retrospective observational study on 8,442 individuals being subject to pre-trial forensic psychiatric assessments in Sweden in 1997–2013. We performed univariate analyses to compare the characteristics of individuals with (n = 537) and without ID (n = 7,905).Results: The prevalence of ID was 6.4% in the Swedish pre-trial forensic psychiatric context during the observational period. Compared with individuals without ID, individuals with ID were younger at the time of assessment, had a lower educational level, and had less frequently started families. ID was associated with lower frequency of diagnosed psychotic and bipolar disorders. However, a similar prescription rate of antipsychotics, and a comparable rate of previous inpatient care was observed among individuals with and without ID. Individuals with ID had more often been prescribed anti-libidinal treatments often used for treating sexual disorders, although did not present a higher prevalence of sexual disorder.Conclusions: The prevalence of ID among pre-trial individuals being subject to forensic psychiatric assessment was more than twice as high as assumed in the general population. Our results suggest that individuals with ID received pharmacotherapy without clear indication. Remaining challenges in the clinical management of individuals with ID were indicated by the discrepancy between the occurrence of psychiatric diagnoses, pharmacological treatment patterns, and rates of inpatient care.

  • Increased Prevalence of Accidents and Injuries in Congenital Adrenal Hyperplasia : A Population-Based Cohort Study

    • CONTEXT: It has been suggested that injuries and accidents are increased in females with congenital adrenal hyperplasia (CAH), but the prevalence is unclear.OBJECTIVE: To study the prevalence of injuries and accidents in females and males with CAH.DESIGN, SETTING, AND PARTICIPANTS: Patients with CAH (n = 714, all 21-hydroxylase deficiency) were compared with matched controls (n = 71,400). Data were derived by linking National Population-Based Registers.MAIN OUTCOME MEASURES: Prevalence of injuries and accidents.RESULTS: Mean age was 29.8 ± 18.4 years. Injuries were more prevalent in patients with CAH than in controls (RR 1.34, 95%CI 1.24-1.44), and this was found in both sexes (females: 1.43, 1.29-1.58; males: 1.25, 1.12-1.38). In the classical phenotype, the prevalence of injuries was higher, especially in females but not in the non-classic phenotype. In the genotype groups, injuries were mainly increased in females. Head injuries were increased in all patients with CAH and in the different phenotypes and were mainly driven by females. More patients with CAH born before the introduction of neonatal screening had had an injury compared to controls (1.48, 1.35-1.62), this was seen in both sexes. In patients with CAH born after the introduction of screening, the prevalence of injuries was overall increased (1.20, 1.07-1.35), and in females with CAH but not in males. Accidents showed a similar pattern to injuries in all comparisons.CONCLUSIONS: Patients with CAH had an increased prevalence of both injuries and accidents, especially in females and in those born before the neonatal screening program. Patients with NC phenotype were hardly affected.

    • Background: Antisocial behaviour may partly develop as a consequence of psychopathic personality. However, neither the direction of effects nor the aetiology of the association has previously been clarified. The aim in this study was to investigate the direction of effects between psychopathic personality and antisocial behaviour, and to investigate the genetic and environmental contribution to this association. Method: Twins (n = 2,255) in the Swedish Twin Study of Child and Adolescent Development were prospectively followed from adolescence to adulthood. We used a longitudinal cross-lagged twin model to study the associations between psychopathic personality and antisocial behaviour. Results: Psychopathic personality in mid-adolescence predicted antisocial behaviour in adulthood (p < .001), but not the other way around. However, bidirectional effects were found when a measure of persistent antisocial behaviour (from age 8-9 to age 16-17) was used. Psychopathic personality predicted both rule-breaking behaviour (p < .001) and aggressive behaviour (p < .01). Genetic factors were of importance in mediating the longitudinal associations between psychopathic personality and antisocial behaviour. Conclusions: This study provides evidence that genetically influenced psychopathic personality is a robust predictor of adult antisocial behaviour, but also that persistent antisocial behaviour has an impact on adult psychopathic personality via genetic effects.

    • OBJECTIVE: To explore the associations of subtypes of adult ADHD with other psychiatric problems, stressful life events, and sex differences.METHOD: Odds ratios were calculated using information from 17,899 participants from a population-based survey of adult twins born in Sweden between 1959 and 1985.RESULTS: Symptoms of attention deficit hyperactivity disorder (ADHD) were associated with an increased risk for symptoms of (odds ratio [95% confidence interval]): generalized anxiety disorder (5.6 [4.3; 6.5]), major depression (2.8 [2.4; 3.2]), bipolar disorder (8.0 [5.1; 12.6]), obsessive-compulsive disorder (3.9 [3.1; 4.9]), and alcohol dependence (2.6 [2.2; 3.1]). Symptoms of ADHD were found to be associated with an increased risk for stressful life events (1.8 [1.3; 2.4]). No significant difference in comorbidity was observed between the two sexes.CONCLUSION: Both women and men with ADHD are at increased risk for symptoms of other psychiatric disorders. They are also at increased risk for stressful life events.
