  • Chronic Pain in Older Adults: A Controlled Pilot Trial of a Brief Cognitive-Behavioural Group Treatment

    • Background: Chronic pain is a common condition among older adults. While cognitive behaviour therapy (CBT) has been tested in numerous studies on adults and children there are fewer studies on older persons. The objective of this study was to investigate the effects of a six-session CBT group treatment for older persons with chronic pain. As a secondary aim we investigated whether treatment credibility was associated with outcome. Method: We included 21 persons (mean age = 72.0 years) who were randomly allocated to either a waitlist condition or treatment consisting of applied relaxation, with the addition of problem solving, assertiveness, communication strategies, sleep management, and relapse prevention. Results: Few statistically significant effects were found on measures of pain, mood, anxiety, and quality of life; however, a significant treatment effect with a between group effect size of d = 1.0 was observed with respect to perceived ability to function despite the discomfort of pain. Conclusion: The study provides some preliminary support for the use of group-based CBT with a focus on applied relaxation for older adults with chronic pain.

    • The internet is no longer something new. It has existed now for a long time and has been part of many peoples' lives for at least 20 years. The internet increasingly informs many aspects of our lives, including the economy, health care, and delivery of psychological treatments. There are many areas in which the internet has had an impact on cognitive behavioral therapy (CBT). This chapter describes challenges when delivering CBT via the internet, comments on assumptions behind treatment programs, and describes the main approaches, outcomes, and potential mechanisms of change. Further, the authors comment on the dissemination of internet-delivered CBT (ICBT), as well as applications for diverse target groups, and possible future developments. Although ICBT may appear as something new, it is almost as old as the internet itself, and the research field is very large, with more than 200 controlled trials.

    • Introduction: Mild to moderate depression has been successfully treated with cognitive-behavioural (CBT) bibliotherapy, including minimal therapist contact. More recently, the Internet has been used to deliver the treatment, with obvious gains in terms of cost reduction and increased accessibility. In the present study we analysed pre-treatment predictors of improvement following Internet-based self-help treatment of mild to moderate depression. Patients and Methods: Included were 71 participants from a randomised trial who completed a 6-month follow-up. Change indexes were calculated from the Beck Depression Inventory (BDI) and the Montgomery Åsberg Depression Rating Scale (MADRS). Results: In line with the literature on depression, the number of previous episodes of depression was negatively associated with improvement after treatment. Follow-up scores on the BDI and MADRS were associated with pre-treatment levels of depression, anxiety and low levels of quality of life. Discussion: As indicated by traditional psychotherapy studies, finding predictors of outcome is a difficult task. Patients with repeated episodes of depression might benefit less from self-help over the Internet, but as the correlation is weak, no firm conclusions can be drawn.

  • Effects of Two Forms of Internet-Delivered Cognitive Behaviour Therapy for Depression on Future Thinking

    • The aim of this study was to investigate if future thinking would change following two forms of Internet-delivered cognitive behavior therapy (ICBT) for major depression. A second aim was to study the association between pre-post changes in future thinking and pre-post changes in depressive symptoms. Effects of psychological treatments are most often tested with self-report inventories and seldom with tests of cognitive function. We included data from 47 persons diagnosed with major depression who received either e-mail therapy or guided self-help during 8 weeks. Participants completed a future thinking task (FTT), in which they were asked to generate positive and negative events that they thought were going to happen in the future and rated the events in terms of emotion and likelihood. The FTT was completed before and after treatment. Data on depressive symptoms were also collected. FTT index scores for negative events were reduced after treatment. There was no increase for the positive events. Change scores for the FTT negative events and depression symptoms were significantly correlated. We conclude that ICBT may lead to decreased negative future thinking and that changes in depression symptoms correlate to some extent with reductions in negative future thinking.

  • Enduring effects of ICBT

    • Introduction: Numerous randomized controlled trials have been conducted on internet interventions. In addition to the effects observed in these trials immediately after treatment there are several long-term follow-ups. The aim of this talk is to review the long-term effects of internet-delivered CBT (ICBT) with a focus on results at 1-year or later following treatment termination.Methods: We were able to locate examples of enduring effects for a range of conditions including mood and anxiety disorders and somatic disorders. The longest follow-up period has been five years.Results: Large within-group effects have been documented in most trials, with effects sizes being moderate to large for anxiety and depression studies.Discussion: Studies have failed to document how much the treatment is used during the follow-up period and in the case of depression it is unclear if episodes of depression have occured during the period covered. We conclude that the effects of ICBT appear to be enduring but that more research is needed.

    • Numerous controlled trials have been published on the effects of internet-delivered psychological treatments for a range of problems and disorders. Generally, trials adhere to the CONSORT statement and include control groups. Often this is attention control, waitlist but also alternative treatments. In experimental psychology factorial designs is the common way to investigate research questions but in psychotherapy research this is rare given the need for large samples in order to have sufficient power to detect differntial effects of  independent variables (like for example different versions of a treatment). With the advent of internet-delivered CBT (ICBT) this has changes and it is now possible to run trials with larger samples. At the same time there is really no need for more studies showing that a treatment is better than just waiting (for some areas at least like depression). In this talk we will present result from three completed factorial design trials in which we have manipulated support form (on demand versus scheduled in one trial and chat-support versus just email in another), and also other aspects like learning support and choice of treatment. The talk will end with a discussion on future directions of ICBT research with regards to design of trials.
