    • Universities and colleges are often regarded as playing a key role in educating veterinarians and animal health workers who advise farmers on herd health and animal husbandry. However, to date, studies examining veterinary students' knowledge of zoonotic diseases of public health importance and the source of this knowledge, as well as their preparedness to respond to these diseases, have focused on the Global North rather than the Global South. This study takes Ethiopia as a case study in exploring veterinary medicine students' knowledge of zoonosis risks, infection control practices and biosecurity measures, recognizing that it is imperative to reconcile national-level veterinary education curricula with emerging global trends, such as One Health-focused training. This training advocates for a collaborative, interdisciplinary response at local, national, and international levels to the adverse impact of zoonotic diseases on animal health and productivity, and human and environmental health. Data for this study were collected through a pre-tested online questionnaire administered to 154 veterinary students from several universities in Ethiopia. The findings of this study suggest veterinary students were aware of the public health risks posed by zoonoses and the important role that collaboration between the disciplines of human and animal health can play in addressing zoonoses and emerging health risks. However, students demonstrated poor knowledge of the need to adopt infection control measures (ICPs) and biosecurity measures to reduce occupational risks and prevent within and between herd transmission of infection. Moreover, students' vaccination rates against zoonotic diseases associated with occupational risks, such as rabies, were low. The results of this study suggest that there are currently gaps in Ethiopia's veterinary curriculum and that enhancing veterinary students' access to information regarding infection control practices and biosecurity measures could contribute to reducing their future occupational exposure to zoonoses. This study highlights the policy implications of the current veterinary medicine curriculum in Ethiopia and the scope for aligning the curriculum with important global initiatives, such as One Health-focused training.

  • Understanding Antibiotic Usage on Small-Scale Dairy Farms in the Indian States of Assam and Haryana Using a Mixed-Methods Approach—Outcomes and Challenges

    • The use and misuse of antibiotics in both humans and animals contributes to the global emergence of antimicrobial resistant (AMR) bacteria, a threat to public health and infection control. Currently, India is the world’s leading milk producer but antibiotic usage within the dairy sector is poorly regulated. Little data exists reflecting how antibiotics are used on dairy farms, especially on small-scale dairy farms in India. To address this lack of data, a study was carried out on 491 small-scale dairy farms in two Indian states, Assam and Haryana, using a mixed method approach where farmers were interviewed, farms inspected for the presence of antibiotics and milk samples taken to determine antibiotic usage. Usage of antibiotics on farms appeared low only 10% (95% CI 8–13%) of farmers surveyed confirmed using antibiotics in their dairy herds during the last 12 months. Of the farms surveyed, only 8% (6–11%) had milk samples positive for antibiotic residues, namely from the novobiocin, macrolides, and sulphonamide classes of antibiotics. Of the farmers surveyed, only 2% (0.8–3%) had heard of the term “withdrawal period” and 53% (40–65%) failed to describe the term “antibiotic”. While this study clearly highlights a lack of understanding of antibiotics among small-scale dairy farmers, a potential factor in the emergence of AMR bacteria, it also shows that antibiotic usage on these farms is low and that the possible role these farmers play in AMR emergence may be overestimated.

  • Antimicrobial Resistance in Equine Reproduction

    • Simple SummaryBacteria can develop resistance to antibiotics, resulting in the appearance of infections that are difficult or impossible to treat. This ability enables bacteria to survive in hostile environments and can result from exposure to even small amounts of antibiotic substances. Bacteria are present in the reproductive tract of the horse; they can develop resistance to antibiotics, because the animal has been treated for an infection, or due to insemination with a semen dose that contains antibiotics. Bacteria colonize the membrane lining the male reproductive tract and are transferred to the semen during collection. They can cause sperm quality to deteriorate during storage or may cause an infection in the mare. Therefore, antibiotics are added to the semen dose, according to legislation. However, these antibiotics may contribute to the development of resistance. Current recommendations are that antibiotics should only be used to treat bacterial infections and where the sensitivity of the bacterium to the antibiotic has first been established. Therefore, adding antibiotics to semen extenders does not fit these recommendations. In this review, we examine the effects of bacteria in semen and in the inseminated mare, and possible alternatives to their use.Bacteria develop resistance to antibiotics following low-level "background " exposure to antimicrobial agents as well as from exposure at therapeutic levels during treatment for bacterial infections. In this review, we look specifically at antimicrobial resistance (AMR) in the equine reproductive tract and its possible origin, focusing particularly on antibiotics in semen extenders used in preparing semen doses for artificial insemination. Our review of the literature indicated that AMR in the equine uterus and vagina were reported worldwide in the last 20 years, in locations as diverse as Europe, India, and the United States. Bacteria colonizing the mucosa of the reproductive tract are transferred to semen during collection; further contamination of the semen may occur during processing, despite strict attention to hygiene at critical control points. These bacteria compete with spermatozoa for nutrients in the semen extender, producing metabolic byproducts and toxins that have a detrimental effect on sperm quality. Potential pathogens such as Klebsiella pneumoniae, Escherichia coli, and Pseudomonas aeruginosa may occasionally cause fertility issues in inseminated mares. Antibiotics are added during semen processing, according to legislation, to impede the growth of these microorganisms but may have a detrimental effect on sperm quality, depending on the antimicrobial agent and concentration used. However, this addition of antibiotics is counter to current recommendations on the prudent use of antibiotics, which recommend that antibiotics should be used only for therapeutic purposes and after establishing bacterial sensitivity. There is some evidence of resistance among bacteria found in semen samples. Potential alternatives to the addition of antibiotics are considered, especially physical removal separation of spermatozoa from bacteria. Suggestions for further research with colloid centrifugation are provided.

  • AFM1 Secretion and Efficacy of Novasilâ„¢ Clay in Kenyan Dairy Cows

    • The occurrence of aflatoxin M1 (AFM1) in milk has been widely reported in Kenya, with levels freqently exceeding national and international thresholds. Exposure to aflatoxin increases the risk of hepatic cancers and can also have other negative health impacts in children such as growth impairment and immunosuppression. Anti-mycotoxin agents (AMAs) included in contaminated feeds can greatly reduce the amount of AFM1 released in milk. A 45-day trial was designed to assess secretion of AFM1 in milk from individual cows fed commercial Kenyan dairy feed, as well as the efficacy of Novasil (TM) Plus in reducing the levels. A four-by-four Latin square cross-over design was used for the experiment. Four cows were fed on naturally contaminated with AFB1 feed, with levels ranging from 19 to 47 mu g/kg, and either no binder or inclusion of binder at the rate of 0.6 or 1.2%. Milk samples were collected each day and analyzed for AFM1. The results showed that AFM1 levels in the milk varied between the cows, even when fed similar levels of contaminated feed. On average, inclusion of 0.6% binder into the diet resulted in 34% decline in milk AFM1 levels, while 1.2% binder dose resulted in a decline of 45%. Significant reduction in AFM1 secretion was observed in all experimental units (p < 0.005), though only minimal reduction was recorded in one of the units (Cow 4) compared to the other three. This trial shows novel data on aflatoxin exposure and excretion in Kenyan dairy cows in a field setting where AFB1 level is uncontrolled. We demonstrate significant reduction in AFM1 secretion in milk using AMA, though AFM1 levels were still above the recommended EC standard of 50 LATIN SMALL LETTER ENGg/kg. This study suggests that AMAs alone cannot be relied on to reduce AFM1 in milk to safe levels. Training and good feeding practices are recommended in addition to use of AMAs.

    • With more than half of the global population living in cities, the urban areas are also teeming with animals, including peridomestic wildlife, pets, and livestock. Urban animals may carry zoonotic pathogens, and crowded conditions in cities can increase the risk for the human population. We used a systematic approach to screen two publication databases as well as gray literature, and quantified the studies conducted on zoonoses in urban animals with respect to the geographic distribution, the host animal and pathogens. Out of 876 references found, 93 were included into final data extraction. Few studies were from the rapidly expanding cities in low- and middle-income countries where urban livestock-keeping is far more prominent than in high-income countries. Most studies were performed in peridomestic wildlife and pets, less in livestock. The most common category of pathogens studied were gastrointestinal parasites followed by gastrointestinal bacteria, whereas studies on some other zoonoses internationally recognized as critical for public health were few or absent. In conclusion, to mitigate the risks of emergence of zoonoses from urban animals this review highlights the research gaps on zoonoses, particularly in livestock in rapidly growing tropical cities and a more comprehensive inclusion of pathogens prioritized by WHO and OIE.

    • Background: Antimicrobial resistance (AMR) has been identified as one of the major threats to global health, food security and development today. While there has been considerable attention about the use and misuse of antibiotics amongst human populations in both research and policy environments, there is no definitive estimate of the extent of misuse of antibiotics in the veterinary sector and its contribution to AMR in humans. In this study, we explored the drivers of irrational usage of verterinary antibiotics in the dairy farming sector in peri-urban India.Methods and materials: The study was conducted in the peri-urban belts of Ludhiana, Guwahati and Bangalore. A total of 54 interviews (formal and non-formal) were carried out across these three sites. Theme guides were developed to explore different drivers of veterinary antimicrobial use. Data was audio recorded and transcribed. Analysis of the coded data set was carried out using AtlasTi. Version 7. Themes emerged inductively from the set of codes.Results: Findings were presented based on concept of 'levels of analyses'. Emergent themes were categorised as individual, health systems, and policy level drivers. Low level of knowledge related to antibiotics among farmers, active informal service providers, direct marketing of drugs to the farmers and easily available antibiotics, dispensed without appropriate prescriptions contributed to easy access to antibiotics, and were identified to be the possible drivers contributing to the non-prescribed and self-administered use of antibiotics in the dairy farms.Conclusions: Smallholding dairy farmers operated within very small margins of profits. The paucity of formal veterinary services at the community level, coupled with easy availability of antibiotics and the need to ensure profits and minimise losses, promoted non-prescribed antibiotic consumption. It is essential that these local drivers of irrational antibiotic use are understood in order to develop interventions and policies that seek to reduce antibiotic misuse.

    • Background: Milk vendors play an important role in India's dairy value chain; however, their food safety practices are poorly understood. From the standpoint of milk safety, vendor behavior is significant because it has the potential to affect both consumer and producer behavior. This study investigates vendors' hygienic knowledge and practices toward milk safety, and antimicrobial resistance (AMR)Methods: A cross-sectional study was conducted in the states of Assam and Haryana (India). In selected villages, all the milk vendors identified at the time of the visit were interviewed. A questionnaire was used to assess the knowledge and practices on antibiotics, milk safety, and hygiene. The milk samples were tested for the presence of antibiotic-resistant bacteria using antibiotic susceptibility testing.Results: A total of 2444 milk vendors were interviewed during the survey. Out of these 156 (59.8%) of the vendors traded in raw milk while 40.2% traded in pasteurizes milk. Vendors were categorized depending on whom they supplied milk. Five categories were identified: (a) those who sold at grocery shops; (b) those who sold on the roadside; (c) those who sold from door to door; (d) those who sold to sweet makers/tea stalls; (e) those who sold from own home/other entity. The level of training among vendors on milk hygiene was non-existent and the knowledge related to antibiotics was low. Most of them (86.07%) agreed that boiled milk is always safer than raw milk but almost half  (48.77%) of them admitted that sometimes they drink milk without boiling it. Most vendors believed that they could identify whether milk is safe or not for consumption just by its appearance and smell. Out of 124 milk samples collected from surveyed milk vendors and tested for the presence of antibiotic-resistant bacteria, 80 (64.52%) tested positive.Conclusion: This study highlights the low levels of knowledge regarding food safety among milk vendors. It shows the predominance of informal milk businesses in the surveyed states and the prevalence of AMR bacteria in milk traded by them. Training may be a beneficial strategy for addressing the issue.

  • Microbial Contamination and Antibiotic Resistance in Marketed Food in Bangladesh : Current Situation and Possible Improvements

    • Antimicrobial resistance (AMR) is a public health problem worldwide. Bangladesh, like its neighboring countries, faces many public health challenges, including access to safe food, inadequate food surveillance, as well as increasing AMR. This study investigated bacterial contamination and the AMR profile of pathogens in marketed food in Bangladesh and explored barriers to reducing AMR in the country. We collected 366 tomatoes, 359 chicken and 249 fish samples from 732 vendors in traditional markets in urban, peri-urban and rural areas in Bangladesh, as well as from 121 modern retails in Dhaka capital to analyse Vibrio cholerae and Escherichia coli in fish, Salmonella in chicken, and Salmonella and E. coli in tomatoes. Antibiotic susceptibility against 11 antibiotics was tested using a disc diffusion test and interpreted by an automated zone inhibition reader. In addition, a qualitative study using key informant interviews was conducted to explore antimicrobial use and AMR reduction potential in Bangladesh. We found E. coli in 14.21% of tomatoes and 26.91% of fish samples, while 7.38% of tomatoes and 17.27% of chicken were positive for Salmonella, and 44.98% of fish were positive for Vibrio cholerae. In total 231/319 (72.4%) of all pathogens isolated were multidrug-resistant (MDR) (resistant to three or more antibiotic groups). Qualitative interviews revealed an inadequate surveillance system for antibiotic use and AMR in Bangladesh, especially in the agriculture sector. To be able to fully understand the human health risks from bacterial hazards in the food and the AMR situation in Bangladesh, a nationwide study with a one health approach should be conducted, within all sectors, including AMR testing as well as assessment of the antimicrobial use and its drivers.

    • BackgroundAntimicrobial resistance (AMR) is a major global public health issue. In India, access to medicines is poorly regulated and therefore antibiotics in dairy cattle are commonly used by farmers without consulting with veterinarians. This study was conducted to understand practices and knowledge related to antibiotic use and AMR among dairy farmers and veterinary professionals in selected urban and peri-urban areas of India.MethodsA total of 28 focus group discussions with farmers and 53 interviews with veterinary professionals were carried out.ResultsMastitiswas identified as the main animal health challenge. Antibiotic consultation behavior of farmers depended on the availability of veterinarians. Except in Bangalore, farmers were found to often treat animals on their own. They were found unaware of the concept of AMR, but knew the importance of vaccination. Veterinarians included in the study had a good understanding of antibiotics, AMR, and zoonotic diseases.ConclusionThe knowledge level and practices observed in the study related to the use/abuse of antibiotics can potentially increase the risk of development of AMR and its transfer in the community. Our findings can help support AMR – mitigation efforts in the country, including the design of better policies on antibiotic use in dairy.
