    • BACKGROUND: Despite screening of blood donors, hepatitis C virus (HCV) infection can occur in patients who receive multiple transfusions. OBJECTIVE: To clarify mechanisms of nosocomial transmission of HCV. DESIGN: Epidemiologic and molecular analyses of hepatitis C outbreaks. SETTING: Pediatric oncology ward. PATIENTS: Children with cancer. MEASUREMENTS: Epidemiologic analysis, HCV RNA detection, genotyping, and hypervariable region 1 (HVR1) sequencing. RESULTS: Ten cases of infection with acute HCV genotype 3a occurred between 1990 and 1993. Sequencing of HVR1 revealed three related strains. Despite an overhaul of hygiene procedures, a patient infected with genotype 1b generated nine subsequent infected patients in 1994. Several patients had high virus titers and strongly delayed anti-HCV antibody responses. All had permanent intravenous catheters. Multidose vials used for flushing or treatment had probably been contaminated during periods of overlapping treatment. CONCLUSIONS: Contamination of multidose vials was the most likely mode of HCV transmission; therefore, use of such vials should be restricted. Rigorous adherence to hygiene routines remains essential to preventing transmission of bloodborne infections.

  • A population based pediatric oncology registry in Southern Sweden : the BORISS registry

    • A population based registry, with the acronym BORISS, was established. It contains all individuals (0-18 years of age at diagnosis) diagnosed with cancer from 1970-01-01 until 2016-12-31 in Southern Sweden. The treatment data has been entered into the registry after confirmation of the diagnosis by the Swedish national cancer registry and updates on vital status from the Swedish population registry. The number of individuals with a pediatric cancer diagnosed during these 46 years are 2928. Of these, 2065 are currently alive and 1882 individuals are 5-year survivors. Data on treatment and malignancy of the 5-year survivors has been collected from medical records and entered into the database. Treatment data contains surgical procedure, target organ of radiation therapy including dose and fractionation, and cytostatic treatment with dose (mg) per body surface area (m2) for all cytostatic agents. Data on individuals receiving stem cell treatment is included. The database is unique in that it is population based, contains all individuals and detailed treatment data on all 5-year survivors after childhood cancer in Southern Sweden since 1970. The database has contributed to several academic theses in the field of late effects after childhood cancer. BORISS also supports the Late Effect Clinic at Skåne University Hospital in Lund, Sweden with treatment details enabling a stratified surveillance.

  • Human liver lipids: representativity of determinations on material obtained by fine-needle aspiration biopsy

    • Triglycerides and phospholipids in human liver material obtained by fine-needle aspiration biopsy have been determined according to an improved version of a previously developed method. The molar ratios of these lipids have been compared with the same ratios determined on material representing whole liver. The average value of the ratios in the fine-needle specimens was 101% (83-113%) of the corresponding whole-liver value. The variation coefficient of determinations on several fine-needle specimens from the same liver was 10% (4-17%). The least difference, between the ratiosof two consecutive fine-needle specimens, to ensure a significant difference was 30%.

    • Children with acute lymphoblastic leukemia (ALL) have been shown to develop changes on computed tomography (CT) scans of the brain. These changes are seen both during and after therapy. Some of the results have been contradictory. Nine children with ALL were examined by CT of the brain during induction therapy. All children showed normal CT scans on the day of diagnosis. One month later all had dilatation of the ventricles and widened sulci. During the next 9 months the CT scans gradually were normalized. We conclude that such changes in CT scans of the brain are transitory and are caused by the high dose of prednisolone administered during induction therapy.

  • Time-course of liver fat accumulation in man after a single load of ethanol

    • Three healthy young male subjects were given a single load of ethanol (approximately 3 g/kg body weight) over 5 hours. Liver aspiration biopsies were performed at the start of the experiment and 6, 14, 24, and 48 hours after the start. Blood alcohol and serum transaminase activity levels were measured simultaneously. Triglycerides and phospholipids of the liver biopsy material were determined by a previously described method. The increase of the molar ratio triglycerides over phospholipids (TG/PL) was used to express triglyceride accumulation in the livers. At 24 hours from the start of the experiment this ratio had increased 2-5 times, and then declined to almost the initial values at 48 hours. Serum transaminases showed a slight increase over the entire experimental period. Considerable individual variation in the parameters was found. The results show that it is possible to follow time-course of rapid metabolic changes in the human liver by a combination of the fine-needle aspiration biopsy technique and biochemical micro-methods.
