  • Specialisation and specialist education in prosthetic dentistry in Europe

    • This presentation reports on the results of a meeting of prosthodontists from selected European countries. The aim of the meeting was to analyse and promote specialisation and specialist education in Prosthetic Dentistry in Europe. Representatives for Europe were selected from the European Prosthodontic Association (EPA) board, the Education and Research Committee of International College of Prosthodontists (ICP), countries with a legally recognised speciality, countries without a recognised speciality but organised training programmes and countries with neither of these situations. Data about specialisation and specialist training in Prosthodontics in Europe was scrutinised and discussed. The programmes for countries with specialist training had relatively similar content, mostly of three years duration. There was strong agreement that a recognised speciality raises the level of care within the discipline for both specialists and non-specialists. In several of the countries where a speciality had been introduced it had been initiated by pressure from public health planning authorities. The conclusions are that from a professional viewpoint an advancement of the speciality over Europe would develop the discipline, improve oral health planning and quality of patient care. A working group for harmonisation was recommended.

    • This article examines the ways in which the best interest of the child has been used as an argument for state-authorized assessments of persons who are aspiring to parenthood, but who are not yet parents, i.e. of parent potential rather than parental performance. The policies included in the analysis concern three different areas in which assessment of parental potential is made: adoption, assisted reproduction and presumptive parents with intellectual disabilities. The status of the best interest of the child as an argument for state-authorized assessments of parent potential, I argue, varies with the amount of involvement from state authorities that is needed in the process of creating a family. The state claims the right to assess the parent potential of individuals only when it contributes to the creation of families in which there are no or only partial biogenetic links between parents and child. This does not mean that the state does not aim to encourage women who belong to what is perceived as a risk group to refrain from having children. The argument used in this effort, however, is not the best interest of the child, but the best interest of the woman herself.

    • År 1942 hade 99,99 procent av svenskarna karies. Den nyöppnade folktandvården dignade under arbetsbördan att laga och dra ut svenska folkets illa anfrätta tänder.I gränslandet mellan forskning, vård och politik genomförde Medicinalstyrelsen på regeringens uppdrag en utredning om hur folksjukdomen skulle kunna stoppas. Strax utanför Lund, på Vipeholms statliga sjukhus för obildbara sinnesslöa, undersökte ett mångvetenskapligt forskarlag mellan 1945 och 1955 vad som orsakade karies. Sjukhusets patienter blev föremål för de ömsom förebyggande och ömsom kariesprovocerande experimenten. Vitaminer, mineraler, kolor och choklad skänktes av en industri med krav på inflytande på forskningen.I denna doktorsavhandling undersöker Elin Bommenel forskningspraktiken som var en del av 1940- och 50-talens politisk-vetenskapliga folkhälsoforskning och som kunde producera politiskt användbara resultat. Hon följer forskningsprocessen på Vipeholm från planeringen 1943 till Medicinalstyrelsens stora upplysningskampanj 1957 och en statlig offentlig utredning som 1960 skulle omdefiniera folktandvårdens mål från det lagande till det förebyggande mål den har än i dag.

  • Surgical repositioning of intraocular lenses after late dislocation: Complications, effect on intraocular pressure, and visual outcomes

    • PURPOSE: To study outcomes after surgery for late intraocular lens (IOL) dislocation and, more specifically, to evaluate different surgical techniques to find predictors of worse visual outcomes, describe postoperative complications, and analyze the effect on intraocular pressure (IOP). SETTING: Department of Ophthalmology, Sahlgrenska University Hospital, Molndal, Sweden. METHODS: Medical records from cataract surgery, IOL repositioning, and follow-up examinations were reviewed. RESULTS: Ninety-one eyes with in-the-bag (80) or out-of-the-bag (11) late IOL dislocation were consecutively included. In 94% of eyes, the IOLs were repositioned using scleral sutures; 76% of cases were operated on with a posterior approach, including pars plana vitrectomy. The median follow-up was 17 months. Pseudoexfoliation was detected in 57% of eyes. A significant decrease in IOP (mean 3.0 mm Hg) from preoperative values (P=.028) was seen in glaucoma patients. Thirteen eyes had additional surgical procedures. Three cases of retinal detachment occurred. Of the eyes, 59% obtained a Snellen corrected distance visual acuity (CDVA) of 0.5 or more at followup; 23% of eyes had worse CDVA during the follow-up than preoperatively. CONCLUSIONS: Repositioning surgery for late IOL dislocation with a posterior pars plana approach using scleral suturing of the preexisting IOL appears to be a safe and effective method for restoring visual acuity. Postoperative complications were comparable to previous findings in this field. Patients with glaucoma may have improved IOP regulation. (C) 2013 ASCRS and ESCRS

    • The transverse momentum (pT) distribution of primary charged particles is measured in minimum bias (non-single-diffractive) p + Pb collisions at root(NN)-N-s = 5.02 TeV with the ALICE detector at the LHC. The pT spectra measured near central rapidity in the range 0.5< p(T) < 20 GeV/c exhibit a weak pseudorapidity dependence. The nuclear modification factor R-pPb is consistent with unity for p(T) above 2 GeV/c. This measurement indicates that the strong suppression of hadron production at high p(T) observed in Pb + Pb collisions at the LHC is not due to an initial-state effect.
