    • BACKGROUND: Oral implant treatment on patients with poor jaw-bone texture has shown increased failure rates in series of studies. PURPOSE: The purpose of the present study was to retrospectively follow patients with osteoporosis of the axial or appendicular skeleton, including the jaw bone, being subjected to oral implant treatment. The outcome of inserted implants, when using an adapted bone site preparation technique and extended healing periods, was evaluated. MATERIALS AND METHODS: Based on data obtained from preoperative radiographs, patient medical history, and resistance of the jaw bone perceived during drilling, 14 of 16 patients were referred to the Osteoporosis Laboratory, Sahlgren University Hospital, Göteborg, Sweden, for bone density measurements. Two patients already had an established diagnosis of osteoporosis. Fourteen jaws in 13 patients (11 females, 2 males; mean age: 68 yr) were subsequently subjected to oral implant treatment with a total of 70 implants (Brånemark System) of various designs. The mean follow-up period was 3 years and 4 months (range: 6 mo-11 yr). RESULTS: Osteoporosis of either the spine, the hip, or both regions was diagnosed in 14 patients, and osteopenia was diagnosed in 2 patients. Two implants failed, and the overall implant survival rate at the end of the study period was 97.0% for maxillae and 97.3% for mandibles. The marginal bone resorption at the 1-year follow-up concurs with the outcome of other studies, irrespective of the preoperative bone texture present. CONCLUSION: The outcome of the present study showed that implant placement in patients in whom the average bone density showed osteoporosis in both lumbar spine and hip as well as poor local bone texture may be successful over a period of many years.

    • Fatty acids are involved in bone development but knowledge in children is limited. The aim of this study was to investigate bone mass and mineral density in healthy preschool children in relation to fatty acids.In 111 healthy 4-yrs-old children (20% overweight) bone was analysed by dual X-ray absorptiometry and serum phospholipid fatty acid by gas chromatography. Fat intake was calculated from 7 days self-reported dietary records and food frequency questionnaire.Total bone mass content (BMC) and mineral density (BMD) differed by sex in normal weight, but not in overweight children showing generally higher bone mass density than children with normal weight. Linoleic acid intake was strongly correlated to BMC and femoral BMD in normal weight children. Serum concentration of docosahexaenoic acid correlated positively to BMD in all children (p=0.01), but linoleic and arachidonic acids, and monounsaturated fatty acids showed diverging associations with bone in normal weight and overweight children.Serum phospholipid DHA was associated with bone density. Other fatty acids associations to bone sites differed in overweight children, analogue to the pattern in healthy 8-yrs-old.The finding need to be confirmed longitudinally and in a larger group of overweight individuals.

    • The aim of this study is to evaluate the prevalence and severity of low back pain (LBP) and the influence of sciatica and neurological deficits in old men. Mister osteoporosis Sweden includes 3,014 community-dwelling men aged 69-81 years. At study start 3,009 participants answered questions on LBP, low back pain and sciatica (LBP + SCI) or low back pain and sciatica with associated neurological deficits (LBP + SCI + NEU) during the preceding 12 months. Data are presented as proportions or medians with mid-quartile ranges. Differences between groups were tested by chi(2) test and Kruskall-Wallis test. 24 % had experienced LBP without SCI, 8 % LBP + SCI and 14 % LBP + SCI + NEU. 10 % of the men with LBP, 22 % of those with LBP + SCI, and 36 % of those with LBP + SCI + NEU rated the pain as severe (p < 0.001). 23 % of the men with LBP, 31 % of those with LBP + SCI and 50 % of those with LBP + SCI + NEU reported limitation in activity of daily living (ADL) (p < 0.001). Men with only LBP had to restrict their activities for 7 days (3-14), those with LBP + SCI 6 days (2-14) and those with LBP + SCI + NEU 10 days (3-30) (p < 0.05). The 1-year prevalence of LBP in community living men aged 69-81 years was close to 50 % but for individuals with LBP or LBP + SCI the morbidity was low with more than two-thirds having no limitations in ADL. In men with LBP + SCI + NEU more than one-third rated the pain as severe and close to half had limitations in ADL.

  • Low Circulating Valine Associate With High Risk of Hip Fractures

    • Context Hip fractures constitute a major health concern. An adequate supply of amino acids is crucial to ensure optimal acquisition and remodeling of bone. Circulating amino acid levels have been proposed as markers of bone mineral density, but data on their ability to predict incident fractures are scarce. Objectives To investigate the associations between circulating amino acids and incident fractures. Methods We used UK Biobank (n = 111 257; 901 hip fracture cases) as a discovery cohort and the Umea Fracture and Osteoporosis (UFO) hip fracture study (hip fracture cases n = 2225; controls n = 2225) for replication. Associations with bone microstructure parameters were tested in a subsample of Osteoporotic Fractures in Men Sweden (n = 449). Results Circulating valine was robustly associated with hip fractures in the UK Biobank (HR per SD increase 0.79, 95% CI 0.73-0.84), and this finding was replicated in the UFO study (combined meta-analysis including 3126 incident hip fracture cases, odds ratio per SD increase 0.84, 95% CI 0.80-0.88). Detailed bone microstructure analyses showed that high circulating valine was associated with high cortical bone area and trabecular thickness. Conclusion Low circulating valine is a robust predictor of incident hip fractures. We propose that circulating valine may add information for hip fracture prediction. Future studies are warranted to determine whether low valine is causally associated with hip fractures.

    • In children, but not adults with cystic fibrosis (CF), associations between essential fatty acids (FA) and bone mass have been reported. Low bone mineral density (BMD) is common in these patients. Previously we found a normal annual increase of BMD, suggesting a potential for attaining normal bone mass. The aim of the present study was to investigate phospholipid FA pattern in relation to bone in young adult men with CF compared with healthy controls. Fourteen male patients with CF were compared with forty-two healthy controls, using dual-energy X-ray absorptiometry for total bone, lumbar spine and femur and peripheral quantitative computerised tomography for tibia and radius. A questionnaire concerning physical activity and nutrition was used. FA in serum phospholipids were measured using capillary GLC. CF patients did not differ in physical activity and anthropometry from controls. There were no differences in bone parameters between the two groups, but patients chronically colonised with Pseudomonas aeruginosa had lower BMD than non-colonised patients. The trabecular BMD in the tibia differed between patients and controls, but not after adjustment for age and weight. The endosteal circumference of the radius was significantly associated with serum phospholipid concentration of DHA and inversely with the n-6:n-3 FA ratio in CF patients but not in controls. The present study showed that young physically active adult males with classical CF obtained similar bone mass as controls, although influenced by pseudomonas colonisation. The association between DHA and long bone endosteal circumference suggested a later peak bone mass in those with CF compared with controls.

    • OBJECTIVES: To determine prospectively for 2 years the change in bone mineral density (BMD) in patients with cystic fibrosis (CF) and to correlate clinical data and routine biochemical parameters of bone metabolism and infection with BMD. METHODS: Fifty-four patients with CF, aged 6 to 33 years, were included. BMD was measured using dual-energy x-ray absorptiometry in lumbar spine (LS) and femoral neck (FN). Anthropometric data and biochemical markers of bone metabolism and infection were measured. The number of intravenous antibiotic courses per year (ivAC) and pulmonary function were assessed. RESULTS: The patients had normal anthropometric data and normal growth, but 36% and 33% of the patients had BMD z score <-1 standard deviation in LS and in FN, respectively. Nevertheless, BMD increased at a normal rate during the 2 years and was correlated to weight and lung function. Intact parathyroid hormone was positively correlated with the increase of BMD in both LS and FN during childhood. Blood sedimentation rate, serum concentration of immunoglobulin G, and ivAC were negatively correlated with BMD in FN. Patients with 2 more severe CF transmembrane conductance regulator mutations had significantly lower BMD in FN than other genetic combinations. CONCLUSION: The study suggests that low BMD in CF is multifactorial and depends on infection and nutritional parameters. Differences in BMD of LS and FN suggested higher susceptibility to infection in FN at all ages. Longitudinal studies starting early before bacterial colonization would be valuable to determine the relative role of infection in the development of BMD in CF.
