  • Long-term outcome of Internet-delivered cognitive-behavioural therapy for social phobia : a 30-month follow-up

    • Internet-delivered guided cognitive behaviour therapy for social anxiety disorder has been found to generate promising short-term results, up to one year posttreatment. No study has however documented longer follow-up periods. In this 30-month follow-up we contacted 57 participants from the original study of which 77.2% (44/57) responded to the Internet-administered outcome measures and 66.7% (38/57) completed a telephone interview. Results showed large pretreatment to follow-up within-group effect sizes for the primary outcome measures (Cohen's d 1.10-1.71), and a majority (68.4%; 26/38) reported improvements in the interview. The findings suggest that the long-term effects seen in previous live treatment CBT trials can occur in Internet-delivered treatment as well.

  • Guided and unguided self-help for social anxiety disorder : randomised controlled trial

    • Background Internet-delivered self-help programmes with added guidance have shown efficacy in social anxiety disorder, unguided self-help has been insufficiently studied. Aims To evaluate the efficacy of guided and unguided self-help social anxiety disorder. Method Participants followed a cognitive-behavioural self-help programme in the form of either pure bibliotherapy or an internet-based treatment with therapist guidance and online group discussions. A subsequent trial was conducted to evaluate treatment specificity. Participants (n=235) were randomised to one of three conditions in the first trial, or one of four conditions in the second. Results Pure bibliotherapy and the internet-based treatment were better than waiting list on measures of social anxiety, general anxiety, depression and quality of life. The internet-based therapy had the highest effect sizes, but directly comparable effects were noted for bibliotherapy augmented with online group discussions. Gains were well maintained a year later. Conclusions Unguided self-help through bibliotherapy can produce enduring improvement for individuals with social anxiety disorder.

  • Emotional Stroop via Internet among individuals with eating disorders

    • The present study investigated whether Stroop interference in eating disordered individuals for food - and body-related words, as repeatedly found previously using standard emotional Stroop tasks, would also be demonstrated when using an Internet based emotional Stroop task. Participants were anorexic women (n = 13), bulimic women (n = 20), non-clinically eating disordered women nevertheless over-concerned about eating and body appearance (n = 27) and normal control women (n = 31). Bulimic individuals showed Stroop interference for body-related words whereas anorexic individuals showed Stroop interference for food-related words. The present results thus suggest that administration of the emotional Stroop task is possible via the Internet. Furthermore, it is possible that the time consuming response delivery, relative to previous studies, could lead to Stroop interference only for the most emotionally significant information, in turn, differentiating between the core concerns of anorexic and bulimic individuals.

  • Web-based measurement : Effect of completing single or multiple items per webpage

    • The current study was conducted to determine whether participants respond differently to online questionnaires presenting all items on a single webpage versus questionnaires presenting only one item per page, and whether participants prefer one format over the other. Of participants seeking self-help treatment on the Internet (for depression, social phobia, or panic disorder), 710 completed four questionnaires (Beck Depression Inventory, Beck Anxiety Inventory, Quality of Life Index, Montgomery-Åsberg Depression Rating Scale) on the Internet on two occasions. The questionnaires were either presented with one questionnaire on one webpage (e.g., BDI on one webpage) or on multiple webpages (e.g., BDI on 21 webpages with one item each). Results suggest that the four web questionnaires measure the same construct across diagnostic group (depression, social phobia, panic), presentation type (single versus multiple items per page), and order of presentation (which format first). Within each diagnostic group, factor means for all questionnaires were equivalent across presentation method and time. Furthermore, factor means varied as expected across samples (e.g., depressed group scored higher on BDI), providing evidence of construct validity. The majority of participants in each diagnostic group preferred the single item per page format, even though this format required more time.

  • Treating university students with social phobia and public speaking fears : Internet delivered self-help with or without live group exposure sessions

    • BACKGROUND: This study investigated the efficacy of an Internet-based self-help program with minimal therapist contact via e-mail for Swedish university students with social phobia and public speaking fears. The main objective was to test if the Internet-based self-help program would be more effective if five live group exposure sessions were added. METHODS: Thirty-eight students meeting the diagnostic and statistical manual of mental disorders, 4th edition criteria for social phobia were randomized into two different treatment groups: Internet delivered cognitive behavior therapy combined with five group exposure sessions (ICBT+ exp) or the Internet program alone (ICBT). RESULTS: Results were analyzed on an intention-to-treat basis. Both treatment groups showed significant improvement from pre- to post-test, and from pre-test to 1-year follow-up, on all measured dimensions (social anxiety, general anxiety, depression levels, and quality of life). For both the groups, the average within-group effect sizes for the primary social anxiety scales, expressed as Cohen's d, were comparable to those seen in traditionally administered cognitive behavioral therapy both at post-test and at 1- year follow-up. CONCLUSIONS: The results suggest that the Internet-based self-help program on its own is efficient in the treatment of university students with social phobia. Adding group exposure sessions did not improve the outcome significantly.
