  • How do people in the early stage of Alzheimer's disease see their future?

    • Older people fear Alzheimer's disease. Central to the fear of the disease is the dread of the loss of identity or self. The aim of this study is to investigate the thoughts people in an early stage of Alzheimer's disease have about their future selves, and the consequences these thoughts have for their temporary lives. The concepts of future and self are understood in terms of the concepts 'possible selves' and 'selfhood'. The participants in the study are two men in their early 60 s. The data consist of four individual interviews and 11 facilitated family conversations including two of their next of kin. The data were collected over a two-year period. The data were interpreted using a hermeneutic analysis inspired by Gadamer. The main findings were the efforts made by the participants to live in the present and their worries about a future as 'living dead'.

    • In this paper, a phenomenological hermeneutical method for interpreting narrativetexts is proposed. Interviewees are asked to tell from their lived experience of participatingin life world phenomena. The interview texts are written down and analysed,inspired by the theory of interpretation of Paul Ricoeur, in three steps: a first,naïve reading, structural analysis and comprehensive understanding. The methodwas presented the first time in 2004. In this paper, the theory behind it is elaborated.Basic concepts like phenomenon, meaning, life world, lived experience and concretereflection are clarified, and the historical background of the method and its ontology,epistemology and methodology are explained.

  • Development and psychometric properties of the Inner Strength Scale

    • BACKGROUND: Four dimensions of inner strength were previously identified in a meta-theoretical analysis: firmness, creativity, connectedness, and flexibility.OBJECTIVE: The aim of this study was to develop an Inner Strength Scale (ISS) based on those four dimensions and to evaluate its psychometric properties.METHOD: An initial version of ISS was distributed for validation purpose with the Rosenberg Self-Esteem Scale, the resilience scale, and the sense of Coherence Scale. A convenience sample of 391 adults, aged 19-90 years participated. Principal component analysis (PCA) and confirmatory factor analysis (CFA) were used in the process of exploring, evaluating, and reducing the 63-item ISS to the 20-item ISS. Cronbach's alpha and test-retest were used to measure reliability.RESULTS: CFA showed satisfactory goodness-of-fit for the 20-item ISS. The analysis supported a fourfactor solution explaining 51% of the variance. Cronbach's alpha on the 20-item ISS was 0.86, and the test-retest showed stability over time (r=0.79).CONCLUSION: The ISS was found to be a valid and reliable instrument for capturing a multifaceted understanding of inner strength. Further tests of psychometric properties of the ISS will be performed in forthcoming studies.

    • Persons with advanced dementia disease (ADD), here labelled PADDs, are shown to preserve parts of their self, which hasopened up possibilities for involving them in their own care and establishing strategies for improving their communicationwith the surrounding world. Using the well-known theory of human development proposed by Erik and Joan Erikson, herelabelled EJET, can operate as an efficient structure for formal caregivers to support PADDs in reclaiming their space.However, very few studies have used EJET as a framework in improving formal caregivers’ competence and the quality ofcare. This article aims to demonstrate the feasibility of applying EJET in the care of PADDs through two examples of successfulapplication of this framework in interacting with PADDs in residential care settings. The examples demonstrate the significanceof competent caregivers and the important role of tailoring nursing care plans to specific situations of the PADDs in theirpresent and previous developmental phases.

  • Perceptions and stress of conscience in relation to burnout among nursing staff in older people care settings : a cross sectional study

    • Background: Considering cultural influences, it is important to study the perceptions and stress of conscience in different contexts. This study aimed to investigate the association between perceptions of conscience, stress of conscience, and burnout among nursing staff working in older people care settings in Tehran.Methods: This was a descriptive, cross-sectional study. A total of 161 participants completed the Perceptions of Conscience Questionnaire, Stress of Conscience Questionnaire, and Oldenburg Burnout Inventory, 2019. All nursing staff working at the 20 contacted nursing homes agreed to participate in the study. The descriptive and inferential analysis was conducted through SPSS, using T-tests and one-way between-groups analysis of variance, Chi-square and t-tests, Cohen's d (d), Eta-squared (η2), and Phi coefficient (φ), Partial least squares regression (PLSR), jackknife approximate t-tests of the regression coefficients, and jackknife 95% confidence intervals of the regression coefficients.Results: The nursing staff perceived their conscience mainly as an authority, asset, and warning signal. Impact of workload on family life was the most common source of stress for the nursing staff. Dealing with incompatible demands, the impact of workload on family life, witnessing insulated patients, inability to meet one’s standards in providing care, and perception of conscience as a burden were strongly associated with the burnout.Conclusions: Perceiving conscience as a warning signal or authority may serve as a buffer against burnout among nursing staff. This study highlights the need for further exploration of perceptions of conscience in different cultural and social backgrounds.

    • Different concepts have been presented which denote driving forces and strengths that contribute to a person’s ability to meet and handle adversities, and keep or regain health. The aim of this study, which is a part of The Umea 85+ study, was to describe resilience, sense of coherence, purpose in life and self-transcendence in relation to perceived physical and mental health in a sample of the oldest old. The study sample consisted of 125 participants 85 years of age or older, who ranked themselves on the Resilience Scale, Sense of Coherence Scale, Purpose in life Scale and Self-Transcendence Scale and answered the SF-36 Health Survey questionnaire. The findings showed significant correlations between scores on the Resilience Scale, the Sense of Coherence Scale, the Purpose in Life Test, and the Self-Transcendence Scale. Significant correlations were also found between these scales and the SF-36 Mental Health Summary among women but not among men. There was no significant correlation between perceived physical and mental health. The mean values of the different scales showed that the oldest old have the same or higher scores than younger age groups. Regression analyses also revealed sex differences regarding mental health. The conclusions are that, the correlation between scores on the different scales suggests that the scales measure some dimension of inner strength and that the oldest old have this strength at least in the same extent as younger adults. Another conclusion is that the dimensions that constitute mental health differ between women and men.

    • The purpose of this study was to gather men's descriptions of their experience of nonmalignant pain of fibromyalgia (FM) type in order to to gain a deeper understanding of this phenomenon. Fourteen men participated in the study. A qualitative method with narrative interviews and content analysis were used. The findings showed that the men's experience of pain fell within two major themes: "perceptions of diversified bodily pain" and "perceptions of fluctuating pain". The pain had progressed from being local to being widespread. Characteristic was that the men described both pain that worried them and pain that did not worry them and also that the pain passed through both calm and difficult phases. This study highlights the importance of acknowledgement that men had specific experiences and used metaphorical expressions to make the pain visible. The clinical indication in this study is that health care staff need to listen intently to each man's experience of long-term pain that involve a great variety of both bodily and emotional symptoms. These findings are a crucial prerequisite for the health care staff's possibility to offer support and relief to men with pain of FM type.

    • Probands (n = 3607) living in long-term care institutions in the county of Västerbotten were assessed to estimate whether the prevalence of dementia in different types of institutions had changed since a similar survey was made 7 years before. The probands' motoric functions, vision, hearing, speech, prevalence of behavioral disturbances and psychiatric symptoms, work load and use of psychoactive drugs were also investigated with special emphasis on the differences between the demented and the non-demented. About 40% were demented. This proportion of demented patients had increased in somatic long-stay clinics, nursing homes and homes for the aged since 1975. Further, the mean age of the probands and their length of stay had increased in these institutions. Demented probands were more impaired with regard to motor functions, speech, vision, hearing, ADL-functions, behavioral and psychiatric symptoms, and they also imposed a higher work load on the staff than the non-demented. Demented probands were also prescribed psychoactive drugs, i.e. neuroleptics but not minor tranquilizers or antidepressants, more often than the non-demented in accordance with the increased prevalence of behavioral disturbances and psychiatric symptoms.
