  • Veiled midwifery in the baby factory : The social pattern between other professions and midwives.

    • Background: There has been a paradigm shift in midwifery over time where different professions now work together in childbirth care. This demands a greater effort of interprofessional collaboration. There is little research on midwives’ work from other professionals’ perspectives, which is of importance to improve midwives work situation and women-centred care. Therefore, the aim of this article was to explore other professions´ views of midwifery work during childbirth. Method: Classical Grounded Theory, using a constant comparative analysis, was applied to focus group interviews with obstetricians, assistant nurses and managers. Result: The substantive theory of ‘veiled midwifery’ emerged as an explanation of the social process between the professions in the ‘baby factory’ context. The other professionals perceive midwifery through a veil that filters the reality and only permits fragmentary images of the midwives´ work. The main concern for the other professions was that the midwives were ‘marching to own drum’. The midwives were perceived as both in dissonance with the baby factory, and therefore hard to control, or, alternatively more compliant with the prevailing rhythm. This caused an unpredictability, which in turn resulted in attempts to cooperate and gain access to the midwifery world, by using three unveiling strategies: Scrutinising, Streamlining and Collaborating admittance. Conclusion: The theory of veiled midwifery could be used as a theoretical basis for future studies, and could be a foundation for a dialogue of philosophical differences in the way birth is viewed in the clinical setting, to improve the work situation and women-centred care.

    • Purpose: To explore and analyse the experience of work situation and professional role for midwives at a labour ward pre and post the implementation of a midwifery model of care (MiMo). Methods: A simultaneous mixed method was used. The qualitative core component departed from three focus group interviews (n=16 midwives). Secondary inductive and deductive content analysis was performed using an unconstrained matrix to make a corresponding comparison of the different time points. The supplemental component was a quantitative survey about the work situation (n=58). Results: The qualitative results pre the implementation showed three categories: Balance between Women and Organization, Midwives-Diverse as both Profession and Person, and Strained Work Situation. Post the intervention they transformed to Balance between Midwifery and Organization, Midwives-An Adaptable Profession, Strained Work Situation, and a new category Ability to concretize midwifery was found. There were no significant differences in the measures of work situation in the quantitative analyses. Conclusions: The synthesized findings based on the qualitative part show that MiMo has a potential to strengthen the professional role and midwifery practice. As such, MiMo has the capability to offer benefits to the labour wards with additional considerations.

    • Background: The work situation for midwives in different countries is related to high levels of stress, burnout and heavy work load. One aspect of the professional role of midwives is woman-centeredness, theoretically described in midwifery models of care. However, no studies are found about the outcome for midwives work related to midwifery models of care. Therefore, the aim of this study was to explore and analyse the experience of work situation and professional role for midwives at a labour ward, pre and post the introduction of a midwifery model of care (MiMo). Methods: A simultaneous qualitative and quantitative mixed method approach was used in this longitudinal study. The core component comprised of a qualitative inductive secondary content analysis of three focus group interviews with 16 midwives exploring how midwives experienced and described their work situation and professional role pre and post implementation of MiMo. The supplemental component were a quantitative survey analysis of the work situation for midwives (n=58) pre and post the intervention, and the deductive analysis was driven by the qualitative result. Findings: The qualitative core component consisted of the concepts Balance between Women and Organisation, Midwives - Diverse as both Profession and Person and Strained Work Situation pre intervention. Post intervention Balance between Midwifery and Organisation, Midwives - An Adaptable Profession, Strained Work Situation and lastly a new category Ability to concretise midwifery emerged. The quantitative items that had corresponding measures connected to the qualitative categories were analysed. There were no significant differences in any of the quantitative analyses pre and post the introduction of MiMo. Conclusion: Working according to MiMo appears not to have any effect on the strained work situation in midwives, in the context and with the measurements studied here. Although MiMo contributed to raise awareness of the professional role.

  • Midwives’ attitude towards labour pain: Expectations and perceptions by Jordanian women

    • Background: In Jordan, as in other countries, the majority of women give birth in public hospitals where the midwives are the main birth attendants. Although most women have trust in their midwives; there are studies describing dissatisfaction of childbirth experience, fears associated with experiencing painful births and ineffective pain management during childbirth. Thus, there is a need to understand the Jordanian midwives' attitude towards labour pain from both the midwives and the women’s perspectives. Aim: To explore the Jordanian midwives' attitude towards labour pain by assessing the midwives' knowledge and attitude towards pain and the women's expectations and perceptions of their midwives' approach towards pain. Methods: The research took place in the largest public hospital in Jordan in the postnatal ward and involved midwives and their clients, including 60 midwives (out of 61 randomly sampled midwives), who responded to a study-specific validated survey for midwives (Survey Questionnaire for Midwives), and 360 women (out of 384 sampled women), who responded to a study-specific validated survey for women (Survey Questionnaire for Women). Findings: The study is ongoing. The findings are presented at the NJF conference (May 2016) and as a part of doctoral thesis that should be defended in May 2017.

  • Norwegian women's motivations and preparations for freebirth - A qualitative study

    • © 2020 Elsevier B.V. Aim: This study was aimed at describing Norwegian women's motivations and preparations for freebirth. Methods: This qualitative study involved 12 individual interviews conducted face to face or via Skype with women from different parts of Norway. The material was analysed using qualitative content analysis inspired by Graneheim and Lundman. Results: Three categories describing the women's motivations and preparations for freebirth were identified. Unsatisfied with the care offered today described how the women thought that hospitals did not support normal birth and made an inadequate homebirth offer. The category earlier uncomplicated and traumatic births influence freebirth choices described two different dimensions of motivations for freebirth. Trust in one's own knowledge and capacity referred to how women viewed birth as a natural process, their faith in themselves, how this view and faith influenced their preparation and how they gained knowledge about the birth process to prepare. An overall theme emerged: deep trust in birth as a natural process and the women's own capacity to give birth embedded in distrust of the maternity care system. Conclusion: This study showed that motivations for freebirth were embedded in overall dissatisfaction with today's maternity care, the inadequate homebirth offer and deep trust in the women's own capacity to give birth.

    • Objective: To investigate the prevalence of childbirth-related fear from pregnancy to one year after childbirth and to identify factors associated with being cured of childbirth-related fear. Design: A longitudinal regional survey. Setting: Three hospitals in a northern part of Sweden. Participants: Six hundred ninety-seven (697) women who completed four questionnaires. Methods: Data were collected by questionnaires in mid and late pregnancy and at 2 months and one year after birth. Childbirth related fear was measured 3 times. Results: There was a statistically significant increase in childbirth fear from 12.4% in mid-pregnancy to 15.1% one year after childbirth (p < .001). Women who were cured of childbirth fear reported a better birth experience and would prefer a vaginal birth in a subsequent pregnancy. These women were also more likely to experience a feeling of control during birth and were more satisfied with information about the progress of labor, but there was no difference in prenatal counseling or having an elective cesarean between the groups. Conclusion: Women with prenatal fear of childbirth may be cured of this fear by having a better birth experience. If women feel in control of their bodies and are well informed about the progress of labor, the chances of being cured will increase. Prenatal counseling or having an elective cesarean birth does not seem to be a solution for relieving childbirth fear.

    • Objective: Intimate partner violence (IPV) is a large public health problem with far-reaching consequences for those involved. The aim of this study was to explore fathers’ experiences of change during pregnancy and early parenthood in the context of IPV. Methods: The methodological approach in this interview study was hermeneutics, based on a lifeworld perspective. Ten men, who had subjected their partners to violence during the childbearing period, and had become fathers within the previous 6 years, participated. Results: The analysis revealed four themes: beginning to acknowledge that you are inflicting violence, receiving confirmation that you are more than just a perpetrator of violence, becoming aware of the child, and the desire to receive support in the process of learning how to become a father. Levinas’ concept “the face of the other” is used to interpret the findings. Conclusion: This study contributes to a more nuanced and expanded picture of IPV. It shows that men who inflict violence want to be and learn how to be fathers. We need more knowledge about how to stop violent acts and support these men in the process of fatherhood.

    • A phenomenological hermeneutic approach was used to illuminate the meaning of the lived experience of becoming a father, in men who had subjected their partners to violence. Ten fathers were interviewed. Four themes were developed: being forced to take responsibility; being afraid that the violence will continue in subsequent generations; striving to stay in control; and being a “good enough” father. The transition to becoming a father is a demanding process with ambiguous emotions, and the situation is especially difficult when the men have a history of violent behavior. The men clearly distinguished between the pregnant woman and the unborn child. Although they exercised violence against the women, they wanted to make their children safe, to protect them from violence, and to become good fathers.

    • The study’s rationale: Women’s experiences regarding labour pain preparation and management have been largely neglected. Aims and objectives: Explore women’s experiences regarding labour pain preparation and management in normal childbirth. Methodological design and justification: The Vancouver School of doing phenomenology was the methodological approach of the study since it provides inside information about the lived experience. Research methods: Data were collected through in-depth interviews with 14 participants; seven primiparous and seven multiparous women. Results: The women described a challenging journey of no return through labour pain, with different landmarks on the journey. They described how they prepared for the pain; the context of the pain experience; how they experienced and managed the pain with different strategies and how they saw the pain at the journey’s end. The quality of the midwife’s presence and professionalism was of great importance to them. The ‘good midwives’ they described created a special atmosphere which was warm and secure and was conducive to their managing the pain. The women also described how important it was for them to have a supportive partner, with whom they had a mutual understanding, in order to manage the pain. Conclusions: In this paper, we are presenting a study within the third paradigm in labour pain preparation and management: the childbearing woman’s paradigm – the first and second being the midwifery and the medical paradigm, respectively. Midwives can play a major role in working with women in their preparation and management of labour pain. In the future, more research has to be done to illuminate this essential part of the childbearing woman’s paradigm.
