  • Complete replacement of open repair for ruptured abdominal aortic aneurysms by endovascular aneurysm repair : a two-center 14-year experience

    • Objective: To present the combined 14-year experience of 2 university centers performing endovascular aneurysm repair (EVAR) on 100% of noninfected ruptured abdominal aortic aneurysms (RAAA) over the last 32 months.Background: : Endovascular aneurysm repair for RAAA feasibility is reported to be 20% to 50%, and EVAR for RAAA has been reported to have better outcomes than open repair.Methods: We retrospectively analyzed prospectively gathered data on 473 consecutive RAAA patients (Zurich, 295; Örebro, 178) from January 1, 1998, to December 31, 2011, treated by an "EVAR-whenever-possible" approach until April 2009 (EVAR/OPEN period) and thereafter according to a "100% EVAR" approach (EVAR-ONLY period).Straightforward cases were treated by standard EVAR. More complex RAAA were managed during EVAR-ONLY with adjunctive procedures in 17 of 70 patients (24%): chimney, 3; open iliac debranching, 1; coiling, 8; onyx, 3; and chimney plus onyx, 2.Results: Since May 2009, all RAAA but one have been treated by EVAR (Zurich, 31; Örebro, 39); 30-day mortality for EVAR-ONLY was 24% (17 of 70). Total cohort mortality (including medically treated patients) for EVAR/OPEN was 32.8% (131 of 400) compared with 27.4% (20 of 73) for EVAR-ONLY (P = 0.376). During EVAR/OPEN, 10% (39 of 400) of patients were treated medically compared with 4% (3 of 73) of patients during EVAR-ONLY. In EVAR/OPEN, open repair showed a statistically significant association with 30-day mortality (adjusted odds ratio [OR] = 3.3; 95% confidence interval [CI], 1.4-7.5; P = 0.004). For patients with no abdominal decompression, there was a higher mortality with open repair than EVAR (adjusted OR = 5.6; 95% CI, 1.9-16.7). In patients with abdominal decompression by laparotomy, there was no difference in mortality (adjusted OR = 1.1; 95% CI, 0.3-3.7).Conclusions: The "EVAR-ONLY" approach has allowed EVAR treatment of nearly all incoming RAAA with low mortality and turndown rates. Although the observed association of a higher EVAR mortality with abdominal decompression needs further study, our results support superiority and more widespread adoption of EVAR for the treatment of RAAA.

  • Evolution of pigment-dispersing factor neuropeptides in panarthropoda : Insights from onychophora (Velvet Worms) and tardigrada (Water Bears)

    • Pigment-dispersing factor (PDF) denotes a conserved family of homologous neuropeptides present in several invertebrate groups, including mollusks, nematodes, insects and crustaceans (referred to here as pigment-dispersing hormone, PDH). Regarding their encoding genes (pdf, pdh), insects possess only one, nematodes two, and decapod crustaceans up to three, but their phylogenetic relationship is unknown. To shed light on the origin and diversification of pdf/pdh homologs in Panarthropoda (Onychophora + Tardigrada + Arthropoda) and other molting animals (Ecdysozoa), we analyzed the transcriptomes of five distantly related onychophorans and a representative tardigrade and searched for putative pdf homologs in publically available genomes of other protostomes. This revealed only one pdf homolog in several mollusk and annelid species, two in Onychophora, Priapulida and Nematoda, and three in Tardigrada. Phylogenetic analyses suggest that the last common ancestor of Panarthropoda possessed two pdf homologs, one of which was lost in the arthropod or arthropod/tardigrade lineage, followed by subsequent duplications of the remaining homolog in some taxa. Immunolocalization of PDF-like peptides in six onychophoran species, using a broadly reactive antibody that recognizes PDF/PDH peptides in numerous species, revealed an elaborate system of neurons and fibers in their central and peripheral nervous systems. Large varicose projections in the heart suggest that the PDF neuropeptides functioned as both circulating hormones and locally released transmitters in the last common ancestor of Onychophora and Arthropoda. The lack of PDF-like immunoreactive somata associated with the onychophoran optic ganglion conforms to the hypothesis that onychophoran eyes are homologous to the arthropod median ocelli.

  • Widespread Pesticide Distribution in the European Atmosphere Questions their Degradability in Air

    • Risk assessment of pesticide impacts on remote ecosystems makes use of model-estimated degradation in air. Recent studies suggest these degradation rates to be overestimated, questioning current pesticide regulation. Here, we investigated the concentrations of 76 pesticides in Europe at 29 rural, coastal, mountain, and polar sites during the agricultural application season. Overall, 58 pesticides were observed in the European atmosphere. Low spatial variation of 7 pesticides suggests continental-scale atmospheric dispersal. Based on concentrations in free tropospheric air and at Arctic sites, 22 pesticides were identified to be prone to long-range atmospheric transport, which included 15 substances approved for agricultural use in Europe and 7 banned ones. Comparison between concentrations at remote sites and those found at pesticide source areas suggests long atmospheric lifetimes of atrazine, cyprodinil, spiroxamine, tebuconazole, terbuthylazine, and thiacloprid. In general, our findings suggest that atmospheric transport and persistence of pesticides have been underestimated and that their risk assessment needs to be improved.

  • Erosion and deposition in the JET divertor during the first ILW campaign

    • Erosion and deposition were studied in the JET divertor during the first JET ITER-like wall campaign 2011 to 2012 using marker tiles. An almost complete poloidal section consisting of tiles 0, 1, 3, 4, 6, 7, 8 was studied. The data from divertor tile surfaces were completed by the analysis of samples from remote divertor areas and from the inner wall cladding. The total mass of material deposited in the divertor decreased by a factor of 4-9 compared to the deposition of carbon during all-carbon JET operation before 2010. Deposits in 2011 to 2012 consist mainly of beryllium with 5-20 at.% of carbon and oxygen, respectively, and small amounts of Ni, Cr, Fe and W. This decrease of material deposition in the divertor is accompanied by a decrease of total deuterium retention inside the JET vessel by a factor of 10 to 20. The detailed erosion/deposition pattern in the divertor with the ITER-like wall configuration shows rigorous changes compared to the pattern with the all-carbon JET configuration.

    • Erosion of plasma-facing materials and successive transport and redeposition of eroded material are crucial processes determining the lifetime of plasma-facing components and the trapped tritium inventory in redeposited material layers. Erosion and deposition in the JET divertor were studied during the second JET ITER-like wall campaign ILW-2 in 2013-2014 by using a poloidal row of specially prepared divertor marker tiles including the tungsten bulk tile 5. The marker tiles were analyzed using elastic backscattering with 3-4.5 MeV incident protons and nuclear reaction analysis using 0.8-4.5 MeV He-3 ions before and after the campaign. The erosion/deposition pattern observed during ILW-2 is qualitatively comparable to the first campaign ILW-1 in 2011-2012: deposits consist mainly of beryllium with 5-20 at.% of carbon and oxygen and small amounts of Ni and W. The highest deposition with deposited layer thicknesses up to 30 mu m per campaign is still observed on the upper and horizontal parts of the inner divertor. Outer divertor tiles 5, 6, 7 and 8 are net W erosion areas. The observed D inventory is roughly comparable to the inventory observed during ILW-1. The results obtained during ILW-2 therefore confirm the positive results observed in ILW-1 with respect to reduced material deposition and hydrogen isotopes retention in the divertor.

    • Hydrogen retention in tokamaks is due to implantation into plasma-facing materials and trapping in deposited layers. In the limiter tokamak TEXTOR-94 hydrogen-rich deposited layers with thicknesses up to 1 mm are observed on recessed parts of the limiters, areas perpendicular to the magnetic field in the scrape-off layer (SOL), neutralizer plates of the pumped limiter and inside the pumping ducts. In the divertor tokamak JET the main deposition is observed in the divertor, additional deposits are observed in the main chamber on the sides of the guard limiters. Codeposition of carbon ions with hydrogen is the major mechanism of layer growth at areas with direct plasma contact. At remote areas without direct plasma contact, sticking of neutral hydrocarbon radicals seems to play an important role for hydrogen trapping.

    • A novel trend to achieve long-lifetime or even self-sustainable wearable devices is to exploit energy harvesting from environmental or body energy. Their power management circuits need to be carefully designed to enable a small-size energy-efficient wearable system, supporting multiple power domains ranging from mW in an active mode to nW in sleep modes. This work presents a fully configurable smart power unit suitable for kinetic energy harvesting transducers, Kinetron MGS, enabling self-sustainability for wearable devices. The proposed power management circuit hosts a low power microcontroller to manage the energy harvesting, voltage conversion from batteries, and wake-up circuits to exit low power states automatically. The source and the load power points are decoupled with multiple DC-DC converters aiming to supply loads with adaptive voltage scaling and high reliability. Experimental results using commercial micro-kinetic generators show the flexibility and efficiency of this approach: the proposed power supply unit achieves a quiescent current of 57 nA and a maximum load current of 300 mA, delivered with a harvesting efficiency of 79 %. We evaluate the proposed system with commercial kinetic energy harvesting transducer generating 1.18 mW when worn on the ankle during walking. © 2020 IEEE.

  • A2Event : A Micro-Watt Programmable Frequency-Time Detector for Always-On Energy-Neutral Sensing

    • Internet of Things (IoT) sensors are becoming more and more intelligence and unobtrusive, surrounding us in everyday life. A key requirement for those devices is to be “always-on” performing “smart” detection to catch only the events of interest when they occur. A recent and trend is to design low power circuit to extract features close to the sensors without an analog-digital conversion. The goal is to achieve place-and-forget deployment with perpetual operation: this poses the challenge of achieving micro-watt power consumption of the always-on analog event detector and of harvesting energy from environmental sources, to supply the smart sensor. This paper presents A2Event (Analog to Event), a microwatt programmable pattern recognition circuit for analog sensors, with up to 128 simultaneous time-frequency features exploiting mixed-signal low power design, which is able to generate an event when a targeted pattern is recognized. Moreover, the paper proposes a self-sustaining always-on smart audio detector that combines A2Event with energy harvesting to detect events of interests perpetually. Experimental results show that A2Event consumes only 26.89 μW in always-on mode, during the time-frequency feature-extraction, while the whole system consumes only 63 μW during pattern recognition including the power for a commercial MEMS microphone and the energy harvesting subsystem. We demonstrate that the whole smart sensor operates perpetually when powered with a small form factor flexible photovoltaic panel in indoor lighting conditions, achieving a detection accuracy of 100% in the detection of two different audio streams. © 2019 Elsevier Inc.
