  • Language impairment in Swedish bilingual children - epidemiological and linguistic studies

    • The aim of this thesis was to provide both epidemiological and linguistic data on bilingual children with language impairment (LI), since few data on this group exist. Data from 438 bilingual and monolingual children, referred over a period of 12 months, were compared in the first epidemiological study. The results showed that the bilingual children with suspected LI were referred significantly later than monolingual, and by more diverse referral sources. The risk of being discharged owing to non-attendance was significantly higher in the bilingual group, and the risk increased with increasing severity of LI. In the second epidemiological study, data from 698 assessed bilingual and monolingual children were compared. They were referred over a period of 24 months. Most risk factors for LI relative to degree of severity seemed to apply for both bilingual and monolingual children. Parental stress and short attention span were risk factors in both groups. Gender was a risk factor in the monolingual group, but did not appear as a risk factor in the bilingual group until triggered by environmental factors. Some risk factors were specific for the bilingual group, as maternal arrival within + 1 year in relation to child birth and parental need of interpreter after >5 years of residence in Sweden. In the linguistic studies, 10 Swedish-Arabic children with severe LI and 10 matched controls were tested in both languages with grammatical and phonological tests, designed to allow for comparisons between the languages. The results showed that all bilingual children followed the same developmental sequence on their way towards the target language, both cross-sectionally and longitudinally. The children with severe LI were able to develop both their languages, although at a much slower pace. They also seemed more vulnerable to a limited exposure of both Swedish and Arabic. The phonological study showed that the children with and without severe LI, tended to develop Swedish and Arabic phonology in the same way as monolingual children, with some exceptions. After approximately 1;6 years of exposure to Swedish in preschool (span 1;2-2;0), the children without LI scored at high grammatical levels in Swedish, and the number of phonological processes decreased substantially. This was not the case with most of the children with severe LI. This time limit has the potential of becoming clinically significant when assessing bilingual children with suspected LI.

  • Complexity in measuring outcomes after communication partner training: Alignment between goals of intervention and methods of evaluation

    • Background: Researchers are currently discussing the need for consensus on a core set of outcome measures to assess interventions in aphasia. For indirect, environmental approaches to aphasia intervention, such as communication partner training (CPT), the roadmap to obtaining consensus on core outcome measures seems especially complex. While the purpose of CPT is to improve communication for people with communication disorders, the intervention is aimed at the communication partner. There is also a variety of goals, activities, and possible settings for CPT. This complexity increases the risk of a mismatch between the goals and content of the intervention and measures used to evaluate the outcome.Aims: The purpose of this paper is to describe the complexity of measuring outcomes from CPT. The aim is to enable clinicians and researchers to reflect on the outcomes to be measured and also on how different types of measures may or may not be aligned with the goals and content of a specific CPT intervention.Main contribution: The current proliferation of outcome measures used in CPT is considered in the light of a survey of general factors to be considered in evaluating intervention outcomes. The complexity of measuring outcomes in CPT is illustrated and the importance of alignment of main objectives, intervention tasks, and projected outcomes is exemplified by referencing two common types of CPT approaches. Objectives relating to knowledge of aphasia, interactional behaviour, and feelings and attitudes are considered in relation to specific outcome measurements. It is suggested that both study-specific and more general measures are needed for capturing and comparing outcomes. The measurement of relevant outcome in CPT is discussed along with implications for future research and clinical practice.Conclusions: Different CPT approaches share the same purpose of facilitating communication in aphasia, but their application in research studies or in the clinic, is specific to the particular context. Special care must thus be taken in both clinical practice and research to safeguard the alignment between objectives, tasks, and projected intervention outcomes and the actual measures used. Further, it is concluded that there is a need for the development of new measures based on a consensus on key outcomes to be measured in CPT.

  • A Cross-Sectional Investigation of the Impact of Stuttering on Swedish Females and Males in Childhood, Adolescence, and Young Adulthood

    • Purpose: We aimed to cross-sectionally describe the impact of stuttering on persons who stutter (PWS): children, adolescents, and young adults. Based on previous research on PWS and psychosocial health in the general population, we hypothesized that (a) the adverse impact of stuttering in PWS would be larger among adolescents than children and young adults and that (b) females, especially adolescent females, would report being more adversely impacted by their stuttering than males.Method: We pooled samples of Swedish PWS, obtaining 162 individuals (75 females and 87 males), aged 7-30 years. We measured the impact of stuttering using age-relevant versions of the Overall Assessment of the Speaker's Experience of Stuttering (OASES). The relationship between OASES score, age, and sex was described using a polynomial model with an interaction term between age and sex to allow for potential differences between females and males' age-related curves.Results: The average trends were that (a) the impact of stuttering was greater for the adolescents than for the children and young adults, and (b) females, especially adolescent females, were on average more impacted by their stuttering than males. Taking self-reported speech fluency into account did not change this pattern.Conclusions: In line with findings on psychosocial health, communication attitude, and self-esteem in the general population, the impact of stuttering seems to be particularly adverse among adolescents, especially female adolescents. Thus, clinicians need to be aware of the risk that young girls who stutter may develop a negative attitude to speech and communication, and this should also be communicated to caregivers and teachers.

    • Background: People with dementia frequently suffer from communication disabilities, which usually influence their quality of life. The communication disabilities may affect a persons possibility to participate in interaction as a result of reduced ability to initiate new topics and difficulties in contributing new information to maintain the conversational topic. Technical aids have been proved useful to facilitate communicative activities by supporting memory and stimulating communicative initiatives. Purpose: The aim of the present study is to further understandings of how digital communication support may be used in interaction involving people with dementia. A further aim is to investigate how participants experience communication with and without the use of communication aids. Methods: The study is carried out in a Swedish context, and three dyads of older women with dementia and professional carers participated in the study. The dyads interact in the home environments of the persons with dementia using tablet computers and two web-based applications with generic pictures, videos, and music files (Computer Interactive Reminiscence and Communication Aid, CIRCA) and personalised pictures and films (Computer Interactive Reminiscence and Communication University of Sheffield, CIRCUS). The data include twenty-one video recorded activities. Results and Conclusion: The applications appear to provide support for the dyads in finding things to talk about. The participants talk both about the material and memories associated with the material. The participants experience the use of communication aids as positive.

    • A previous study has shown that Swedish children with language impairment have prosodic problems at different levels oflanguage. The aim ofthis study is to investigate prosodic problems at the discourse level in two ofthe children to see whether they are related to pragmatic problems. Prosodic and pragmatic abilities were assessed by perceptual and acoustic evaluations. The children's communication checklist (CCC) was used to evaluate the children's pragmatic abilities and analysis ofspontaneous speech samples was made. The results of the perceptual evaluations showed that the children have prosodic problems at the discourse level. The acoustic analysis also provided some support of this result. According to the CCC and the analysis ofspontaneous speech samples the children had pragmatic problems. The results support the assumption that the prosodic problems at the discourse level are related to the pragmatic problems and indicate that the prosodic problems might be secondary to the pragmatic problems. However, far-reaching generalizations can not be made from this case study of two children.

    • The purpose of the present study was to investigate the relationship between perception and production of metrical patterns in Swedish children with language impairment (LI), in order to add to the knowledge on underlying explanations of LI. A further aim was to explore whether omissions are mainly related to prosodic aspects or to linguistic function. Children with LI omitted significantly more unstressed syllables than did children with typical language development. Exploration of the relationship between perception and production of phrasal stress patterns demonstrated that children with LI might be divided into three subgroups: Group a: children who perform better on perception than production; Group b: children who perform better on production than perception; and Group c: children with rather poor results on both perception and production.
