  • Policy Changes in Community Mental Health : Interventions and Strategies Used in Sweden over 20 Years

    • The deinstitutionalization of mental health care has changed the responsibilities of involved authoritiesand has led to a continuous need for new treatment forms and interventions. This articledescribes this development in Europe, and in particular how these new conditions have been handledin Sweden over the past 20 years at the level of governmental policy-making. Three major policydocuments from 1994, 2009 and 2012 were included in this study. To increase our understandingof the policies’ contents, we have used theoretical concepts concerning governance,implementation and political risk management. Although our main interest was to find out howthe government handles interventions for users of the mental health care system, we found that thepolicy work is progressing stepwise. The first document, from the deinstitutionalization era, did notdiscuss interventions clearly. Instead, it was mainly concerned with both practical and economicalareas of responsibility. The second document, from the post-deinstitutionalization era, was morefocused on what services should be delivered to the users, while the most recently published documentto a greater extent addressed the question of how the support is supposed to be designed. The trendin European community mental health policy has been to advocate services in open forms that areintegrated into the society’s other care systems. This is also the case in Sweden, and continuous workis being done by the government to find strategies to support the development, and to meet the needs atboth political and local levels.

    • Objectives: To define requirements that condition trust in artificial intelligence (AI) as clinical decision support in radiology from the perspective of various stakeholders and to explore ways to fulfil these requirements.Methods: Semi-structured interviews were conducted with twenty-five respondents—nineteen directly involved in the development, implementation, or use of AI applications in radiology and six working with AI in other areas of healthcare. We designed the questions to explore three themes: development and use of AI, professional decision-making, and management and organizational procedures connected to AI. The transcribed interviews were analysed in an iterative coding process from open coding to theoretically informed thematic coding.Results: We identified four aspects of trust that relate to reliability, transparency, quality verification, and inter-organizational compatibility. These aspects fall under the categories of substantial and procedural requirements.Conclusions: Development of appropriate levels of trust in AI in healthcare is complex and encompasses multiple dimensions of requirements. Various stakeholders will have to be involved in developing AI solutions for healthcare and radiology to fulfil these requirements. Clinical relevance statement: For AI to achieve advances in radiology, it must be given the opportunity to support, rather than replace, human expertise. Support requires trust. Identification of aspects and conditions for trust allows developing AI implementation strategies that facilitate advancing the field.Key Points:• Dimensions of procedural and substantial demands that need to be fulfilled to foster appropriate levels of trust in AI in healthcare are conditioned on aspects related to reliability, transparency, quality verification, and inter-organizational compatibility.  • Creating the conditions for trust to emerge requires the involvement of various stakeholders, who will have to compensate the problem’s inherent complexity by finding and promoting well-defined solutions. © 2023, The Author(s).

    • Detta dokument är framtaget som en intern rapport inom MerSam – Meritvärde av samverkansskicklighet, varsprojekttid löpt under 2017 – 2020. MerSam är ett av Vinnovas 17 finansierade K3-projekt. Rapporten utgör ensammanställning av insamlad information från dokument, dialoger, workshoppar, intervjuer och enkäter.Rapporten visar på användning av begrepp och tolkning av begrepp. Syftet är att beskriva en nulägesbild ochutifrån den samla och lyfta fram vilka begrepp som förslagsvis kan användas i projektets slutleveranser. Slutsatser och rekommendationer har tagits fram med delaktighet av hela MerSam projektet och hardiskuterats på projektmöten, projektworkshopar den 17-18 juni 2020 samt den 15 oktober 2020.

  • The practice of risk governance: lessons from the field

    • In contradistinction to generic and formal risk governance models such as the IRGC framework, this paper advocates the relevance of a contextual and practice-based approach to organizational risk governance. Three cases illustrate the socially situated dynamics of risk governance practice: public transportation management, river management, and railway planning. Risk governance is shown to derive from how actors, in their daily activities, mediate multi-level and regulatory institutional constraints, and solve actual problems through routines, trust, mutual understanding and not least, shared commitment to the societal role of infrastructure. Our findings underscore that risk governance takes place in contexts that are historically, spatially and institutionally situated. We therefore suggest that one needs to pay attention to the characteristics of this contextuality to understand the social dynamics of governance.

Emphasis is placed on the role of thesocial sciences in elaborating on the politicisation and pluralisation of transformation processes and outcomes, helping situate, frame, reflect and generate societal action, while acknowledging the complexity of societal transformationin different contexts.

    • © 2017 Elsevier Ltd The aim of this paper is to explore, illustrate and conceptualise how the introduction of mobile phones transforms the practice of shopping. Drawing on a focus group study of young adults and making use of Practice Theory, this paper shows that the introduction of mobile phones reconfigures the practice of shopping subsequently transforming the agency of consumers. Mobile phones enable consumers to access, store, and process information in new ways; supporting new modes of social shopping, enabling consumers to change the experience of shopping, and making them better equipped economic actors with more access to financial systems and new calculative capacities. While this new agency is beneficial to consumers, it also causes them stress and anxiety.

    • Including the voices and knowledge of service users is essential for developing recovery-oriented and evidence-based mental health services. Recent studies have however, suggested that challenges remain to the legitimization of user knowledge in practice. To further explore such challenges, a co-production study was conducted by a team of researchers and representatives from user organizations in Sweden. The aim of the study was to explore the barriers and facilitators to the legitimacy of user knowledge, as a central factor in sustainably implementing user influence in mental health practice. A series of workshops, with representatives of mental health services and user organizations were conducted by the research team to explore these issues. The analysis built on the theoretical framework of epistemic injustice, and the underlying aspects, testimonial, hermeneutic and participation-based injustice, were utilized as a framework for a deductive analysis. Results suggest that this is a useful model for exploring the complex dynamics related to the legitimacy of user knowledge in mental health systems. The analysis suggests that the legitimacy of user knowledge is related to the representativeness of the knowledge base, the systematic formulation of this knowledge inapplicable methods, access to resources and positions within the mental health system and participation in the process of integrating this knowledge-base in mental health contexts. Legitimizing user knowledge in practice additionally challenges mental health systems to support readiness for change in working environments and to address the power and role issues that these changes involve.     • Tourism in the wake of films, literature, and music is gaining interest among academics and practitioners alike. Despite the significance of converging tourism and media production and popcultural consumption, theorizing in this field is weak. This chapter explores complex relationships among popcultural phenomena, destination image creation, and tourism consumption. By taking a broader social science approach, it revisits and connects research themes, such as symbolic consumption, negotiated representations, fans and fandom, technology mediation, and media convergence. The chapter concludes with an integrative model, or "popcultural placemaking loop," which is qualified through six propositions. Copyright © 2015 by Emerald Group Publishing Limited.
