  • Computerized profile perimetry in glaucoma

    • The new profile testing mode of the COMPETER automatic perimeter was clinically tested and compared with careful manual profile perimetry on the Tübinger perimeter. Each of 110 patients with glaucomatous field defects, patients with suspected glaucoma, and normal subjects had one meridian tested. All of the 55 patients who showed an abnormal result on the manual profile also had abnormal findings on the automatic test. Two (4%) of the profiles labeled as normal on initial manual perimetry were identified by the automatic perimeter to be abnormal, and these defects were confirmed on repeated manual profile perimetry. The automatic perimetry gave five (9%) false-positive results in the 53 normal manual profiles, thus giving a specificity of 91%. In two of these cases, the automatic profile indicated defects in areas where the manual profile appeared normal, but there was abnormality in the manual kinetic perimetry in the affected area. The automatic profiles of the COMPETER automatic perimeter are accurate, sensitive, time-saving, and clinically useful.

  • Prenatal diagnosis of hemophilia B by an immunoradiometric assay of factor IX

    • An immunoradiometric assay of factor IX was developed based on homologous antibodies that arose in a hemophilic patient. With this assay, 11 of 12 patients with severe hemophilia B had factor IX antigen levels below 1 U/dl and 6 patients with mild hemophilia B had various levels. Factor IX antigen in 8 fetuses (16th-20th gestational week) aborted for therapeutic reasons ranged from 1.8 to 10.0 U/dl. Six amniotic fluids contained 0.28-1.2 U/dl factor IX antigen. Using the immunoradiometric assay, we could diagnose hemophilia B prenatally in one fetus at risk. No factor IX antigen (< 0.2 U/dl) was detectable in the fetoscopic sample. After termination of the pregnancy, analysis of blood from the abortus confirmed the diagnosis of severe hemophilia B. We conclude that very sensitive immunologic assays, such as the one described here, will prove useful in prenatal diagnosis of severe hemophilia B, since determination of factor IX activity in fetoscopic samples is unrealiable because of possible contamination with thromboplastic material.

  • Effect of specific binding of human albumin, fibrinogen, and immunoglobulin G on surface characteristics of bacterial strains as revealed by partition experiments in polymer phase systems

    • Four strains of gram-positive cocci with different combinations of positive binding of human proteins were investigated with respect to changes in physicochemical surface properties after specific protein binding. Staphylococcus aureus Cowan I, two group A beta-hemolytic streptococci, and one group G streptococcal strain were studied; they represented three different combinations of reactivity for human serum albumin, human immunoglobulin G, and fibrinogen. Using single-tube partition of bacterial cells in a dextran-polyethylene glycol system of constant polymer concentration but varying ionic compositions, it was possible to detect changes in the partition of bacteria after specific protein binding. There was a correlation between the binding of radiolabled human proteins to the bacterial strains and the effect of human proteins on the partition of the bacteria in the phase systems. Thus, the specific binding of proteins to the bacteria changes their physicochemical surface properties. These types of bacteria-protein interactions may play an important role in modulating host-parasite relationships.

    • Using batch microcalorimeters of the thermopile conduction type, heat production was measured in human blood in the presence of defined antigens and their specific antibodies. A large increase in heat production, reflecting increased metabolic activity in the blood cells, was found when antigens and specific antisera at certain antigen-antibody proportions were mixed with the blood. The reproducibility and specificity of the method were found to be very good. The present results indicate that calorimetry may be a useful technique for detection of antigen-antibody complexes.

  • High isolation rate of Branhamella catarrhalis from the nasopharynx in adults with acute laryngitis

    • Branhamella catarrhalis was isolated from the nasopharynx in 55% of 40 noncompromised adult patients suffering from acute laryngitis. Diplococcus pneumoniae and haemophilus influenzae were found in another 5 and 8%, respectively; one patient had group A streptococci in the throat specimen. In 90% of the patients the laryngitis complaints were preceded by symptoms of an acute respiratory tract infection. Two of the patients with B. catarrhalis showed a significant titre conversion against influenza B and parainfluenza type e virus, respectively. Attempts to isolate virus failed in all cases. The results indicate that B. catarrhalis, known to cause acute otitis media in small children and respiratory tract infections in adult compromised hosts, may be involved in the etiology of acute laryngitis in otherwise healthy adults.
