  • Evaluating a questionnaire to measure improvement initiatives in Swedish healthcare

    • Background: Quality improvement initiatives have expanded recently within the healthcare sector. Studies have shown that less than 40% of these initiatives are successful, indicating the need for an instrument that can measure the progress and results of quality improvement initiatives and answer questions about how quality initiatives are conducted. The aim of the present study was to develop and test an instrument to measure improvement process and outcome in Swedish healthcare. Methods: A questionnaire, founded on the Minnesota Innovation Survey (MIS), was developed in several steps. Items were merged and answer alternatives were revised. Employees participating in a county council improvement program received the web-based questionnaire. Data was analysed by descriptive statistics and correlation analysis. The questionnaire psychometric properties were investigated and an exploratory factor analysis was conducted. Results: The Swedish Improvement Measurement Questionnaire consists of 27 items. The Improvement Effectiveness Outcome dimension consists of three items and has a Cronbach’s alpha coefficient of 0.67. The Internal Improvement Processes dimension consists of eight sub-dimensions with a total of 24 items. Cronbach’s alpha coefficient for the complete dimension was 0.72. Three significant item correlations were found. A large involvement in the improvement initiative was shown and the majority of the respondents were satisfied with their work. Conclusions: The psychometric property tests suggest initial support for the questionnaire to study and evaluate quality improvement initiatives in Swedish healthcare settings. The overall satisfaction with the quality improvement initiative correlates positively to the awareness of individual responsibilities.

    • I suggest an electrostatic phase plate designed to broaden the contrast transfer function of a transmission electron microscope operated close to Scherzer defocus primarily in the low resolution direction. At higher defocus the low frequency behavior is equal to that close to Scherzer defocus, but CTF-correction becomes necessary to extend image interpretation to higher resolution. One simple realization of the phase plate consists of two ring shaped electrodes symmetrically surrounding the central beam. Since no physical components come into contact with the central beam and charge on the electrodes is controlled by an external voltage supply, problems with uncontrolled charging are expected to be reduced.

    • This case study investigates whether collective entrepreneurial team cognition remains consistent throughout all stage processes when setting up a large-scale, distributed research infrastructure. A new “action phase model” has been devised, known as the “4 I’s of entrepreneurship”, with each “I” elucidating the entrepreneurial rationale behind various stages of the creation process:Intention, Initiation, ImplementationandIntrospection. The case investigated was BBMRI.se (BioBanking and Molecular Resource Infrastructure in Sweden) – a Swedish large-scale distributed research infrastructure aimed at harmonizing biobanking standards. Managers and key personnel involved in founding and/or operating the organization were interviewed. The results showed there was agreement regarding the need for the research infrastructure, while there were disagreeing perceptions of what the organization should be doing and regarding the difficulties it had faced. These developments would ultimately lead to BBMRI.se’s demise. The homogenous mindset would begin to dissipate once theInitiationstage was reached and worsened throughout theImplementationstage. The results indicate that BBMRI.se’s managerial structure, personal ambitions and lack of transparency and communication were key contributors to its ultimate failure. The implications highlight the risk of fragmented motivations as collective entrepreneurs turn their ambition into reality, if given too much autonomy.

  • Understanding Salutogenic Approaches to Managing Intensive Work : Experiences from Three Swedish Companies

    • BACKGROUND: Recent research has highlighted the risks involved in work intensification; i.e. the risk of human resources being consumed, incurring higher risks of emotional ill health such as burnout among employees. At the same time, there are some indications that individual employees are being left to themselves to manage work tasks in intensive work environments.OBJECTIVE: This article explores how strongly engaged people master intensive work through coping and job crafting in a salutogenic way.METHODS: The article is based on 34 in depth interviews of persons in various professions within research and development departments in three Swedish companies.RESULTS: The interviews indicated that employees generally experienced work as almost constantly intense. We identified two strategies for mastering intensive work; i.e. an active strategy and a cognitive strategy, involving elements of both coping and job crafting. The specific elements of these strategies determine whether salutogenic processes are present.CONCLUSIONS: The results suggest that it is the specific elements of the coping and job-crafting strategies that determine whether or not salutogenic processes occur, and that organisational support is an important supplement to the individual processes for mastering intensive work.

  • Detecting driver sleeepiness using optimized non-linear combinations of sleepiness indicators

    • This paper addresses the problem of detecting sleepiness in car drivers. First, a variety of sleepiness indicators (based on driving behavior) proposed in the literature were evaluated. These indicators were then subjected to parametric optimization using stochastic optimization methods. To improve performance, the functional form of some of the indicators was generalized before optimization. Next, using a neural network, the best performing sleepiness indicators were combined with a mathematical model of sleepiness, i.e., the sleep/wake predictor (SWP). The analyses were based on data obtained from a study that involved 12 test subjects at the moving-base driving simulator at the Swedish National Road and Transportation Research Institute (VTI), Linkping, Sweden. The data were derived from 12 1-h driving sessions for each test subject, with varying degrees of sleepiness. The performance measure (range [0,1]) for indicators was taken as the average of sensitivity and specificity. Starting with indicators proposed in the literature, the best such indicator, i.e., the standard deviation of the yaw angle, reached a performance score of 0.72 on previously unseen test data. It was found that indicators based on a given signal gave essentially equal performance after parametric optimization, but in no case was it better than 0.72. The best generalized indicator (the generic variability indicator) obtained a performance score of 0.74. SWP achieved a score of 0.78. However, by nonlinearly combining SWP with the generic variability indicator, a score of 0.83 was obtained. Thus, the results imply that a nonlinear combination of a measure based on driving behavior with a model of sleepiness significantly improves driver sleepiness detection.

  • Kinematics of Child Volunteers and Child Anthropomorphic Test Devices During Emergency Braking Events in Real Car Environment

    • Objectives: The objective of this study was to present, compare, and discuss the kinematic response of children and child anthropomorphic test devices (ATDs) during emergency braking events in different restraint configurations in a passenger vehicle.Methods: A driving study was conducted on a closed-circuit test track comprising 16 children aged 4 to 12 years old and the Q3, Hybrid III (HIII) 3-year-old, 6-year-old, and 10-year-old ATDs restrained on the right rear seat of a modern passenger vehicle. The children were exposed to one braking event in each of the 2 restraint systems and the ATDs were exposed to 2 braking events in each restraint system. All events had a deceleration of 1.0 g. Short children (stature 107–123 cm) and the Q3, HIII 3-year-old,and 6-year-old were restrained on booster cushions as well as high-back booster seats. Tall children (stature 135–150 cm) and HIII 10-year-old were restrained on booster cushions or restrained by 3-point belts directly on the car seat. Vehicle data were collected and synchronized with video data. Forward trajectories for the forehead and external auditory canal (ear) were determined as well ashead rotation and shoulder belt force.Results: A total of 40 trials were analyzed. Child volunteers had greater maximum forward displacement of the head and greater head rotation compared to the ATDs. The average maximum displacement for children ranged from 165 to 210 mm and 155 to 195 mm for the forehead and ear target, respectively. Corresponding values for the ATDs were 55 to 165 mm and 50 to 160 mm. The change in head angle was greater for short children than for tall children. Shoulder belt force was within the same range for short children when restrained on booster cushions or high-back booster seats. For tall children, the shoulder belt force was greater when restrained on booster cushions compared to being restrained by seat belts directly on the car seat.Conclusions: The forward displacement was within the same range for all children regardless of stature and restraint system. However, themaximum forward position depended on the initial seated posture and shoulder belt position on the shoulder. Differences could also be seen in the curvature of the neck and spine. Short children exhibited a greater flexion motion of the head, whereas a more upright posture at maximumforward position was exhibited by the tall children. The ATDs displayed less forward displacement compared to the children.

    • Work related musculoskeletal disorders (WMSDs) in the shoulder/neck area are a common and increasing problem among European computer workers, especially women. Long-term low-level workloads with low degree of muscle rest are a potential risk factor for developing WMSDs. The purpose of the present study of female computer users (age 45-65 years) in Denmark and Sweden was to investigate if subjects with self-reported neck/shoulder complaints (cases, N = 35) show less trapezius muscle relative rest time (RRT) than controls (N = 44) when performing standardised short-term computer work tasks in controlled laboratory conditions. Surface electromyography (EMG) signals were recorded bilaterally from the upper trapezius muscles during a type, edit, precision and colour word stress task. Besides RRT, 10th percentile RMS values were calculated. On the average, 15 of the cases and 18 of the controls showed analysable EMG files per task. For the colour word stress task, the results showed lower RRT values and higher 10th percentile RMS amplitude levels among cases compared to controls. No such signs could be found for the other tasks performed. The present results indicate an increased motor response to a psychological stressor among subjects with self-reported neck/shoulder complaints. (c) 2006 Elsevier Ltd. All rights reserved.

    • Sorption (i.e., adsorption and absorption) of small-molecule compounds to polydimethylsiloxane (PDMS) is a widely acknowledged phenomenon. However, studies to date have largely been conducted under atypical conditions for microfluidic applications (lack of perfusion, lack of biological fluids, etc.), especially considering biological studies such as organs-on-chips where small-molecule sorption poses the largest concern. Here, we present an in-depth study of small-molecule sorption under relevant conditions for microphysiological systems, focusing on a standard geometry for biological barrier studies that find application in pharmacokinetics. We specifically assess the sorption of a broad compound panel including 15 neuropsychopharmaca at in vivo concentration levels. We consider devices constructed from PDMS as well as two material alternatives (off-stoichiometry thiol–ene–epoxy, or tape/polycarbonate laminates). Moreover, we study the much neglected impact of peristaltic pump tubing, an essential component of the recirculating systems required to achieve in vivo-like perfusion shear stresses. We find that the choice of the device material does not have a significant impact on the sorption behavior in our barrier-on-chip-type system. Our PDMS observations in particular suggest that excessive compound sorption observed in prior studies is not sufficiently described by compound hydrophobicity or other suggested predictors. Critically, we show that sorption by peristaltic tubing, including the commonly utilized PharMed BPT, dominates over device sorption even on an area-normalized basis, let alone at the typically much larger tubing surface areas. Our findings highlight the importance of validating compound dosages in organ-on-chip studies, as well as the need for considering tubing materials with equal or higher care than device materials.

    • Work-related musculoskeletal and cardiovascular disorders are still prevalent in today's working population. Nowadays, risk assessments are usually performed via self-reports or observations, which have relatively low reliability. Technology developments in textile electrodes (textrodes), inertial measurement units, and the communication and processing capabilities of smart phones/tablets provide wearable solutions that enable continuous measurements of physiological and musculoskeletal loads at work with sufficient reliability and resource efficiency. In this paper, a wearable system integrating textrodes, motion sensors, and real-time data processing through a mobile application was developed as a demonstrator of risk assessment related to different types and levels of workload and activities. The system was demonstrated in eight subjects from four occupations with various workload intensities, during which the heart rate and leg motion data were collected and analyzed with real-time risk assessment and feedback. The system showed good functionality and usability as a risk assessment tool. The results contribute to designing and developing future wearable systems and bring new solutions for the prevention of work-related disorders.
